I conducted a telephone interview with the complainant. The complainant said Mrs. Taylor allows kids infected with hand, foot and mouth disease to attend and there was no proper cleaning done. On 11/29/2023, I conducted an unannounced inspection at the facility. I Interviewed Toya Taylor, Mekena Brown and Milan Minner. Mrs. Taylor said back in November Child A (3-year-old female) was diagnosed with hand, foot and mouth disease. She did not return to the center following her diagnosis. Mrs. Taylor is aware of the need to properly clean and sanitize the center daily, but especially when there is concern of communicable diseases. Childcare staff members were instructed to clean all things that Child A may or would have come into contact with. There were no additional children or staff were diagnosed following her disenrollment. 2 While I was at the center, I noted that the facility appeared to be clean. I interviewed Ms. Brown and Ms. Minner, and both were new to the center and did not have any knowledge of the incident. Both showed me where cleaning products were stored and spoke of the daily cleaning routine. On 03/02/2023, I interviewed Child A's Mother over the phone. She stated Child A did not return to the center after she was diagnosed with hand, foot, and mouth disease. She did not feel comfortable taking her back. She is uncertain as to how she was exposed to hand, mouth and foot disease stating her daughter only spends time at the daycare and at her grandfather's house. On 03/02/2023, an exit conference was held with Mrs. Taylor via email. She stated she would not knowing allow an ill child to attend that could possibly endanger other children, her staff, or herself.

On 11/29/2022, I interviewed the complainant over the phone. The complainant said the center is understaffed. The GRSP class does not have a lead teacher. Child care staff member, Ms. Mull, is in the room with 17 children by herself. On 11/29/2022. I interviewed the licensee designee, Tonya Taylor. She said she was understaffed after two of her child care staff members quit unexpectedly. She believes that the source of the complaint is one of the two former staff members. 3 She has been filling in as the lead teacher in the GSRP classroom until she is able to hire a new lead. One of the two staff members that left was the lead. She currently has seven employees not including herself. Mrs. Taylor is qualified as a lead teacher and qualified to cover the GRSP classroom, as she is the approved program director for the center. She said that at any time ratio is a concern she stands in. She continues to work to hire more approvable childcare staff members and admitted there is a high turnover in her center at no fault of her own. She is aware of ratio requirements and denies noncompliance. I reviewed attendance records for the week of 11/14 thorough 11/18. The center was in ratio everyday in every classroom. While at the center, I observed her in the GRSP classroom with nine children ages 4 to 5 years. She was in compliance as ratio requires one child care staff member to every 10 children in that age group. In another classroom, there were six children, ages two to four years, with two childcare staff members. The ratio compliance was also achieved in that classroom as it is required that there be one child care staff member for every four children in that age range. I interviewed cook/floater, Cyrese Williams and GSRP assistant, Anna Hunt. Ms. Williams shared information regarding the CCSMs that were let go. She was at the center when one had to be taken to the police department following a visit from a disgruntle parent. Ms. Williams has had to work in the classroom to ensure compliance with ratio. When ask what the ratios were in the preschool classroom where she works most often, she did not know. She said she fills in where she is needed. Ms. Hunt was aware of ratio requirements and denied that it is a problem at this time. She said she make sure she comes to work daily because "its not fair to Ms. Taylor". I interview childcare staff members, Makena Brown, and Miss Gordon. Both childcare staff members were able to tell me the ratio requirements for the children that they serve. They both are new and had no additional information about ratio in the past. I reminded them that they are mandated reporters, and that the licensing rule book is posted in the center for their review. Collectively I spoke with the CCSMs about ratio and encouraged them to limit call-offs as they know if even one of them do not come to work there would be ratio non-compliance. 4

Based on my review of the attendance records, interviews with CCSMs and my observation of the classroom on 11/29/2022, there is insufficient evidence to should that the center has not maintained the required ratios for each room throughout the center.

