On 04/11/25, the outside area was not in good repair and maintained in a safe condition. The front porch was observed to have a step hazard. The porch is missing concrete around the ledge, and the metal support is exposed. Also, the exterior south side of the home, has several cable wires hanging/not attached to home.

R 400.1944(1) · R 400.1944(1) Smoke detectors; fire extinguishers. Operable smoke detectors approved by a nationally recognized testing laboratory must be installed and maintained on each floor of the home, including the basement, and in all sleeping areas and bedrooms used by children in care.

On 04/11/25, the smoke detector in the living room was not maintained. The smoke detector has a low battery and needs to be replaced with a fully charged battery.

R 400.1945(4) · R 400.1945(4) Emergency; plan; drill. Fire drills must be practiced while children are in care at least once quarterly, and a written record that includes the date and time it takes to evacuate must be maintained.

On 04/11/25, there was no record of fire drills being practiced at least once quarterly in 2024.

The individual knows that he or she is mandated by law to report child abuse and neglect. Technical Assistance Technical assistance is support and training given to the licensee/licensee designee on a specific rule. Technical assistance includes consultation on how the facility can best maintain compliance with a rule.

Each individual listed in subsection (1) shall give written consent at the time of the license application and before a group child care home or family child care home allows an individual to be a member of the household, or before becoming a child care staff member to allow the department of state police to conduct the criminal history check required under subsection (1). The department shall require the individual to submit his or her fingerprints to the department of state police and the Federal Bureau of Investigation for the criminal history check as required in subsection (1).

On 04/11/25, two childcare staff members had not completed the required health and safety modules prior to caring for children.

When the department of licensing and regulatory affairs or the department of education publishes a notice that a new health and safety update document or a new health and safety update training activity has been published on MiRegistry, the licensee shall ensure that all personnel read and acknowledge the document or complete the activity within 6 months of the notice.

On 04/11/25, two child care staff members had not completed the 2024 Health and Safety Refresher training.

On 04/11/25 the one staff on site, did not have a signed and dated statement acknowledging the following: abuse and neglect is against the law, childcare staff are mandated by law to report child abuse and neglect and has received a copy of the licensee's discipline policy.

R 400.1917(1) · R 400.1917(1) Telephone. Personnel shall have immediate access to an operable telephone within the child care home at all times.

The childcare home failed to maintain immediate access operable telephone inside the childcare home. Prior to conducting the interim inspection on 4/11/25, unsuccessful attempts to contact the licensee by telephone were made on 03/31/25, and 04/02/25. In addition, and email was sent to the licensee on 04/01/25, in which the licensee failed to respond.
