Patricia Adams, Betty Menifield, and Fatima Hunter "whoop c ildren in front of others. Betty Menifield threatens she "will get a belt to whoop their butts". The center has cockroaches and bedbugs and have been seen in the children's food.

On 4/21/2021, and I interviewed licensee, Patricia Adams, and child care staff members, Betty Menifield, Fatima Hunter and Child Care Staff Member 1. All four denied that there was ever a fire in the building. They confirmed that there was a rubber belt on the furnace that went bad and did make a burned rubber smell. The custodian turned the furnace off and fixed the belt. There was no need to evacuate the building. _ did view the furnace room and there was no sign of a fire.

This complaint came in as anonymous. I was unable to interview the complainant. On 4/21/2021, Ms. Adams stated that there was no child enrolled by the name given in the allegations for Child A. She provided a list of children enrolled. There was one child years old, with a similar name, whom will be referred to as Child A. Ms. Adams denied ever grabbing or laying hands on Child A. Ms. Adams stated that none of the caregivers use physical discipline. Ms. Adams stated that in January she noticed that the teachers were having to speak louder to talk over the children, so she bought whistles to get the children's attention. There have not been any problems since then. Ms. Adams spoke very highly of Ms. Menifield and insisted she would never threaten or hurt anyone. Caregiver 1 stated that she has never heard the other child care staff members yell at or threaten the children and has never seen anyone hit the children. The child 3 care staff members usually talk to children on the side to calm them down or talk through their behavior. Child care staff member, Fatima Hunter, stated that nobody "hollers" at the children but they do use "heavy" voices when needed. No child care staff members have threatened to "whoop" anyone and nobody uses cuss words. Child care staff member, Betty Menifield, confirmed that nobody, including herself, ever says they will "whoop" any children. She stated that there is no inappropriate yelling. Ms. Menifield says that the child care staff members there use verbal warnings. She has never seen any child care staff members grab children by the face or use physical discipline. On 511412021,

On 4/19/2021, I spoke with licensing consultant, Nicole Gibby. She stated that she has never seen the center in an unclean state or with bugs. On 4/21/2021, I spoke with child care staff member, Betty Menifield. She stated that she was not exactly sure what a bed bug looked like but did not think she had ever seen one. She denied that there were ever bed bugs at the center. Ms. Menifield did state that the church that the childcare is housed in does have an exterminator come every month as a preventative measure. If the church or childcare were to see anything between his applications, he would come out and address it. Ms. Menifield stated that she has observed a roach periodically over the years, but that there was not an infestation. She could not recall the last time she had seen a roach. She denied that any bugs were ever in the children's food. Patricia Adams stated that two years ago, an enrolled family did have bed bugs at their home. She closed the center down and to do a deep clean, just in case. She now provides sheets and blankets for nap time and is very diligent in cleaning and sanitizing the cots. Ms. Adams has not seen a roach in a long time. She retrieved the exterminators file with documentation of his visits. There was also a log for the church and the childcare center to record when they had sightings or concerns for the exterminator to address. The last entry was a roach in October 2020. Ms. Adams stated she has not seen any insects since that time. She has never seen or heard of any insects in children's food. Fatima Hunter stated she had never seen bedbugs or roaches in the center. Child care staff member 1 stated she had never seen bedbugs or roaches in the center. I observed the three childcare rooms and the kitchen. They appeared clean and organized. I saw no evidence of insects. 5

Act 116 of the Public Acts of 1973 require that child care staff members be at least 18 years old. On 4121/2021, Child Care Staff Member 1 stated that she was 722.111(1)(a) "Child care staff member" means an individual who is 18 years of age or older....

During my onsite investigation on 4/21/2021, Ms. Adams was unable to provide me with the child care staff members signed abuse and neglect statements.
