On 10/25/2022 1 arrived at the center and was greeted by program director Ms. Sarah Schumacher. She was aware of Child A being left unsupervised in the classroom as she had notified licensing by telephone and submitted an incident report on 10/24/2022. Ms. Schumacher and I exited the foyer and entered the conference room. Ms. Schumacher stated Child A was left unsupervised in the classroom when child care staff members and the children in care exited the classroom to play outside. Child A was unsupervised for about two minutes and was found in the classroom bathroom. Child care staff Ms. Olivia Bryant called her on the center telephone, informing her of the incident. Ms. Schumacher talked with staff to determine what happened. Staff did not follow corporate's management procedures by completing a head count with name to face taking eleven children outside and leaving Child A in the bathroom. Ms. Schumacher provided a copy of corporate's policies and said staff received written warnings. The expected behavior is to conduct a name to face every 30 minutes and at each transition to ensure safety and constant supervision. Ms. Schumacher provided copies of the warning notices. On 10/25/2022, child care staff member Ms. Ileisha Rodriguez-Melendez stated she was pretty sure eleven children were present in care on 10/24/2022. Ten children went outside with Child A being left in the classroom alone. Ms. Rodriguez-Melendez said children and staff were surrounding the door and not in any line. Staff did not count children before exiting the classroom or when they were outside. She trusted that they had all of them. She recalled Child A being near the door but must have wandered off without being seen. Within five minutes child care staff member Ms. Clarisse Pendo brought Child A outside to rejoin the group. On 10/25/2022, child care staff member Ms. Clarisse Pendo stated on 10/24/2022 she was walking by the bathroom that is adjoining to the Lyric classroom and she observed Child A sitting on the floor and playing with the diapers. She knew the staff and children from the Lyric classroom were outside playing. She immediately took Child A to the playground and informed Lyric staff that she found Child A alone in the bathroom. Ms. Pendo is unsure of the amount of time Child A was unsupervised in 3 the bathroom as she works in the Pedal classroom. Ms. Pendo said she had Ms. Bryant call Ms. Schumacher on the center's telephone to inform her of the incident. On 10/25/2022, prior to exiting the center, I provided consultation to Ms. Schumacher and discussed teaching children how to line up. There are numerous ways to accomplish this which will assist child care staff to complete a head count with name to face accurately; preventing children from being left in an area unsupervised. Ms. Schumacher checked attendance for 10/24/2022 confirming twelve children were present in the Lyric classroom when Child A was left unsupervised. On 12/13/2022, Child A's Mother stated she received a telephone call on 10/24/2022 from program director Ms. Schumacher informing her Child A was left unsupervised in the classroom when the class went outside to play. Ms. Schumacher said Child A was fine and reunited with her group within a minute or so. Child A has been at the center since July 2021 and she is pleased with the care child receives. She has absolutely no concerns regarding staff or the care Child A receives. On 12/13/2022 ,Child C's Mother stated she is one thousand percent comfortable with the level of care Child C (male, 20 months) receives at the center. Child C has been enrolled since May 2022 and is absolutely comfortable with child care staff and office staff. She has no concerns at all. On 12/13/2022, child care staff member Ms. Tatum Weiss stated twelve children were present on 10/24/2022. She was near the kitchen area in the classroom and cleaning up crayons and papers. Children were lined up at the door to go outside. She quickly went to the door

On 9/15/2022 I arrived at the center and informed Ms. Sarah Schumacher of the allegation. Ms. Schumacher was expecting to see me as she contacted me via telephone within three hours of the incident and submitted an incident report on 09/13/2022. Ms. Sarah Schumacher stated that on 09/12/2022 during rest time, Child A swallowed her barrette. Child care staff immediately called her in the office and she contacted Child A's Mother. Child A's Mother contacted the nurse at the pediatrician's office for guidance. Child A's Mother was instructed to take Child A for an x-ray to ensure the barrette was inside and working its way out. The physician recommended Child A's Mother wait for Child A to pass the barrette through her stool. Child A returned to care the following day. While in care, Child A passed the barrette within 50 hours of swallowing it. On 09/15/2022, child care staff member Ms. Laurie Mercado stated Child A does not normally sleep. Children are staggered in the classroom with child care staff spread out to supervise the children. Ms. Mercado instructed Child A to be safe on her cot and lay or sit. Some children were just falling asleep when Child A started crying. She was face down on her cot. She asked her what was wrong and Child A said she 2 swallowed it. Her language was not real clear so she asked her again what she swallowed. Child A said her hair clip. Ms. Mercado said she took care of her while child care staff member Ms. Katy Kohley looked for the hair clip in the classroom. Child A has thin fine hair and the clip slides out easily. She has put it back in her hair on previous occasions. Ms. Mercado asked Child A to open her mouth and she did not see it. She assessed Child A and noted her breathing was fine. She took Child A to Ms. Schumacher in the front entry. Ms. Schumacher checked her and called Child A's Mother. Child A's Mother picked her up and took her for an x-ray. Child A returned to care on 09/13/2022. Staff were instructed to have her drink a lot of water and save any stools she has. Child A's Mother would take the stools home to watch for the barrette. On 09/14/2022 when she arrived for work, she was happy to hear Child A had passed the barrette. On 09/15/2022, child care staff member Ms. Katy Kohley stated she was providing breaks to child care staff on 09/12/2022. She was present in the classroom when Child A swallowed the barrette. Child A was not asleep and was playing with her barrette. Ms. Kohley said she put it back in her hair once and told her to leave it in her hair. Child A will sometimes put toys in her mouth; however, it is not excessive and she did not anticipate Child A would put the barrette in her mouth or swallow it. Ms. Mercado took Child A down to the office immediately. On 09/15/2022, child care staff member Ms. Rachel Harris stated she was on break and not present in the classroom when Child A swallowed her barrette. Child A normally has her hair done and sometimes wears barrettes. Ms. Harris said she was shocked that she swallowed her barrette. When she exited the classroom for her break Child A was laying and relaxing on the bad. When Child A does not sleep, she generally plays with her toy octopus that she brings from home or will look at books. On 09/19/2022, Child A's Mother stated she received a telephone call from Ms. Schumacher on 09/12/2022 informing her Child A swallowed her barrette during rest time. She picked up Child A and took her for an x-ray. Child A passed the barrette in her stool two days later. Child A's Mother is unsure how many times Child A played with the barrette and would have preferred child care staff had taken it away from her. Child A's Mother has no other concerns with the care provided to Child A.
