    On 03/21/2023, the complainant stated that on Monday 03/20/2023, the Infant 2 classroom was out of ratio from 8:39 am - 9:18 am when CCSM A was alone in the classroom with seven children. She stated that CCSM A acknowledged that she has frequently been alone with more than four children in the Infant 2 classroom. The complainant was only at the center on 03/20/2023. She did not speak to Ms. Quinn on 03/20/2023 because she was working in a classroom and there was no one working in the office. She spoke with Ms. Quinn on 03/21/2023 and she apologized and said that she would address the situation, but she did not say how the situation would be corrected. The complainant did not return Child A (1-year) to the center. On 03/24/2023 CCSM A stated that the Infant 2 classroom is frequently out of ratio. The children in the Infant 2 classroom are 1 year of age and the CCSM to child ratio is 1:4 with a maximum group size of 12. At least three or four times during the week she would have more than four children alone. This occurs mostly on Tuesdays and Thursdays. She will move the children to different classrooms to maintain the CCSM to child ratio whenever possible. Ms. Quinn, CCSM F, or CCSM E will come into the classroom which helps until additional children arrive. On 03/20/2023 from 8:15 am- 8:54 am, CCSM A was alone with five to eight children until CCSM F arrived at 8:54. From 8:56am- 9:09 am, CCSM A and CCSM F had nine to ten children. At 9:09 am, CCSM E arrived. At 9:32 am, CCSM F left the classroom. From 9:45 am- 10:00 am, CCSM A and CCSM E had nine children. CCSM A believes that three CCSM's called off on 03/20/2023 which caused the ratio issues. On 03/24/2023 CCSM B, floater, stated that she has worked in the Infant 2 and Toddler classrooms. This has been a difficult week due to staff calling off, parents dropping their children off early before their scheduled time and enrolling more children without having adequate staff to cover the classrooms. When possible, children are moved to other classrooms to maintain CCSM to child ratio when there 3 are not enough CCSM's to cover the classrooms. The toddler classroom is usually out of ratio with ten children and two CCSM's when she arrives at 10:00 am. She does not remember the number of children that were present on 03/20/2023 in the Infant 2 classroom when she arrived at 10:00 am. On 03/24/2023 I reviewed the center's face-to-name sheets for the Infant 2 and Toddler classrooms for the week of 03/20/2023. In the Infant 2 classroom, children are 1 year of age and the CCSM to child ratio is 1:4 with a maximum group size of 12. On 03/20/2023 From 8:15 am- 8:54 am CCSM A was alone with five to eight children. From 8:56 am- 9:09 am CCSM A and CCSM F were alone with nine to ten children. From 9:42 am- 10:00 am CCSM A and CCSM E were alone with nine children. An additional CCSM was required. On 03/21/2023 From 8:46 am- 9:30 am, CCSM A and CCSM B were alone with nine to eleven children. An additional CCSM was required. On 03/22/2023 From 9:14 am- 9:25 am, CCSM A and CCSM B were alone with nine children. From 10:21 am- 10:59 am, CCSM A and CCSM B were alone with nine children. From 11:11 am- 11:16 am, CCSM A and CCSM B were alone with nine children. An additional CCSM was required. On 03/23/2023 From 8:06 am- 8:10 am CCSM A was alone with nine children. An additional CCSM was required. During the short durations of times the classroom was out of ratio, the staff was waiting for an additional CCSM to arrive or someone was needed in another classroom. In the Toddler classroom, children are 1 2-year-olds and the CCSM to child ratio 1/2- is 1:4 with a maximum group size of 12. On 03/20/2023 From 7:37 am- 7:42 am CCSM C and CCSM E were alone with nine children. From 8:20 am- 8:37 am CCSM C and CCSM E were alone with nine to eleven children.     On 08/22/2022, I completed an unannounced onsite inspection. I spoke with Program Director, Susan Quinn. Ms. Quinn was made aware of the allegations and asked about an infant being in her office during a parent meeting. She confirmed there was an infant in her office who was fairly new to their facility. The infant was having a difficult time adjusting during naptimes, as she's a co-sleeper. She shared the infant was in her office the day Child A's mother came in to inquire about her child's injuries while on the field trip. The infant was inconsolable in the classroom and had cried so much that she lost her voice. Due to this, she wanted to provide assistance to the child care staff members (CCSM) who had unsuccessfully tried to console the child. Ms. Quinn shared she brought in toys and had the infant on her floor playing as she observed her. I inquired about the office being an approved child use space. She shared she's used her office as a sick space for children in the past, and in this case, although the infant wasn't sick, she was struggling in the classroom.     