On 03/06/2023 I completed a telephone interview with Child A's Mother. Child A is being potty trained and began around 02/20/2023. Child A is not verbally expressing that he needs to use the bathroom, so Child A's Mother informed licensee designee Joleka Taylor that she had begun the potty-training process. Child A's Mother informed Joleka that since Child A is not verbal the center staff could decide whether he needs to have on underwear or pull ups. Child A is dropped off at the center in his underwear. On the date of the incident (03/02/2023) Child A's Mother admits that she forgot Child A's diaper bag that contained two to three sets of extra clothing. Child A's Mother received a call from the center that Child A had a toileting accident. The staff did not inform Child A's Mother that Child A had messed his shirt up. When Child A's Mother picked Child A up from the center, she sent her daughter in to get Child A. When Child A got home, she realized that Child A did not have a shirt on under his coat. When Joleka called her to inform her of the accident Joleka said Child A's clothes were messed up, but never mentioned his shirt. Child A's Mother assumed it was his pants and requested that Joleka change Child A into the spare pants that he keeps at the center. Joleka never mentioned to her that Child A's shirt was ruined or that she needed to bring an extra shirt. Child A's Mother said she also has never been provided an infant toddler daily record. On 03/08/2023 I completed an unannounced onsite inspection at Tamara's All About KIDZ. I interviewed CCSM 1, CCSM 2, and Joleka Taylor. Joleka Taylor stated that she, CCSM 1, CCSM 2, and CCSM 3 were all present on 03/02/2023. Joleka stated that all week Child A had been sent to the center in underwear and on this date (03/02/2023) he was sent in a diaper. Joleka asked Child A's Mother why he had on a diaper, and she stated that she was running late this morning and didn't have time to put on his underwear. Child A woke up from nap and had a bowel movement that got all over his clothing, including his shirt. Joleka called Child A's Mother around 2:15 PM to request an extra change of clothing. Child A's Mother stated, "he has an extra change of pants in the cubby." Joleka informed Child A's Mother, "No, he needs an extra change of clothes." Child A's Mother informed her that she was already out and implied she was not going to get any. Child A was sent home in his undershirt and a thick coat. Joleka did not want to give another child's clothing to Child A. Child A's Mother called about 20 minutes after Child A was picked up and asked why Child A did not have a shirt on. Child A's Mother was yelling at Joleka and telling her, "y'all need to do better." Child A's Mother stated that she assumed that only Child A's pants were messed up. Joleka told her, "Well that's the problem with assuming." This caused Child A's Mother to yell further. Child A's Mother stated that Joleka should have put another child's clothing on Child A. Joleka felt that by telling Child A's Mother that he needed a change of clothing, she was being explicit enough in explaining the situation. Child A's Mother told Joleka she should have just put the soiled shirt back on Child A. Joleka told Child A's Mother that they are not to clean soiled clothing or keep children in soiled clothing. All families are allowed to leave spare clothing at the 3 center. The center requests at registration that families leave extra clothing as a part of enrollment. CCSM 1 stated that she believes that Child A came in with a diaper on. CCSM 1 stated that Child A arrived at the center before she did on that date. CCSM 1 provided the same information regarding the incident as Joleka. Child A did have on a thick winter coat and was sent out of the center with just his coat on, no shirt. CCSM 1 feels that if it was just his pants that were messed up, they would have no reason to call the parents and inform them of the accident. The staff w
