On 04/05/22, I made an unannounced onsite inspection at Ms. Amy Morrish's child care group home. There were 12 children with 2 staff, Ms. Morrish and Cheryl Lorts. Ms. Morrish stated Ms. Lorts was helping today due to her staff member calling last night and stating she could not work in the morning. Ms. Morrish stated she has never had 20 children, let alone 30 children at one time at her home. She stated she only has 15 children enrolled and they are not all full time. Ms. Morrish provided me with a copy of her attendance sheets and a list of children enrolled. The list had 15 children's names on it and 12 children were signed in at this time. Ms. Morrish then provided me with a copy of the attendance sheets for three separate weeks: 03/14/22 - 03/18/22 Monday: 11 children total for the day Tuesday: 12 children total for the day Wednesday: 10 children total for the day Thursday: 11 children total for the day Friday: 10 children total for the day 03/21/22 — 03/25/22 Monday: 9 children total for the day Tuesday: 12 children total for the day Wednesday: 12 children total for the day Thursday: 12 children total for the day Friday: zero children due to being closed 03/28/22 — 04/01/22 Monday: 5 children total for the day Tuesday: 7 children total for the day Wednesday: 6 children total for the day Thursday: 6 children total for the day Friday: 6 children total for the day Ms. Morrish sated the week of 03/28/22- 04/01/22 was the week of spring break and she did not have many children due to this. She stated during this week, she interviewed and had an assistant caregiver come in for one day for a trial day. The staff member worked from 9 AM — 2 PM and then left for lunch and never returned or called. Ms. Morrish stated while the caregiver was there, she asked a lot of questions as she had come from a center and did not understand how children of all ages can be in the same room. Ms. Morrish stated she provided the caregiver with a child care home licensing book to review so she could see the differences in rules. 3 On 04/20/22, I spoke to Child A's mother via phone, and she stated that Child A has been attending Ms. Morrish's child care home since Child A was born, and she is almost five years old and attends Monday through Friday. Child A's mother stated she has never had concerns with too many children at Ms. Morrish's home. She stated she does do pick up and drop off in the living space at the top of the stairs and does not go into the basement and count children. Also, whenever she brings in birthday treats or snacks for special dates, Ms. Morrish always has her bring in 12 for the children. She stated she has never been asked to make treats for more children. On 04/20/22, I then spoke to Child D and E's mother via phone, and she stated she has never had concerns of too many children at Ms. Morrish's home. She stated that there is always two staff members at the home and that she does not exceed 12 children. Child D and E's mother stated there have been times when she knows children could not come because there were already 12 children. On 04/20/22, I spoke to Sarah Kilgore, assistant caregiver for Ms. Morrish's home. She stated she has worked there since February 2022 and works Monday through Friday all day. Ms. Kilgore stated that there is between 10 to 12 children at the home each day. She denied ever having more than 12 children, even if school age children are there. Ms. Kilgore stated that during spring break, they had much less children each day. She stated that she did not work full time the week of spring break due to not many children and getting a break herself. Ms. Kilgore stated that she does not have any concerns of Ms. Morrish having more children than her capacity is allowed. She stated that they are very strict on making sure they are always in ratio and capacity each day. On 04/21/22, I spoke to previous assistant caregiver Amy Leydig who stated she worked with Ms. Morrish from June 2021 until March 24, 2022.

During my onsite inspection at Ms. Morrish's home, I observed Ms. Morrish and Ms. Lorts with the children. Ms. Morrish stated that at no time are the children left unattended for 10 to 20 minutes. She stated that if a parent comes to the door, they will meet her at the top of the stairs to either drop off or pick up their child. Ms. Morrish stated this is always a quick transition, and when she goes up there is still a caregiver downstairs with the children. At the end of the day and its only Ms. Morrish, she stated all the children will come upstairs for each pick up. Ms. Morrish stated when the older children go outside and the younger ones are still inside, one caregiver will be outside, and one will stay inside. This is to make sure that all children are always supervised. She stated if a child needs to come in for the bathroom or any reason, the outside caregiver can yell to Ms. Morrish, and Ms. Morrish will meet at the top of the steps to get the child. No children are ever alone while in her care. On 04/20/22, I spoke to Child A's mother via phone, and she stated she has never had concerns with supervision. She stated that she will meet Ms. Morrish at the top of the steps in the living room for drop off and pick up. She stated that if Ms. Morrish is alone, the other children will come up with her; however, there is mostly someone else there and they stay with the children in the basement. Child A's mother stated that she feels if the children were not supervised well, then her child would be injured more or not as happy to be there. She stated her child is always happy to go to Ms. Morrish's and never has unexplained injuries. On 04/20/22, I spoke to Child D and E's mother, and she stated she has never had concerns of the children not being supervised at Ms. Morrish's home. She stated that both of her children love attending and have been for about two years. Child D and E's mother stated her children attend three days a week and there are always enough caregivers and feels the children are always supervised. On 04/20/22, I spoke to Ms. Kilgore via phone, and she stated that children are always supervised while in care. She stated she will stay with the children when Ms. Morrish answers the door, so the children are never alone. Ms. Kilgore stated she takes the children outside in groups so that there is always someone outside and someone downstairs at all times. 5 On 04/21/22, I spoke to Ms. Amy Leydig via phone, and she stated that she has never had concerns with supervision at Ms. Morrish's childcare home. She stated children are never left downstairs alone or outside alone. There is always someone with the children at all times.