On 08/22/2022, I spoke with Child A's mother. She confirmed Child A is 6 years old and enrolled in the school-age summer camp program at the facility. She was made aware of the incident by Child A who contacted her via his smart watch at approximately 1p m. He was crying and shared he was bleeding and alone. He said he was left alone by the water area. When Child A shared this with her, she called the Hands-on Museum, and staff went to find Child A. She shared she immediately called Child A back and Ms. Daniella Young answered his watch. At this time the museum staff was with Ms. Young. Ms. Young shared she was in the same area as Child A and isn't sure why he said he was alone. Ms. Young shared she wasn't sitting down with Child A because she was checking on the other children. Child A's mother shared when she arrived to pick her son up, she was told by Ms. Sari that Child A had fallen while at the museum. It was at this time multiple children all said Child B pushed Child A. She later found out while in the office speaking with Ms. Quinn that Child B was the one who pushed Child A. Child A's mother shared while speaking with Ms. Quinn she was very dismissive in her responses and behavior towards her as a concerned mother. She was questioning why Child A had a smartwatch to call her. Child A's mother shared while she was at the door picking up her son, she observed Child B being aggressive towards another child and his mother. Ms. Sari said something to him. She expressed concerns regarding the age gap of the children in the camp program and that her son shouldn't be in the same age range group of older children, such as Child B. I asked if Child A has ever expressed safety concerns or had unexplained injuries prior to this incident. She denied any knowledge or concerns expressed by Child A. On 08/22/2022, I spoke with Ms. Quinn regarding the allegations. Ms. Quinn shared she was not present during the field trip. This was a trip for the school age summer camp program. I asked to view the attendance records for the field trip. I observed a total of 24 children and two CCSM's (Ms. Daniella Young and Ms. Kelly Sari). Ms. Quinn shared the incident was brought to her attention upon return to the child care facility. Within minutes Child A's mother arrived at the facility and immediately demanded to know what happened and wanted the information of the child who pushed Child A. Ms. Quinn shared she was told by Ms. Young that Child B pushed Child A out of an ambulance play exhibit. Ms. Young shared with her that she did not personally observe the incident, however, other children confirmed it was Child B. Ms. Quinn shared she tried to speak with Child B about the incident, but he would not answer her when asked if he know who pushed Child A. 4 I spoke with Ms. Daniella Young. She shared she did not observe Child B push Child A, only Child A being tearful and on the ground. She was told by other children that Child B pushed Child A off the stationary ambulance exhibit. Ms. Young treated Child A's knee (which had a small scrape and was bleeding) with the first aid kit brought from the facility in their emergency bag. She then gave comfort to Child A who appeared to be sad but was no longer tearful. She asked Child A if he wanted to join his peers in playing in the water area, but he didn't want to and wanted to just sit on the bench which was in the same area all in view. As she was standing doing observations of the other children, a staff member of the museum approached her and shared Child A's mother was on the phone and shared concerns that her child was bleeding and left alone. She informed the staff member of who Child A was as she was within close proximity of him. She was able to speak with Child A's mother via Child A's smartwatch when she called Child A back. At this time, the museum staff member was present and could hear her speaking with Child A's mother. She informed her that she had provided supervision of Child A and a

    On 08/22/2022, I arrived at the child care facility at 12pm. Lunch had just been served. I was able to observe the serving bins, and the food did not appear to be served in unsanitary conditions. In addition, all the food was covered with a foil or saran wrap covering. I spoke with Ms. Quinn about the allegations made against the facility regarding lunch preparation. She shared there has never been any concerns brought to her attention regarding unsanitary conditions of their kitchen or how they serve lunch. She shared Ms. Sari is the current lunch worker, and she has done an exceptional job to ensure lunch is out in a timely manner and she's never observed Ms. Sari to serve lunch uncovered or in filthy carts. I spoke with Ms. Sari who confirmed being the kitchen staff. She denied that lunch is ever served uncovered. She shared the food is only taken to each room when it's time for them to be served their lunch. She confirmed knowledge about food temperatures and shared an understanding of cleanliness of serving carts. I spoke with Ms. Young regarding this allegation. Ms. Young shared she been employed at the facility since January 2022, and she's never had any concerns pertaining to food sanitation. She confirmed all food received from the kitchen comes in a plastic bin which is clean, and she's never received food uncovered. I asked if any of the children complain about the temperature of the food. She denied this as well. 7 On 08/31/2022, I spoke with Child C's parent and inquired about any snack/lunch concerns or complaints made by Child C. Child C's parent denied having any concerns. On 08/31/2022, I spoke with Child E's parent and inquired about any complaints or concerns Child E has ever had regarding lunch. She denied any concerns. On 09/19/2022, I completed an exit conference with Ms. Quinn. I informed her of the citation regarding the use of an unapproved space and discussed ways and spaces she could accommodate ill children or assist in caring for children who are in distress in the approved spaces.

