  The premises was not maintained in a clean and safe condition. There were two areas where the floor tile needs to be fixed because they are chipped and cracked. Three ceiling tiles had water damage and seven are old and disintegrating. The bathroom located at the back of the preschool room had a broken ceiling tile. At the time of the inspection, the roof had been fixed and was no longer leaking and there is no evidence of mold. Toys outside were in good repair. The air quality was good as an air scrubber was used as a precaution.