On 11/18/2023 I interviewed Child A's Mother over the phone. She said that everyday that week was something with Child A. She and Child A's Father believe the child care children are not being supervised. While in care Child A had her backpack damaged and she came home with injuries that the staff could not explain. She said on 11/08/2022, Child A went to daycare with a brand-new backpack. She returned home with a scratch on the left side of her face and her backpack was cut up. On 11/09/2022, Child A returned from daycare with a scratch on her left collarbone and on 11/10/2022, she returned from daycare with a red mark 2 on the left side of her neck and a scratch on the right side of her face near the temple. Child A no longer attends the center. On 11/29/2022, I conducted an on-site inspection at the center. I interviewed the licensee designee/program Director Toya Taylor. She said Child A only attended the center for that week and added that on Friday (her last day) both parents came to the school inquiring about aftercare. She found it strange when she was contacted later by the parents concerned about improper supervision. She wondered why after the week in care, if Child A was allegedly not being watched, would the parents then want her stay for addition time on Friday. Ms. Taylor denied any knowledge of Child A's backpack being damaged or her receiving any injuries during her time in care. When she spoke with the parents, they did not produce any pictures for her to be reviewed nor did they bring the backpack to the center. She spoke with the childcare staff member, CCSM 1, who said she was not aware of any injuries that Child A sustained. While at the center, I interviewed CCSM1. She -is the lead caregiver in Child A's classroom. She is the only CCSM in the room. CCSM1 said she was oriented to the centers policies and felt properly trained to provide appropriate care and supervision of children. She denied there was a time when Child A was injured while at the center. She heard that the parents were upset and making threats but didn't understand why. Child A's parents never expressed any concerns to her regarding Child A's book bag or injuries she may have received while at the center. Before leaving the center, I retained contact information for other children in care. While in the center's parking lot I interviewed Child B's (4-year-old female) Mother via phone. She denied any concerns about the services she was receiving from the day care center. On 02/27/2023 I interviewed Child C's Mother (3-year-old female) over the phone. She also had no concerns and spoke highly about the relationship her children had with the staff and Ms. Taylor. On 02/27/2023 an exit conference was held over the phone with Ms. Taylor. I shared that there were no finds related to the allegation of improper supervision and that I was recommending no change in the status of the license.

On 09/06/2022, I completed a telephone interview with the complaint source who stated a toddler was fed salad and choked. On 6/2022, I completed a telephone interview with volunteer There as a child, not sure of the child's name, who choked on a piece of salad. informed another CCSM, not sure of her name, that the child was in distress. The CCSM swiped the food out of the child's mouth and just walked away. This occurred oni " first and only day working with children at the center 08/22/2022. On 09/30/2022, I completed a telephone interview with CCSM Diera Smiley-Butler. Diera stated that child that the allegations were made about was Child A (1-year-old, female). Child A did not choke on her food. Diera did sweep Child A's mouth to clear out food. Child A was not in any distress before or after sweeping her mouth. When Child A began attending the center Child A's Mother informed Diera and staff that Child A has tendency to hold food in her mouth, and if allowed will sleep with it in her jaw. Child A's Mother requested that Child A be monitored and if necessary, remove food from Child A's mouth. Diera knew that Child A had food in her mouth and had it in there for a while. CCSMs were beginning to get children ready for rest time. Diera did not want Child A to go to sleep with food in her mouth, so she swept her mouth and cleared out the food she had been holding in her jaw. On 09/30/2022, I completed a telephone interview with Child A (1-year-old, female) and Child B's (1-year-old, male) Mother. Child A and B have been attending the center since April 2022. Child A and Bs Mother stated that her children do eat salad and will often hold food in their mouth. Child A and B's Mother often has to sweep food out of their mouth. Child A and B's Mother did inform CCSM Diera that her children will do this and will require her to sweep food from their mouth. Child A' and B's Mother knows that her children will go to sleep with food in their jaw and did not want to risk them choking in their sleep. She informed staff to prevent that from happening.

