On April 17, 2026, Licensee Designee, Alexandra Humerickhouse, reported to the department allegations of inappropriate contact which were provided to her on April 16, 2026. Alexandra Humerickhouse was in compliance with the 24 hour requirement to report allegations to the department.

Jeannie Ellis, licensing consultant and I conducted an on-site inspection on 07/12/2022. We interviewed Taylor Westcott, program director. I informed her of the allegations and Ms. Westcott informed me that they have video on-site. Ms. Westcott said that the dates that Child A was present in the center were 6/22/2022, 6/24/2022, and 06/29/2022. Ashley Antone is the infant lead caregiver and never leaves the infant room. Ms. Westcott said that Sara Rood worked on 06/22/2022 from 6:30-12:00pm, 06/24/2022 she did not work, and on 06/29/2022 from 6:30-12pm. The schedules for the staff were provided by Ms. Westcott. Ms. Ellis and I reviewed video for the alleged dates of the incident. We reviewed video from the day of 06/22/2022 and 06/29/2022. Child A arrived at 9:16 am on 06/22/2022 and left the center at 3:21 pm. We did not see Child A getting slapped by any staff member or by any other child from the center. We reviewed the entire time 2 Child A was at the center and the staffs interactions with Child A and the other children at the center were appropriate. We reviewed video from 06/29/2022. Child A arrived at 9:41 am and left the center at 3:24 pm. We reviewed the video and did not witness Child A getting slapped by any staff or any other children. Ms. Ellis and I interviewed Sara Rood, child care staff member. Ms. Rood said that she has a good relationship with Child A. She did not slap Child A. She is related to Child A's family. Ms. Rood confirmed the dates that she was present at the center and that she was not working on one of the days in question. The date that she was not present at the center was 06/24/2022. Child A was transitioning well into the center and she plays mostly with her cousin who attends the center. Ms. Ellis and I interviewed Ashley Antone, child care staff member. Ms. Antone said that she works in the infant room as the lead caregiver. She really did not know who Child A was as she does not work with the children in the other classrooms. She did not have any concerns with other child care staff members working at the center. She is usually in the infant room between 6:30 am to 8:30 am as this is the time the first infants arrive. Ms. Ellis and I verified that Ms. Antone did not work with the other children via video and the center provided us information on when the first infant checked in during the morning. On 07/13/2022, Ms. Ellis and I were onsite to view video of 06/24/2022. We reviewed the video and Child A was not slapped by any staff member or child. Child A arrived at the center at 9:24 am and left at 3:31 pm. Ms. Ellis and I interviewed Alexandra Humerickhouse, licensee designee. Ms. Humerickhouse said that she viewed the video and did not see Child A getting slapped by any of her childcare staff members. She did meet with Child A's parents, and they informed her of the allegations. The dates that were provided to her were 06/22/2022, 06/24/2022 and 06/29/2022. Ms. Humerickhouse confirmed that on 06/24/2022 Ms. Rood was not present. During the meeting on 07/01/2022 with Child's A parents, Child A's father did most of the talking. Child A's parents showed Child A pictures of staff at their home and Child A pointed at Ms. Rood and Ms. Antone. Then Child A said that the staff member had glasses. Ms. Humerickhouse talked with Ms. Rood who said nothing happened and that she did not slap Child A. Ms. Humerickhouse said she talked with Ms. Antone and she did not know who the child was as she works in the infant room. Ms. Ellis and I interviewed Isabella Nelkie, child care staff member. She floats in different areas of the center. She did not have any concerns about Child A and Child A was quiet child. Ms. Nelkie explained that Child A is spends a lot of her time with her cousin during the day. 3 Ms. Ellis and I interviewed Paige Northrup-Nickell, child care staff member. Child A does not like taking naps. Ms. Northrup-Nickell does not have any concerns with the center or with any of the staff membe
