On 11/3/2022, I conducted an unannounced investigation at the child care center. This center is a head start program. There are two classrooms in a small space. The classrooms are separated by a shelving unit. I observed 24 children present and four CCSMs. In classroom A there were two CCSMs and 12 children participating in "work time". In classroom B, I observed two CCSMs and 12 children participating in gross motor and music activities. The center is noisy with all the children and activities. The CCSMs were able to hear me ring the buzzer and knock on the entrance door. The CCSMs were providing appropriate supervision. I interviewed the program director Sandra Brown. She denied that CCSMs are not supervising the children. She is the teacher in Classroom A along with her assistant. Classroom B has a lead teacher and an assistant. She stated the children know not 2 to open the doors and they had not had an issue with children trying to leave the classroom. I interviewed the licensee designee, Evetta Morris. She also denied the CCSM was not supervising the children. Ms. Morris stated they are a head start program, so they have people come to the center and no one has stated a problem to her. They had not recently had any agencies or visitors at the center. On 1/9/2023, I interviewed CCSM Ms. Jackson. She stated the children are supervised at all times. The center can get messy and loud during "work" time as the children are playing with different toys, talking, doing art projects etc. At the end of their activities, they clean up before transitioning to the next activity. There is no overflowing trash, and the tables are cleaned and sanitized before eating. Ms. Jackson is not aware of any incidents where a child tried opening a door. Her classroom is not near the door. I interviewed FSW Mr. Canty. He also serves as a CCSM in the classrooms. He stated he was not aware of any incidents were a child tried to leave the center or open any doors. He states the CCSMs are always supervising the children. He denied the trash is overflowing and the center is messy.

The facility is loud, and the Child Care Staff Members (CCSM) are not supervising the children. One of the kids started to run out by the church chapel and the CCSM didn't notice until another child yelled. The center is messy, trash overflowing and the tables where the food is served was full of papers.

On 11/3/2022, I was at the center during "work time". There were a variety of activities that the children were doing. There were toys out, art projects going on and the center looked like a typical head start classroom. At the end of "work time" the children and CCSMs cleaned up the area before going to the next activity. The trash was not overflowing. The maintenance person was emptying the trashcans. I was present while the classes were getting ready for lunch. The kitchen staff cleaned the tables before the children sat down. Ms. Brown, Ms. Morris, Ms. Jackson, and Mr. Canty each stated the tables are cleaned and sanitized before they serve lunch. The only papers that would be on the 3 table would be the food program papers that have to be filled out in real time while the children are eating. The trashcans have lids, so they are not overflowing. The center is still practicing strict COVID precautions. Parents are not allowed in the center, so they would not be able to comment on the condition of the center.