On 09/06/2022, I completed a telephone interview with the compliant source who stated that the food the children are feed is horrible. On 09/06/2022 and 09/30/2022, I completed a telephone interview with CCSM LeAnn Taylor and LaKiya Saunders-Carter. Both CCSM's felt that the food that the children are feed is horrible or unacceptable. CCSM LeAnn Taylor referenced a time in which she felt that children were fed food that was left in the refrigerator too long. The area had a power outage, and the food was in the refrigerator during the power outage. LeAnn felt that a particular food item they had served had spoiled. CCSM LaKiya Saunders-Carter just felt that the food was horrible, was not able to provide any direct example as to why she felt that way. On 09/30/2022, I completed a telephone interview with Child A and B's Mother and Child C and D's Mother. Child A and B's mother did not have any concerns about food. Child C (2-year-old, male) and D's (1-year-old, male) Mother is concerned her children are not being fed enough. Child C and D's Mother children often complain or appear to be hungry. On 09/09/2022 and 09/30/2022, I completed telephone interviews with licensee and program director Toya Taylor. Toya states that she uses a rotating set menu. There is a menu for weeks one through five. Whatever is on the menu for week one is what children eat for week one of the month and so on. The center participates in the child and adult care food program (CACFP). The menus have been approved by her food program sponsor as appropriate and sufficient for the children served. rj Toya also stated that when the center lost power all food was thrown out and discarded except for food that was at the bottom of the deep freezer. The food at the bottom of the deep freezer was still frozen solid and she felt that it was free from spoilage. Toya stated that, "if I feed children spoiled food someone would have been sick and I didn't not have any sick children after that power outage." During my on-site inspection on 09/13/2022 and 10/26/2022, I reviewed the centers menus. The center provides two meals and one snack daily. Children are offered an a.m. snack, lunch, and dinner. The center has a deep freezer, a refrigerator, and pantry. The freezer and deep freezer were filled with bulk meats, pre-cooked potato foods, and vegetables. The pantry was filled with bulk canned fruits and vegetables and dry pasta. The pantry also contained bulk snack foods such as graham and animal crackers. The refrigerator had bulk milks, vegetable, cheese, and condiments. During my onsite inspection I observed the ending of lunch. Lunch for the day was nachos which included salsa, tortillas, lettuce, and cheese as the meat component. The supper was already prepared for the evening and was in covered containers. The supper included macaroni and cheese and salad. The menu for 10/26/2022 did not have the correct lunch and supper that was prepared and served. Toya stated that the menu was going to be amened to show the change. Toya stated that if a component of a meal that is on the menu for the day is missing or low then the cook will either substitute that component or change the meal completely. Additionally, today's meals did not include meat but instead used cheese as a meat alternate. Meat alternates such as cheese are approved by CACFP. Portion sizes in the center are determined by the CACFP. Toya serves food family style. All children sit together and have the meals placed on the table with the portion size scoop in the food container. Children are then allowed to use the scoop to place the portion on their plate or the CCSMs will place the portions on the children's plates. Toya stated that children are allowed additional portions of food if they would like. The children are allowed to add food to their plates as they like. I observed left over food available from lunch service that would have been available to children should they have wanted additional portions.

On 09/06/2022, I completed a telephone interview with the complaint source who stated the center was nasty and dirty. completed a telephone interview with volunteer also stated the one day that she worked at the center it was nasty and she was cleaning from the time she arrived until she left. was unable to provide any direct examples how the center was nasty. On 09/30/2022, I completed telephone interviews with CCSMs Cyrefe Williams, Diera Smiley-Butler, and LeAnn Taylor. All three CCSMs stated that cleaning procedures are not followed. CCSM Diera Smiley-Butler stated that the center was unsanitary. Toya Taylor wanted, "everyone to get off the clock instead of cleaning to avoid having to pay employees for any additional time." CCSMs were not able to clean throughout the day as they should because it was too chaotic. Diera stated that daily cleaning procedure such as cleaning tables after children eat and cleaning restrooms are not occurring. There is a cleaning schedule and routine, but it is never followed. CCSM LeAnn Taylor stated the center was dirty. In particular, "the floors aren't cleaned daily, the bathrooms are cleaned once or twice a week." LeAnn stated that there were not any cleaning procedures in place. Toya Taylor would tell CCSMs not to clean because she wanted them off the clock. Toya would send CCSMs home as children left. CCSMs would not be given the opportunity to clean before leaving, leaving the last CCSM present to clean the entire center. This would be too much work for one CCSM, so it wouldn't get done. CCSM Cyrefe Williams stated that when she first began working at the center it was dirty and not kept clean. Currently, Cyrefe feels that she and another CCSM are, "getting things together and able to clean and get things done." There are daily cleaning procedures in place, and they are completed. Cyrefe admitted that she is not always able to clean the bathrooms, but another CCSM does for her. The 11 cleaning procedures are to sanitize toys daily and as needed throughout the day. On Friday's all the linen is washed. Tables are to be wiped down after the children eat. On 09/09/2022 and 09/30/2022, I completed telephone interviews with licensee and program director Toya Taylor. Toya stated that CCSMs are to use the three step process throughout the day. Toys are to be sanitized as needed throughout the day. Daily, CCSMs are to sweep and mop the floors, clean the bathrooms throughout the day, and clean dishes after each meal service. CCSMs are to clean constantly throughout the day. On Friday's CCSM's wash the linen and deep clean the toys and cots. Toya does not, not allow CCSM's to clean or tell them to get off the clock to avoid cleaning. Toya is constantly reminding staff to ensure that they are cleaning throughout the day and as required. During my onsite inspections on 09/07/2022, 09/13/2022, and 10/26/2022 the center did not have visible dirt or signs of filth. The center appeared to be clean and maintained.