    On 0411912022, I interviewed Susan Quinn (program director) via telephone. Ms. Quinn stated that Ms. Sari typically works in the kitchen and drives the bus but occasionally works as a caregiver in the classrooms when they are short staffed. Ms. Quinn stated that Ms. Sari came into her office on 04/14/2022 at around 4:45pm. Ms. Sari was upset because had yelled at her regarding the way she interacted with Child A. Ms. Sari said that Child A was throwing a tantrum on the playground and trying to throw herself on the ground. When Ms. Sari tried to grab onto stop her from falling, she grabbed Child A's coat by accident. Ms. Sari said started to yell at her and told her what she was doing was inappropriaCthei*an o*sopp touching Child A. Ms. Quinn indicated that there have been 1 ongoing personal issues between and Ms. Sari and they do not work well together. M Ms. Quinn spoke with " regarding the incident. Ms. Quinn said that described the incident as Ms. Sari holding Child A up while she was trying to go wet noodle" during her tantrum. Ms. Quinn concluded that it was more of a disagreement between the coworkers and did not think the incident included Ms. Sari acting inappropriately with Child A. Ms. Quinn spoke with Child A's mother who stated that Child A often has tantrums and throws herself to the ground and was not concerned. After talking with Ms. Sari,-, and Child A's mother, Ms. Quinn did not feel the situation met the criteria for submitting an incident report to the department. Ms. Quinn stated that Ms. Sari is not "officially" banned from working in the infant classroom due to any safety concerns. Ms. Sari does not work in the infant classroom because she does not get along with a staff member whose child is in that classroom. I requested to view the video recording of the PreK playground at the time of the alleged incident. On 04120/2022 l conducted an unannounced on-site inspection We interviewed Sierra Washington (assistant preschool staff member), and Kelly Sari (cook/bus driver/floater). The interviews were privately conducted in the program director's office. Ms. Washington stated that she was on the playground at the time of the incident. Ms. Washington said that she saw Ms. Sari pulling Child A by the arm towards the playground structure area. Ms. Sari pulled Child A by the upper arm, and Ms. Washington described the pulling as "hard" and that Ms. Sari was kind of dragging Child A along as Child A pulled back and refused to move her feet. Ms. Washington then observed Ms. Sari grab Child A by the coat lapels and forcefully put her down onto the ground. When Ms. Sari forced Child A to the ground, she also sat down onto the ground. At that point, went up to Ms.. Sari and told her to stop what she was doing. Ms. Washington spoke with Child A and asked her if she was okay, in which Child A said that she was fine. Ms. Washington said that she had never seen Ms. Sari act like this before and said that she would describe her actions as inappropriate. Ms. Washington described Child A as a child that often has tantrums. Ms. Washington stated that typically she and other staff members allow Child A to have some space and calm down during these tantrums before they try to redirect her to another activity. Ms. Sari stated that on 04/14/2022 she had just got back from a bus run and was told by Ms. Quinn to help in the PreK classroom. While outside on the playground with the children, Ms. Sari observed Child A was taking the hat of another child. Ms. Sari asked Child A to return the hat, but Child A refused and laid on the ground with the hat tucked under her stomach. After asking again for the hat, Child A stood up, ran past Ms. Sari and threw the hat to the ground. Ms. Sari went after Child A and took her hand and walked Child A up the hill to the play structure area. Ms. Sari said that she was holding Child A's hand and using her other hand on Child A's back to guide her up the hill. Ms. Sari attempted to get Child A to apol

    On 04/05/2022 Program Director Susan Quinn emailed an incident report that stated that on 04/04/2022 Child A's mother came into the center around 12:50pm. Child A's mother said that she was watching the center's video streaming application. Child A's mother observed a caregiver drag Child A by the arm and throw her onto a cot. Child A then was seen running to the corner of the classroom and crying. On 04/05/2022 I spoke with Ms. Quinn and submitted a request for video recording of the toddler classroom at the time of the alleged incident. On 04/05/2022 I spoke with Child A's mother via telephone. Child A's mother stated that this was Child A's attending the center. Child A's mother indicated that Child A had previously only attended , so this was Child A's first time napping at the center. Child A's mother said tatss he had been watching the classroom video stream throughout the day. After lunch was served, a caregiver