On 03/22/23, I made an unannounced onsite inspection to the center. I interviewed Stacey Horstman, program director and licensee designee. Ms. Horstman said that Child Care Staff Member 2 (CCSM 2) called her at 10:05 AM on 03/17/2023 to tell her that CCSM 4 found Child A in the hallway outside the toddler classroom door. CCSM 1 had just taken children to the bathroom in the hallway. When she came back into the classroom Child A was not with her. The other child care staff members in the classroom did not notice that Child A did not come back in. Ms. Horstman reviewed the video recording and at 10:05:28 CCSM 4 knocked on the classroom door and told CCSM 2 that she had found Child A standing in the hallway waiting outside the door. Child A was in the hallway unattended from 10:04:09 AM to 10:05:28 AM. CCSM 3 called Child A's Mother at nap time. Child A's Mother took the news very well and understands that things happen. She said she would call back if she had other questions. She did call back later and asked if Child A could be moved to the other primary caregiver. CCSM 2 was assigned as Child A's primary caregiver. Ms. Horstman met with the child care staff members that afternoon to talk about what happened. Ms. Horstman provided coaching to the child care staff members on how to prevent this from ever happening again. CCSM 1 said that she was so focused on changing one of the other children's pants that she forgot to count the children. Since reporting the incident, Ms. Horstman gave each CCSM in the classroom a corrective action in their file. She assigned CCSM 1 four training hours from Childcare Education Institute (CCEI) that includes CUR120 Trouble-Free Transitions that Teach, CCE1900 Safety in the Infant/Toddler Classroom, and CCE111 04 Indoor Safety in the Early Childhood Classroom. All CCSMs in the classroom must complete the Active Supervision webinar through Early Childhood Learning and Knowledge Center (ECKLC) and a self-reflection. CCSM 2 and CCSM 3 created a transition chart to use for taking children to the bathroom. There's a pocket chart with children's names and pictures. When CCSMs take children to the bathroom they take the picture card out of the pocket. Name to face is done when they leave the classroom, when they enter the bathroom, when they leave the bathroom, and when they return to the classroom. The lead and assistant CCSMs created a training document specifically structured for their classroom. The document includes how to complete name to face correctly. This training will be part of orientation for new child CCSMs. I brainstormed with Ms. Horstman different options for taking children to the bathroom safely. I viewed the video recording of the incident. The video shows CCSM 1 entering the classroom with three children at 10:04:09 AM. CCSM 4 knocks on the door at 10:05:28 AM. It was approximately 1 minute 19 seconds between when CCSM 1 entered the classroom and the knock on the classroom door. The child was unharmed and was found waiting outside the classroom door. She did not appear visibly upset. I interviewed CCSM 1, acting assistant CCSM. CCSM 1 has been employed by the center for 7 years. She said that it was a very "hectic" morning in the classroom that 3 day. The "teachers" had some projects they were trying to get done and it was one of the busier days with children. CCSM 1 isn't used to taking four children to the bathroom at the same time. She has only taken them a handful of times before. When she took the four children to the bathroom, she changed all their diapers. As soon as she got done changing all their diapers, Child A soiled her diaper again. CCSM 1 went back to the changing table, cleaned her up and changed her diaper. CCSM 2 opened the door and said that another child had to use the bathroom. She asked if she could take one of the children who was done with the bathroom back to the classroom and leave her with Child B. Child B went to the bathroom by herself. CCSM 1 got Child

On 03/30/2021, I received a voice message from Stacey Horstman, program director/licensee designee, explaining that Child A left the classroom and was outside unattended for about four minutes. I received a written report regarding the incident the same day. On 04/01/2021, I completed an unscheduled on-site inspection. I first spoke with Ms. Horstman. She said on 03/30/2021, after she left for the day, she received a telephone call at about 3:58 p.m., from Ms. Katie Anderson, child care staff member. Ms. Anderson said, "You're going to have to call licensing, I just found Child A outside by himself." Ms. Horstman said she immediately returned to the center. She watched the video surveillance and met with Ms. Zuhrahon Kalandarova, Ms. Delaney Bond and Ms. Hailey Woodworth, the child care staff members working in the classroom at the time of the incident. At the time of the incident, the classroom was transitioning to go outside. The child care staff members were assisting the children with their shoes, coats and, due to the UV index for the day, applying sunscreen. During the transition, there were two children on the ramp that leads to the exterior classroom door. While Ms. Woodworth, was guiding the two children off the ramp, Child A walked past Ms. Woodworth up the ramp. Ms. Woodworth did not see Child A walk past her. Ms. Horstman explained only part of the ramp is visible from the camera, after a certain distance, part of the ramp and the exterior door Child A went out of are not visible on camera. There are two exterior doors directly next to each other. The door next to the one Child A exited can be seen by the camera. Ms. Horstman said when she watched the recording, she could see a slight change of lighting, that appeared to be outdoor light, in the area where the door is located. She is "guessing" that is when Child A left the classroom. At that time, the child care staff members were still assisting the children, preparing for outdoor time. Approximately four minutes later, Ms. Bond ran up the ramp towards the door and appeared back in camera with Child A in her arms. Ms. Horstman said this has never happened before and she held each child care staff member responsible for the incident and developed a correction action and training plan for each child care staff member. In addition, stop signs and a cow bell were added to the door and a door chime was ordered. She explained that due to COVID-19, the exterior door was used as an arrival and departure location for the parents and suspects Child A associated that door with "mom and dad." Due to this, the exterior door will no longer be used for arrival and departure; parents will be required to use the main entrance. Ms. Horstman said she was very "transparent" with Child A's parents about what happened, and Child A is still enrolled at the center. I then watched the video surveillance. The recording does not have sound. The timestamp on the recording is approximately 14 minutes behind "real time." I started 3 watching the video at the time stamp of 3:32 p.m. There were eight children and three child care staff members in the classroom. At that time, Ms. Kalandarova, Ms. Bond and Ms. Woodworth were all actively assisting the children with shoes, coats, and sunscreen as the class was preparing to go outside. Ms. Kalandarova was sitting on the floor assisting a child on the south side of the classroom. Ms. Bond was sitting on the floor assisting children on the north side of the classroom and Ms. Woodworth was standing assisting children on the east side of the classroom. At approximately 3:33 p.m. Ms. Woodworth walked up the ramp to assist two other children who were on the ramp. There is gate at the bottom of the ramp, but the gate was not closed. As Ms. Woodworth was guiding the children off the ramp, the two children were on her right, she looked down at the children and guided them forward with her hands.
