On 7/31/2023, I conducted an unannounced visit. I interviewed the Licensee and Program Director, Ms. Kimberly Sanders. I informed her that there was a concern that staff was not properly providing care for children with autism. She reported that there is one child diagnosed with autism that attends the center, but she and CCSM2 provides all the care for that child. She stated that the child does not have severe autism and is capable of completing many educational activities. She said all staff have received all their proper training as required by the department. During the onsite, I observed one child in care who is diagnosed with Autism. He did not appear to require one-on-one supervision. I also reviewed CCSM1 and CCSM2 files and observed that CCSM1 had recently been hired and had the proper 2 documentation in her file for the length of time she has been employed at the center. CCSM2 has had all the required training for a child care staff member and as a lead caregiver, with over 15 years of daycare experience. CCSM2 is the lead caregiver for the older children up to 5 years of age. Ms. Sanders reported that none of the children require any additional attention or one on one interaction, and work well in the assigned group or classroom. I interviewed CCSM1. She said she has some interaction with all the children at the center at some point during the day, but there is no one-on-one care required for any of the children. She said she has received some training since she started three weeks prior. She did not report any issues with caring for any of the children.

At the time of my arrival, there was two CCSMs present with eight children. There was one toddler present, and the other children were up to 5 years old. CCSM 1 was present with Ms. Kimberly Sanders. I observed one child in a highchair during the duration of my onsite visit on the lower level, as Ms. Sanders said she was working with the child on learning activities at the time. The other seven children were in the upper level with CCSM1. It was naptime, but only four of the seven children were sleep. I interviewed CCSM 1 while on site. She said she has only been working at the facility for a few weeks. She said CCSM 2 leaves early and she is often left with six to seven children of mixed ages, including one infant, and the other children range up to 5 years old. CCSM 1 states that she is only the assistant, but she has to ask for help when CCSM 2 leaves the center. Ms. Kimberly Sanders stated that she is always present, and the center is not out of ratio. I explained to Ms. Sanders that at the time she brings the toddler into the room with the older children, that she must go by the ratio of the youngest child. She stated that CCSM 2 leaves to go to lunch and the children are sleep. I pointed out to Ms. Sanders that at the time of my arrival, all children were not sleep in the room with CCSM 1 which requires the room to be in ratio compliance, by the age of the youngest child in the room. CCSM 2 arrived before I left the center. She said she leaves for lunch at 12:00pm and returns around 1:00 pm. She is the lead caregiver, for the older children. She stated they provide good quality care to the children and denied that they are out of ratio compliance. I also explained to her that although it may be naptime, if all the children are not asleep, then the center must maintain ratio compliance. On 8/7/2023, I called Child A’s Mother. She said Child A (5-year-old, male) has attended the center for over 2 years. She has no concerns about the ratio issue, as when she drops off or picks up Child A, there is usually three to four staff members present with the children. On 8/7/2023, I called Child B’s Mother. She reported no concerns with the care of Child B (4-year-old, male) or no concerns with there being too many children and not enough staff. She stated the center does not have a whole lot of children attending at the time. On 8/7/2023 and 9/12/2023, I called Child C’s Mother. She reported that Child C (2- year-old, female) is the fourth child that has attended Sanders, and she has been very happy with the services provided to her. She has never had any concerns with 5 ratio issues or any other concerns since they have been providing care for her children.

During the onsite visit, there was 1 caregiver present with 7 children of mixed age group. CCSM1 said she is often left alone with up to 7 children of mixed age groups.