was seen putting the cots down for naptime around 12:30pm. Child A's mother saw Child A sitting in a chair at the lunch table, a caregiver grabbed Child A's left hand and then Child A threw herself back onto the floor. The caregiver lifted Child A by her left hand until she was completely off the floor. Child A appeared upset and looked like she was crying. The caregiver then aggressively carried Child A to the cot and dropped her onto the cot. Child A rolled off the cot and crawled to the corner and started crying. The caregiver walked off and left Child A in the corner to cry. Child A's mother stated that she and Child A's grandmother immediately drove to the center and picked Child A up. Child A's mother was not interested in talking with the program director at the time she picked Child A up and has since disenrolled Child A due to concerns for her safety. On 04/07/2022 I conducted an announced on-site inspection We watched the video recording of the incidentwith Susan Quinn programRreclo~r, and Tanya Zoro (district manager/licensee designee) in the program director office. The video was of the toddler classroom and was time stamped 04/04/2022 from 12:48pm-12:50pm. Ms. Zoro stated that when they reviewed the recording of naptime, this was the only time in the video that a caregiver is carrying Child A to the cot. The three caregivers in the classroom at the time of the video recording were Angie Hughes, Madison Brunson, and Brittany Turner. The video recording shows Ms. Brunson entering the classroom from the adjoining infant classroom. Ms. Brunson is cradling Child A in her left arm and is carrying a blanket in her right arm. Child A does not appear to be in distress but does look like 3 she is slipping down Ms. Brunson's side and is not being supported by Ms. Brunson's hip. Ms. Brunson carries Child A over to the cot and sits on the floor while still holding Child A in her arms. Ms. Brunson then places Child A onto the cot and sits next to Child A while patting her on the back. At no point in the video does it show Ms. Brunson picking Child A by the arm or dragging her by the arm. Child A does not crawl off the cot and does not appear to be in distress. We interviewed Susan Quinn (program director) and Angie Hughes (assistant toddler caregiver) The interviews were privately conducted in the program director office. Ms. Quinn stated that she has reached out to Child A's mother to discuss the incident but has not received a call back. Child A has not attended the center since the incident. Ms. Quinn said that she has never had concerns regarding the care Ms. Brunson provides and denied receiving complaints involving Ms. Brunson from staff members or parents. Ms. Quinn indicated that Ms. Brunson was shocked and upset when she heard about the allegations. Ms. Quinn said that the other caregivers often request for Ms. Brunson to be assigned to their classroom because she works so well with the children.     On 06/24/2021 I spoke with Child A's mother via telephone. Child A's mother stated that on 06/17/2021 she logged onto the WatchMeGrow application, which allows her access to Child A's classroom cameras. At approximately 3:54pm, Child A's mother observed Child A sitting in a chair adjacent to Ms. Scott. A few moments later, Child A gets up and approaches Ms. Scott. Child A's mother saw Ms. Scott take her hand and forcefully push Child A away from her 2 three times in a row. Child A's mother stated that she was so concerned about what she saw on the video that she immediately called the center and spoke with Ms. McClain, the program director. Child A's mother informed Ms. McClain about what she witnessed and asked for Child A to be promptly removed from the classroom. She told Ms. McClain that she was on the way to pick Child A up. Child A's mother and Child A's father arrived at the center at 4:35pm and immediately disenrolled Child A from the program due to their concerns regarding the incident. On 06/24/2021, I spoke with April Cole (district manager) and scheduled a meeting to view the video recording of the incident. The meeting was scheduled for 06/28/2021 at 10:00am at the center. On 06/28/2021, I conducted an announced on-site inspection to view the video recording of the incident. Both Ms. Cole and Ms. McClain were present during the viewing. We viewed the video on Ms. Cole's computer in the program director's office. The video was from the Pre-K classroom on 06/17/2021 and time stamped for 3:55pm. The video shows Shaunel Scott and Michaela Burnett present in the Pre-K classroom. Ms. Scott is seated by the multipurpose tables, and the children are getting up and finishing their snacks. Ms. Scott pulls a chair next to her and physically guides Child A to sit down in the seat. A few moments later, Child A gets up and comes towards Ms. Scott, who is still seated. Ms. Scott pushed Child A away from her two times; she does this by placing her