    On 03/29/2022, I com leted an unannounced onsite inspection at RHS/Kidz in Action with I interviewed program director Deborah Moore-Hardaway, CCSM 1, CCSM 2, CCSM 3, and licensee designee Evetta Morris. Program director Deborah Moore-Hardaway stated that CCSM 1 and CCSM 3 are lead teachers in Child A's room. CCSM 3 was working in Child A's room on the date of the incident (01/31/2023). During the incident CCSM 1 was downstairs doing dismissal. CCSM 2 and CCSM 3 were present in the class with Child A. Deborah stated that she was in her office when she heard what sounded like a child falling or a thud and then crying. Deborah stepped in the hallway and saw CCSM 3 bringing Child A out into the hallway. Deborah is not sure if there were other children in the room with CCSM 2 and CCSM 3. She does remember seeing CCMS 3 standing in the middle of the room when she walked into the hallway to check on the crying. CCSM 3 brought Child A into the conference room and provided him with cold applesauce due to their not being any ice packs. Deborah and CCSM 3 noticed that Child A had a knot forming on his head. The incident occurred during dismissal time, so Deborah took Child A to the door for pick up. Deborah held Child A in her arms while waiting for pick up. Child A cried himself to sleep. Deborah assumed he fell asleep because he did not have a nap that day. Child A's Father arrived to pick him up within two to three minutes of her taking Child A downstairs. CCSM 1 initially began explaining to Child A's Father that Child A had an incident. When Child A's Father was told about the incident, he treated it as it was a "boys will be boys' type of situation." Deborah then explained to Child A's Father that they would be moving Child A from active enrollment back to the waiting list because they were worried about his safety. A week prior to the incident the center was considering moving Child A back to the wait list due to his behavior. Child A has had some behavior issues in the past and the center did inform the parents of the issues. Child A was initially enrolled in CCSM 3's room and was placed in CCSM 1's classroom 11/2022. After the Christmas break Child A's Mother was provided video of Child A running and crawling and tipping things over. The 3 center has several reports that were completed internally of Child A's behavior. Not all the reports were shared with Child A's parents. When Deborah informed Child A's Father that he would be moved back to the waiting list due to this recent incident his father did not have any response and just left. A week later Child A's Father called and requested a copy of all the reports that the center has ever done on Child A. Deborah provided them to Child A's Father and Child A's Father insinuated that the reports all appeared to be made up and done on the same day. CCSM 1 stated that she is the lead teacher in Child A's room. CCSM 1 requested that Child A be transferred into her room because he had been having behavioral issues and she thought he would benefit from being with older children. Child A is very smart and CCSM 1 has 4 to 5 year olds in her room. On the date of the incident Child A was having a rough day. That day, the center combined CCSM 1 and CCSM 3's room together due to staffing. On the date of the incident CCSM 1 stated that although Child A really like dinosaurs, those did not even work in calming him down. Child A was punching friends, throwing chairs, and even put his feet on the table during lunch. When Child A is having a bad day, they usually walk him up and down the hall and try to give him a break from the room. On the date of the incident taking walks did not help. At 3:00 p.m. each day CCSM 1 goes downstairs to dismiss students. CCSM 2 and CCSM 3 were in the room when she went to do dismissal. CCSM 1 did not see Child A bump his head or hurt himself.
