On 5/11/23, Child A (5 years, male) was playing on the monkey bars and fell onto the surfacing. The child care staff contacted his parents, and he was picked up from child care. Child A was not taken to the doctor until 5/13/23 when he was complaining that the arm still hurt. He was diagnosed with a broken left arm.

On 5/16/23, I interviewed Brianna Schroer, child care staff member. On Thursday last week she was on the black top assisting with a child when she observed two children walking with Child A (male, 5 years) who was crying. Child A was holding his arm and upset. She took him inside the building while Jonah Przepiora, child care staff member stayed outside with the other children. Once inside she and Ms. Woodfin tried to contact parents and make Child A comfortable with ice. Child A was upset and did not state what happened. She was told by the other children that he was trying to hang upside down and fell. He broke his left wrist. She did not see it happen. She is not aware if new surfacing has been put on the playground this year. She did not think it looks like surfacing had been added to the playground. I interviewed Madison Woodfin, child care staff member. She was not on the playground when Child A was injured. She was inside with a child. Ms. Schroer brought Child A inside and he was upset and crying extremely hard. Ms. Schroer went to get ice for his arm, and she took Child A to sit down and got a pillow to put his arm on in hopes that it will help him keep his arm still and help with the pain. They put ice on it and contacted his mother, father, and grandfather. None of them answered the phone. A few minutes later Child A's Mother came to the center and picked him up. They had grandpa on the phone when she arrived. She asked Child A if he wanted to go home. They notified the program director, Mindy Ross, and requested that she contact Child A's parents to see if Child A was taken to the doctor. Ms. Ross contacted Child A's Mother. She told Ms. Ross that there was not swelling or bruising on his arm, and Child A was doing fine. They would not be taking him to the doctor. Ms. Woodfin understood that Child A was taken to the doctor on Saturday and was diagnosed with a broken left wrist. Jonah Przepiora, child care staff member was interviewed. He was out on the playground with the children and Ms. Schroer during the time of the accident. He did 3 not see what happened to Child A. He was with other children in the open grassy area. He just was informed that Child A broke his wrist when he fell off the monkey bars. I conducted an on-site inspection and interviewed Mindy Ross, program director. Ms. Ross received an invoice from the maintenance person and the last time surfacing was put on the playground was in 2018. She understands that surfacing is needed and will inform maintenance. She will not allow the children in her program to play on the playground until surfacing is placed on the playground. She was informed of Child A going to the doctor when she contacted parents on Saturday evening. She talked to them Thursday evening after the child care staff notified her of the accident. Child A's mom stated that there was no swelling or bruising and Child A calmed down after he was home, so they decided not to take him. On Friday afternoon she contacted Child A's Father, and he stated that if Child A said his arm still hurt, he was going to take him later that evening. She became aware that he was taken to the doctor on Saturday when she contacted the parents to see how he was doing. Child A's Father told her that Child A had a broken wrist. I interviewed Child A's Mother. Child A's Father took Child A to the doctor on Saturday, 5/13/23, because Child A was complaining that his arm hurt. A soft cast was put on and they will see an orthopedic surgeon tomorrow. She had no concerns about how he was hurt or how the staff managed it. They called everyone on the child information card. They were talking to his grandpa when she arrived to pick up Child A.
