On 09/12/2025 there was garbage and debris on the playground accessible to the children. The bench outside had chipped paint. The bottom of the sliding board, one side had been scrapped, which has caused the slide to be a hazard when the children slid down. The children could be scratched by the plastic sticking out.

Emergency procedures. (1) Written procedures for the care of children and staff for each of the following emergencies must be developed and implemented: (e) Crisis management including, but not limited to, intruders, active shooters, bomb threats, and other man-made events.
On 05/19/2022, 06/30/2022 and 07/26/2022, I contacted the complainant and left a message. To date, the complainant has not called back. According to the complaint, on 05/17/2022, there was an active shooter, and the center kept the children. The program director did not know what to do. The director was waiting to hear back from the owner about what to do. The CCSMs members were saying that the center needed to be shut down and everyone needed to go home. Parents were still dropping children off during the situation. The center might need some training on active shooters. The complainant feels that a lot of lives were being put in jeopardy and danger. On 05/19/2022, I completed an unannounced onsite inspection at The Nest Institute Villages at Parkside child care center. The child care center is located inside the community center and it has several offices (i.e. leasing office, WIC, etc.) in the building. There are many housing units inside the housing complex near the community center. I spoke to Lashawnda Richburg, assistant program director, licensee designee, Joy Dean and CCSMs, Mercedes Lawrence. The child care center was observed. In room 1, there were two children, ages 6 weeks to 12 months, with two CCSM. In room 2, there were four children, ages 18 months to 3 with two CCSMs. Child information forms, attendance record, crisis management plan and CCSM contact information was provided for review. I spoke to Ms. Richburg, regarding the allegations. She stated that on 05/17/2022, she arrived at the center at 6:45 a.m., there was no concern. Around 8:35 a.m., she went out to clean the playground area and she seen the police in the housing complex. The police had the area blocked off. Ms. Richburg was unsure of what was going on in the housing complex. There were approximately 12 children and seven CCSMs inside the center. They had arrived at the center prior to the incident. The CCSMs were told about there were police outside and they could not leave. They were told to stay in their classrooms so Ms. Richbury could find out what was going on. Ms. Richbury contacted licensee designee, Joy Dean to inform her what was going on. Ms. Dean stated that she was on her way to the center. The CCSMs stated that they felt unsafe, and Ms. Richbury told them they were safer in the building then outside. The children's parents that had not arrived at the center were called and informed to not bring their children. Two parents arrived at the center for drop-off, after they were informed of the situation, and they took their children back home. Ms. Richbury found out that there was a domestic dispute that involved a gun in the surrounding housing complex. They did not hear any shooting, and she was told the shooter was gone. At 10:00 a.m., five of the CCSMs left the center and took their children with them. Ms. Richbury and CCSM, Asia Wilson remained at the center with six children. Ms. Dean arrived at the child care center and she was upset to see that the CCSMs had left the center. She contacted the main office to notify them of the situation. They decided to have an early dismissal at 1:00 p.m. and the remaining children were picked up by their parents. I asked Ms. Richbury if the center had a crisis management plan for active shooters and she stated, "Yes, however they did not follow the plan due to the shooter not being in the center and the incident occurred off the premises. I asked Ms. Richbury, how often are the 3 CCSMs trained regarding emergency procedures. She stated they are trained at the beginning of each year in August during preservice. However, they have some new CCSMs that have not been trained yet. Ms. Dean stated that the CCSM were not supposed to leave the child care center as they were not in any danger. She spoke to the police department on her way to the center. She was told that the domestic violence incident was between two individuals. The male pulled a gun threating the women, then he ran off. Ms. Dean denies sending nude pictures to anyone. said Ms. Dean did not send the pictures and that Witness 1 sent them. The police have not charged or convicted Ms. Dean of any crime at this time.
