On 03/29/2023, the department received a complaint regarding Child A (3-year-old female). It is alleged that Child A is diagnosed with Autism, and staff are not updating parents on the progress of the child. On 03/29/2023, I called 2 Child A's Mother. She stated that the center does communicate with her about things that are happening with her daughter at the center. She gets phone calls and messages. She stated that the center uses the Brightwheel App and that the center documents when her pull-up is changed as well as her eating and sleeping habits. When asked if she has ever asked the center for any other progress updates, she said no. Child A's Mother stated that she gets messages and phone calls from the center when anything happens during the day. Child A's Mother said that her concern is that Child A gets sent home frequently. She stated that she believes that the child care center finds reasons to send her child home because they are frustrated with Child A since she is non-verbal. She was recently sent home with a rash on her butt, where the pull-up meets the skin. When Child A's Mother took her home and looked at her butt, there were no marks or rash, and Child A was acting normal. There was also a time when Child A was sent home with a fever but when she got home, she did not have a fever and was acting normal. On 3/30/2023, I completed an unannounced inspection at the facility. I spoke to Program Director Wanda Nathan and CCSM 1. Ms. Nathan stated that Child A started at the facility in January. Since starting it has been very difficult working with Child A's Mother. Child A is nonverbal and any time they try and ask Child A's Mother questions, she gets very defensive. There have been multiple occasions when Mom has cussed staff members out when trying to communicate. Ms. Nathan stated that they provide daily reports for all children enrolled regardless of age. They use the Brightwheel app to communicate to families. They message though the app when there is an issue as well as calling families when the situation requires it. When asked if Child A's Mother ever asked for additional progress updates, she said no. Child A's Mother has never asked for additional progress updates. It was hard to set goals for Child A since the Child A's Mother had not provided the center with Child A's IEP (Individualized Education Plan). The center finally got a copy of her IEP and they were schedule to meet with Child A's Mother on 3/31/2023, to discuss progress as well communication and how to move forward. Ms. Nathan said after a situation that happened on 03/28/2023 where mom was cussing staff out again, they decided to disenroll Child A from the facility. During the onsite inspection, I interview CCSM 1 who is Child A's lead teacher as well as assistant director. CCSM 1 stated that when children arrive each day, she always asks parents how their child's morning was as well as the night before. Any time she asked Child A's Mother these questions she would get defensive and did not like to share any information. Since Child A was nonverbal, they could always tell how Child A's morning was going based on her behavior. When asked if mom ever asks for any information on how Child A is doing at school, she stated, no, mom never asks about Child A's day. The center provided me with the daily reports that they sent Child A's Mother from 03/16/2023-03/28/2023. The facility communicated what Child A ate every day at mealtime. On 03/17/2023, they included pictures of Child A participating in different activities. They only documented diaper changes a few times. They also 3 documented an incident on 03/22/2023, where Child A got injured as well as an incident on 03/28/2023, where staff were trying to contact mom regarding a rash. On 04/03/2023, I called and interviewed Child B's Mother, Child C's Mother, Child D's Mother, Child E's Mother. Child B and Child C both have special needs. On 09/07/2022, the department received an incident report from the licensee designee, which states that Child A walked out of the center and was found outside by a person passing by. On 9/8/2022, I conducted an onsite inspection at the center. I interviewed licensee, Wanda Nathan and CCSM, Carol McClinton. Ms. Nathan was cleaning up after morning snack and dropped a food tray off with leftovers. Child A walked out the classroom and the front door. Ms. Nathan said an unknown witness was walking up the alley and observed Child A in the parking lot. The witness heard children playing outside and walked around to the play area. CCSMs, Allyssa Curry and Kiara Reed were in the play area and saw Child A with the witness as she tried entering the play area. At that time, Ms. Nathan realized Child A was missing from the classroom, as 2 Ms. Curry was bringing her inside. Ms. Nathan and CCSM, Carol McClinton was in the classroom with six children sitting at the table. Ms. Nathan was cleaning up for lunch when the incident occurred. Ms. Nathan informed me that she called Child A's Mother around 11:15 a.m. and left a message. Child A's Mother called her back around 11:30 a.m. She arrived early to pick up and Ms. Nathan informed her of the plan she was putting in place after the incident. According to Ms. McClinton statement in writing, she was in the kitchen preparing lunch. Ms. McClinton informed me that she was back and forth between the classroom and the kitchen. She heard the doorbell ring and thought someone was entering through the back door. It was the front doorbell that went off. At that time, she was made aware that Child A left out of the front door. She was going back and forth and sitting at the table while the children were playing. She does not understand how Child A got out of the classroom. According to CCSM, Allyssa Curry statement in writing. On 9/6/2022, around 10:45 a.m., she was in the outdoor play area and heard a man yell "you lost a kid". She along with two other staff members began to count and call off the names of the children they took out with them. Once she counted the correct number of children with her, she went to check the outside door that faces W. Chicago. She turned the corner and observed a woman reaching her arm over the gate to open the latch. She asked the woman her identity and saw her holding Child A's hand. The woman asked if she knew who Child A was. Ms. Curry informed her that Child A belonged to the center, but Child A's classroom never came outside. She entered the building with Child A and the witness informing Ms. Nathan of the incident and went back to her children outside. According to CCSM, Kiara Reed statement in writing. She was outside with her toddler children and heard a man from across the street say, "there is a kid outside the gate". Ms. Reed and Ms. Curry began to count their children and they all were accounted for. A woman came to the gate, opened the gate, and gave them Child A. Child A belonged to the group of children in the building and ran out of the front door. On 10/17/2022, 10/18/2022, and 10/19/2022, I attempted to interview Child A's Mother and Child A's Father. I left three voicemail messages. To this date, I have not received a return call. I was informed by Ms. Nathan that Child A was no longer enrolled at the center.
