On 03/23/2026, Child A was not provided adequate supervision. Child A received bite marks on both her left and right hand. Child A was found to have multiple contusions and abrasions. Child A had superficial abrasions found on bilateral hands, left shoulder, upper back, cheeks, and left scalp. Child abuse specialist physician stated that the injuries observed on Child A’s torso and shoulder and back were likely caused by an adult.

I initiated the complaint on 12/01/2021 via telephone call to Child A's Grandmother. She stated that she takes Child A to and from the daycare. A few weeks prior, she said Child A's leg was sore and tender to touch and she told her it hurt because someone whooped her at the child care center. Child A started attending the center 11/01/2021, as they were highly recommended by a co-worker of hers. A few weeks after attending the center, Child A started whining and not wanting to go anymore. One day while at Meyers, Child A said, "my leg, they whoop me at the school". She asked Child A who whooped her, Child A said "Sha Sha". Child A's Grandmother stated she examined Child A and there were no marks or bruises. She then immediately called licensee, Shavera Lipscomb as known as "ShaSha" to ask her about the allegations. Ms. Lipscomb denied she or anyone at the center whooped Child A. On the morning of 12/2/2021, I received a voice mail message from Child A's Grandmother at 1:40 a.m. and 3:20 a.m. Child A could be heard in the background crying uncontrollably. Child A was repeatedly saying "I'll be good". The message also stated, "this is what they did to my grandchild." She said Child A was having a rough night and would not sleep. An unannounced onsite inspection was conducted at the center on 12/07/2021. During the investigation, the licensee, Shavera Lipscomb, stated that she was called by Child A's Mother on 11/06/2021 with concerns about Child A being hit at the center. They talked about the allegations for approximately an hour. Child A complained that she had been whooped, but she had also said that her dad whooped her because she would not potty. Ms. Lipscomb, informed Child A's Mother that she nor any staff members had whooped Child A. Child A was returned to the center on Monday, 11/08/2021. She also stated that Child A requires a lot of attention while at the center. Someone constantly must hold her hand and give her attention. Child A will cry if she is not getting all the attention from CCSMs. She offered Child A's Grandmother the information to download an app that would allow her access to see how and what Child A is doing while she is in care at the center. Child care staff member, Mireca Pollard stated that she provides a lot of care for Child A. She also denied allegations that someone at the center whooped Child A. No one has ever touched Child A inappropriately. Child A cries a lot at drop off time but stops after the grandmother leaves the center. She noted that Child A returned to the center after Child A's Grandmother made the allegations, and there was no change in her behavior. She never notices any marks or bruises on Child A, but Child A is potty trained and rarely needs help. I spoke to Child A's Grandmother again on 12/8/2021. I asked her if she informed Ms. Lipscomb that Child A also said that her "Dad" whooped her. She denied making the statement to Ms. Lipscomb, but she confirmed that Ms Lipscomb provided her 3 with an app called Duo that she could log onto and see Child A's activities while at the center. On 12/13/2021, I spoke to Child A's Mother. She stated that her mother takes Child A to and from daycare due to her work hours. She never wanted Child A to attend a child care facility because "too much happens". Child A's Mother stated that Child A told her mother and the father's mother about being whooped at the child care center by Sha Sha. Child A never told her about being whooped and she only reported it to her grandmothers'. Child A' Mother did notice a change in Child A's behavior after attending the center. When she would check Child A, she never noticed any marks or bruises. I asked if she ever whooped Child A, and she responded, "I don't believe in whooping children, and I never whoop her." I then asked if she was aware that it had been stated that Child A's Father whooped her. She said she wasn't aware but did recall telling Child A's Father that he had to be patient with her while bei
