On 03/17/2023, it was reported to the department that earlier that day three children walked out of the gymnasium unsupervised. The children were observed on the center's video camera system and returned to the gymnasium. A written report of the incident was also submitted the same day. Child Care Staff Member 1 stated that on 03/17/2023, she was alone in the gymnasium with eight 4-year-old children. One of the children, it was either Child A, Child B, or Child C, had to use the restroom. Child Care Staff Member 1 called up to the center's office to request an additional child care staff member come to the gymnasium so that she could take the child to the restroom. Child Care Staff Member 1 spoke to Child Care Staff Member 2 in the office. Child Care Staff Member 2 stated that she would send a child care staff member down to assist. As soon as Child Care Staff Member 1 ended the phone call, Child Care Staff Member 1 observed Child A, Child B, and Child C walk out of the gymnasium unsupervised. She didn't want to leave the remaining five children that were playing in the gymnasium unsupervised to go get the three children that walked out of the gymnasium, so she tried to call up to the office. Child Care Staff Member 1 was trying to move quickly and because of that couldn't get the call to go through. She stated that she was new to the center and has not mastered the phone system yet. Child Care Staff Member 1 stated that a minute or two after the children left the gymnasium, Child Care Staff Member 2 brought the three children back down to the gymnasium. An additional child care staff member came down to the gymnasium to assist Child Care Staff Member 1. Child Care Staff Member 2 stated that on 03/17/2023, she received a phone call from Child Care Staff Member 1 indicating that she had a child who needed to use the restroom. Child Care Staff Member 2 informed Child Care Staff Member 1 that she would send a child care staff member down to assist. When Child Care Staff Member 2 got off the phone, she noticed three children walking up the stairs on the video monitor on her desk. They did not appear to have a child care staff member with them. Child Care Staff Member 2 arose from her desk and walked to the stairs to intercept the children. She promptly brought them back downstairs to the gymnasium where Child Care Staff Member 1 was with the remaining children. Child Care Staff Member 1 indicated to Child Care Staff Member 2 that she saw the children walk out of the gym after they ended their phone call, so she tried to call her back but couldn't get through. Child Care Staff Member 2 added that as this incident was happening, an additional child care staff member was on their way down to assist with the child who needed to use the restroom. 3 On 03/22/2023, I completed an unannounced onsite inspection at the center. I viewed the gymnasium area and the staircase where the children were observed. The gymnasium is located on the lower level of the building. There is a staircase that leads to the main level of the building. The video camera that Child Care Staff Member 2 viewed in the office shows the staircase leading to the main level. There is also a door that leads outside on the landing where the staircase is located. The camera is positioned so the door is visible. Based on Child Care Staff Member 1 and 2's recounting of the incident Child A, Child B, and Child C did not attempt to leave the center through the door while unsupervised. They walked up the stairs from the gymnasium where they were intercepted. Child A's Mother, Child B's Father, and Child C's Mother all stated that they were informed of the incident right away. They had no concerns regarding the care and supervision that the center child care staff members provide. I spoke to Laurie Linscott, licensee designee regarding the incident.