hand on Child A's chest. Child A continues to come towards Ms. Scott, at which point Ms. Scott knees Child A in the chest resulting in him being pushed backwards again. Child A comes toward her a fourth time. The last time when Child A approaches Ms. Scott, she uses her hand and pushes Child A back forcefully, causing Child A to stumble backwards towards the table. I interviewed Senetra McClain, Shaunel Scott and Michaela Burnett. Interviews were privately conducted in the program director's office and in the staff break room. Ms. McClain stated that on 06/17/2021 she received a telephone call from Child A's mother. Child A's mother was upset and told her that she had observed Ms. Scott push Child A with a closed fist. Child A's mother requested that Child A be immediately removed from the classroom and indicated that she was on her way to pick up Child A. Ms. McClain removed Child A from the classroom and had him wait in her office for his mother to arrive. When Child A's mother arrived, she disenrolled Child A from the program. Ms. McClain denied past concerns with Ms. Scott and denied receiving any complaints regarding the way Ms. Scott interacts with the children. Ms. McClain provided me with copy of the center's discipline policy, which states that hitting, shaking, biting, pinching, pulling, pushing, physical force or inflicting any 3 other form of corporal punishment is prohibited. Ms. McClain confirmed that all staff have been trained on the center's discipline policy. Ms. McClain provided me with the face-to-name sheets for the Pre-K classroom for 06/17/2021. The documents show that Ms. Scott and Ms. Burnett were both working in the classroom and had 17 children in attendance. Ms. Scott stated that on 06/17/2021 she was working in the Pre-K classroom with Ms. Burnett. Ms. Scott typically works as an assistant caregiver in the preschool classroom, but she was assigned to the Pre-K classroom that day. Ms. Scott said that Child A was very active that day.     On 11/19/20, I received a phone call from the program director, Ms. McClain stating that a staff member had left a child unattended in the classroom for some time. She said that they were not sure as to the exact time that Child A was in there but will check the video and figure out the time. She said that she would send the incident report shortly. On 11/19/20, I received the incident report which stated that on 11/18/20, at about 7:50 am, staff member Ms. Marszalek took Child A and left her in an empty classroom. Child A was later discovered in the room by Ms. Walczak who reported the incident to the education manager, Ms. Campbell, who then communicated this to the program director, Ms. McClain via phone at 8:24 am. On 11/23/20, I conducted an unannounced onsite inspection at the center. I interviewed all the staff members that were involved in the incident. On 11/23/20, I interviewed Ms. Campbell who said that she and Ms. Marszalek were responsible for receiving the children at the front door, checking the temperatures of the children, and asking the COVID response questions to the parents. Ms. Marszalek was also taking care of the breakfast in the kitchen. Ms. Campbell said that they took turns bringing the children inside depending on each person's availability. At this time, the maintenance person came in to conduct some repairs and Ms. Campbell had to take care of that, thereby leaving the front lobby to Ms. Marszalek. Ms. Campbell said that at about 8:05 am, She heard Ms. Walczak yelling from the hallway stating that Child A was left alone in the classroom. Ms. Campbell went to the classroom and saw Child A standing inside the preschool room and the classroom was empty. Ms. Walczak explained to her that Child A was standing at the closed door and peeking outside the glass door panel and she came out of the school age classroom to see her. She had immediately brought that to Ms. Campbell's attention. Ms. Campbell said that she called the program director, Ms. McClain right away and informed her about the incident. She said that she was not sure as to how long Child A was left in the empty classroom. She was not even sure as to how the incident report stated 7:50 am. On 11/23/20, I checked the attendance records for 11/18/20. Child A was checked in at 8:07 am. This check in time was recorded by Ms. Langford who was in the combined pre-k classroom after Child A was brought to her classroom after the incident. LANSING, MICHIGAN 48909 www.michigan.gov/lara . 