On 04/05/22, during my unannounced onsite inspection, I spoke to Ms. Morrish, and she stated all her children take naps during the day. The infants sleep whenever they fall asleep, and the older children have nap time after lunch. She stated that she does have a couple children who are trying to fight nap time, but they still lie down and rest. Most will fall asleep; some for two hours and some for an hour. When they wake up, they are allowed to play quietly at the table until the other children wake up. Ms. Morrish stated that during spring break, she had a couple school age children who did not nap. They were allowed to play quietly during nap time. Ms. Morrish stated that Child A is four years old, and her mother has requested that she not sleep anymore during nap. She stated that Child A still lies down and rests; however, due to her not sleeping at night, Child A's mother has requested that Child A not actually sleep during naptime. Ms. Morrish stated that Child A will sometimes still fall asleep, and she allows her to rest for a short period and then will wake her up. Child A is still provided rest time and still given the opportunity to fall asleep if needed. This is the only parent that has requested this, all other children sleep. On 04/20/22, I spoke to Child A's mother via phone, and she stated that about a year ago when Child A was three years old, she was having a hard time sleeping at night and not going to bed until midnight. She stated she had asked Ms. Morrish to help during the day and start reducing Child A's nap time to see if this helps. Ms. Morrish has always been so helpful with this and still gives Child A rest time; however, she does not let her sleep for as long as she used to. Ms. Morrish has Child A do quiet activities and still lie on her mat. Child A's mother stated she has always loved how Ms. Morrish is willing to work with her families to make sure the children are happy as well as the parents. Child A's mother stated she does not know if any other children do not nap as she is not there during nap time. On this same day, I spoke to Child D and E's mother via phone, she stated that her children both take naps every day. She stated she would know by their behavior when they got home if they didn't. Child D and E's mother stated that her children have never complained about napping or stated that Ms. Morrish was not allowing other children to nap. She stated Ms. Morrish always makes sure the children are happy while with her, and she has no concerns about napping. On 04/20/22, I spoke to Ms. Kilgore via phone, and she stated that all the children nap after lunch each day. She stated if school age children are there, they will still have rest time but will be able to read books, do puzzles or color during this time. Ms. Kilgore stated Child A does not sleep the whole time; however, she does fall asleep most days and then will wake up earlier. Once she is awake, she will do quiet activities as well. Ms. Kilgore stated that nap time has never been a problem with any of the children and everyone will lie down and have rest time for however long they need. Once the children awake, they go to the bathroom and then start doing a quiet activity. No children are forced to sleep, and no children are forced to stay awake. On 04/21/22, I spoke to Ms. Leydig via phone, and she stated when she worked with Ms. Morrish, there was never a concern of children not being allowed to nap. She stated Ms. Morrish always had all the children lie down after nap time and all children would rest. If a child did not fall asleep, they were not forced to, they would lie on their mat and then have a quiet activity to do while the other children napped. Ms. Leydig stated Child A did not fall asleep often; however, she would rest and lie down for about 30-45 minutes daily. On 04/05/22, during my onsite inspection, I observed multiple children use the restroom, each child washed their hands after. Four children were in highchairs eating snack and each child that Ms. Morrish got down, she took to the sink and had them wash their hands before going to play. Ms. Morrish stated that she always has the children wash their hands before and after eating, and she also washes her hands before and after serving children food. Ms. Morrish stated she washes her hands all day, every day to make sure not to spread any germs. She stated that the children are told a thousand times a day to wash their hands as well. Ms. Morrish stated she is very cautious about this with the spread of any viruses. On 04/20/22, I spoke to Child A's mother, and she stated she has never had concerns with Child A being dirty or having dirty hands when picked up. She stated she feels Ms. Morrish and all staff that work at her home always keep the children clean and wash their hands all day. Children in her care are not sick, and germs are not spread as quickly as they are at other places and she feels this is because Ms. Morrish washes the children's hands, her own hands and keeps things clean at her home. Child A's mother stated she is not there all day to actually observe Ms. Morrish washing hands but feels that she does. On 04/20/22, when speaking to Child D and E's mother, she stated she is not in the home to know how often anyone washes their hands; however, her children always come home clean. On 04/20/22, when speaking with Ms. Kilgore, she stated that she and Ms. Morrish always wash their hands before and after feeding children. She stated all children wash their hands before and after each meal as well and when they use the bathroom. Ms. Kilgore stated that Ms. Morrish is always having the children wash their hands, and Ms. Morrish is always washing her hands as well. She stated she has never had any concerns with cleanliness at Ms. Morrish's home. On 04/21/22, when speaking to Ms. Leydig via phone, she stated when she worked there, Ms. Morrish washed her hands all the time; before and after each meal and always after helping a child in the bathroom. She stated that Ms. Morrish always had the children wash their hands as well and the children were always very clean. Ms. Leydig stated the home was always clean and Ms. Morrish made sure the children kept clean faces and hands, so germs did not get spread.
