On 08/09/2023, I conducted an unannounced inspection at the Royalty Daycare Center. While onsite I met with Program Director, Pamela Salters 2 as well as CCSM 1, CCSM 2 and Child C (15-year-old female). Ms. Salters stated that she was not present for some parts of the day on 08/03/2023, but that there were three infants/toddlers present. Child A's Mother texted her late that night asking her about the mark on Child A's face. She told Child A's Mother that she would talk to staff members to see what happened. It was reported to her from CCSM 1 that Child A and Child B were on the floor together. They were playing together during their circle time. It was possible that she accidently got injured at that time. It was also reported to her that Child C, noticed that Child A had a mark on her face that morning at drop off time. Ms. Salters stated that all infants and toddlers are kept away from the older kids and that there is a staff member always sitting with them. They typically play up by the alter area throughout the day. She also mentioned that she had an incident report that staff members wrote up, but that Child A's Mother would not sign it. While onsite I interviewed CCSM 1. She stated that Child C noticed a red mark on Child A's face at drop off time and asked her about it. She said, she did not know what it was and went into the kitchen to make breakfast. She stated that her phone was messed up so she could not message Child A's Mother though the Bright Wheel App to talk to her. Child A's aunt picked up earlier than usual, so she did not have a chance to call Child A's Mother to talk to her about the mark. She again reported that she thought she came into school with the mark on her cheek which is why nothing was reported to Child A's aunt at pick up time. When Child A's aunt picked up that day, she didn't say anything to the staff members. There were a few incidents where Child A and Child B (1-year-old female), were together playing. Child B was walking towards Child A at one time, and she raised her hand like she was going to hit Child A, but CCSM 1 grabbed her hand and said "No, gentle hands". There was also another time where Child B was pushing up on Child A to stand up. CCSM 1 said on the day Child A had the red mark on her face, she never seemed to be in distressed, never seemed upset and CCSMs did not notice any other marks on her. CCSM1 again stated the Child A arrived at the center with the red mark on her. With Child C's Mother's permission, I was able to talk to Child C while onsite. Child C stated that as soon as Child A was dropped off, while she was still in her car seat, she noticed a red mark. About 5 minutes later she asked CCSM 1 about it. As the day went on the mark did not get any bigger but she noticed it get a little brighter in color. She said she believes that the mark was on her left cheek. While onsite I also spoke to CCSM 2, who is usually with the older children but was with the infants while CCSM 1 was preparing lunch on 08/3/2023. She stated that she didn't notice any mark. The older kids were doing bible study and watching tv and she was on the floor with the babies. The babies were walking around, playing, no moments of crying, and Child A was very peaceful that day and calm. On 08/09/2023, I interviewed Child A's Mother by phone. She stated that she noticed Child A's cheeks when she got home. Child A's aunt picked her up from school and she didn't notice anything so she did not ask staff about anything. Child A's aunt also 3 dropped Child A off that morning, and she didn't notice anything then either. Child A's Mother stated that she saw Child A that morning and she had nothing on her face. Child A's Mother sent me a photo of Child A with a small red mark on her lower cheek. She also sent me a video of Child A from that morning. There were no marks on her face at the time of the video but during the video it appears that Child A's Mother grazed Child A on the cheek with her fingernail in
Child A (9-month-old, female) was left unattended and was pinched by an unknown child.