517-335-1980 On 11/23/20, I interviewed Ms. Walczak who stated that on 11/8/20, she was in the school age classroom that was opposite to the preschool classroom where Child A was found. She said that she came outside the classroom and wanted to use the restroom. The preschool classroom door was closed, but she saw Child A looking outside the narrow glass door panel to see if something was happening. She looked scared. Ms. Walczak said that she checked in the pre-k room through the window and Ms. Padina (preschool lead caregiver) was still in there, which meant that the preschool children were still combined with the pre-k children. Child A was not wearing her jacket. Ms. Walczak realized that Child A was alone in the classroom. She immediately called out to Ms. Campbell to come and take care of the situation. Ms. Walczak said that Child A was in her classroom the previous year and is a very independent. She also knows that she was not supposed to open the door by herself until a staff member is present. Ms. Walczak assumed that Child A probably hung her jacket, left her things in the cubby, and stayed in the classroom. Ms. Walczak said that it was probably between 8:05 am and 8:15 am that she saw Child A at the door. She said that she took the school agers to her classroom at about 7:52 am and she would have seen Child A at the time if she was by the

    On 9/30/20, the department received a voicemail from Ms. Cole stating that Child A's mother complained to her that there was an unknown caregiver that smacked Child A on his cheek near a wall. On 9/30/20, the department received an incident report stating that Child A's mother told Ms. Cole on 9/29/20 her son told her that on 9/25/20, he was smacked on his cheek by a staff member. Child A's mother did not know the name of the staff member or the time when the incident occurred. On 10/7/20, I interviewed Child A's mother who said that on 9/25/20, Child A's cousin went to the center at about 6:30 pm to pick up Child A. As they were driving out, Child A told Child A's cousin that a staff member had smacked him on the cheek. Child A's mother said that Child A did not know the name of the staff member or the time that it occurred. She said that Child A has been diagnosed with Autism and he is very sensitive to touch and therefore remembers when somebody touches him. Child A's mother said that she knew for sure that a staff member had hit him. Child A's mother said that Child A's caregiver, Ms. Langford was very good to him and she knows to manage him, but it could be somebody else. Child A's mother indicated that this was his first week at the center and he attends part time on Mondays, Wednesdays, and Fridays only. Child A's mother said that she called the manager, Ms. Cole and reported this to her and she said that she would further investigate the matter, but it is hard without the name or when it happened. On 10/9/20, I interviewed Child A's cousin, who said that after she picked up Child A from the center on 9/25/20, Child A told her that his "teacher" was mean. When Child A's cousin asked him further, he said, "she hit me" and mentioned a name, too. Child A also said that a "a big lady" hit him on the face. Child A's cousin said that she could not remember the name that he mentioned. As soon as she heard that she turned around and went back to the center, but the building was closed. She could not talk to anybody and she told Child A's mother about it. On 10/19/20, I conducted an unannounced onsite inspection at the center. I conducted interviews with all the staff members involved with Child A on 9/25/20. I could not review the attendance records in Child A's classroom since there were no available records of attendance for the children for 9/25/20. There was no record of the attendance records on 9/25/20 for the staff members checking in and out of the classroom with Child A. 611 W. OTTAWA. P.O. BOX 30664. LANSING, MICHIGAN 48909 www.michigan.gov/lara . 517-335-1980 On 10/19/20, I interviewed Education Manager, Ms. Campbell who said that she got a call from the regional manager, Ms. Cole asking about an incident where Child A was smacked on his cheek on 9/25/20. Ms. Campbell said that Child A could never have said that since he could not speak in sentences. Ms. Campbell said that he was enrolled on a part time basis and attended the center only 3 times a week. She said that she was present at the center on 9/25/20 and she did not hear any complaints about anybody smacking any child. She said that Child A's mother was very upset and angry since she was asked to pay late fee for all the 3 days that Child A was picked up late from the center. Child A's mother was especially angry when Ms. Campbell asked her on the phone to pay the tuition that was due along with the late fees on 9/25/20. Ms. Campbell said she asked the staff members about the incident and none of them knew anything about smacking any child. She did not believe that Child A could have made a complaint about smacking. On 10/19/20, I interviewed Ms. Smith who said that she worked with Child A on 9/25/20. She said that it was very difficult to manage Child A since he does not listen, hits other children and staff members too. On 9/25/20, he hit her twice and screamed hard if he did not get his way. She said that Ms. Langford worked with her through the day on 9/25
