On 04/14/2022, I conducted an unannounced investigation at the facility. There were three staff present with five children. I walked throughout the center and found it to be extremely clean and well organized. I interviewed staff on duty regarding all the allegations and requested child information cards to interview parents. Ms. Tekeita Walker, program director, was not in the building. Staff 3 immediately called her and handed me the telephone. I explained the reason for the onsite investigation, and she denied all allegations. No child, or staff, can stay at the facility when showing signs of illness. Parents are called, informed of a sick child and requested to pick them up. Children wait in the office until they arrive. Ms. Walker does not know why someone would makeup allegations. She has a new center and values the relationship with parents. I was encouraged to call them regarding the accusations. While onsite, I interviewed child care staff members, Takeisha Walker and Majeana Peterson. They denied any children are allowed to stay at the center when sick. If they present with any signs of illness, the policy is to contact parents and they go home. While at the center I observed no child, or staff, exhibiting any signs of illness. On 04/15/2022, I called and interviewed the person who made the complaint. Complainant has no firsthand knowledge or additional information regarding the allegations. The information was told to her by "someone else." I requested my contact information be provided to the person who shared the information with her or they can contact me directly. I have received no calls as of this entry. On 04/20/2022, I contacted Child A's Mother, Child B's Mother and Child C's Mother regarding the allegations. They have no problems or concerns with the facility. Neither of the mothers had any knowledge of children attending while sick, and their children have not returned home from the center showing signs of being exposed to an illness.

Ms. Walker said she shops on a regular basis and feeds the children very well. They always have a balanced meal, and the menu is followed. If there's a reason for a substitution, it's documented. She would never give children bologna and cheese daily and finds it "crazy" someone would suggest it. 4 I inspected the kitchen and found a posted, dated, menu on the refrigerator. The area was clean and there was sufficient food in the cupboards and freezer. The building smelled of sausage which had been prepared for breakfast and was on the menu. Child care staff member Majeana Peterson was interviewed about the allegation. She often prepares the meals and always follow the menu. The children are always given balanced meals. She does not recall any bologna and cheese being served since she's been at the facility. I contacted Child A's Mother, Child B's Mother and Child C's Mother regarding the allegations. They have no problems or concerns with the facility and know the children eat very well and "the place is always clean." Child B's Mother states, "I know those kids eat good and I have pictures to prove it. My child would tell me if something was wrong at the place. I'm always up there and it's super clean and smells of home cooking."
