On 12/15/2022, I initiated the investigation via telephone with the complainant. The complainant observed lacerations to both sides of Child A's forehead and a bruise on Child B's forehead. The complainant said the injuries are consistent with the explanation of the injuries. Child A received stitches for forehead injury that occurred where he fell and hit his head on a table. Child B's bruise was consistent with banging her head on the wall. Child A and Child B's aunt reported to the complainant that both injuries occurred at childcare. On 01/12/2023, during an on-site inspection, I interviewed child care staff members (CCSM) , and the program director LaSonda Williams. Ms. Williams denied inappropriate supervision or inappropriate discipline. She reported that during the dates in question, Child A seemed "off'. On 12/13/2022, he kept falling. When he stood, he'd "tilt over". The behavior was unusual and was reported to Child A and Child B's aunt as Child A hit his head on the floor which formed a knot to his forehead. Child A and Child B's aunt was notified and was asked to pick up Child A. The next day, 12/14/2022, Child A's legs appeared to be "giving out" on him and "buckling". This caused him to tilt over and hit his head again. This time he hit his head on a table. Child A and Child B's aunt was called, and she was advised to take him to the doctor, and he could not return without a medical note from a doctor. First aid was applied to Child A after his fall. Ms. Williams reported that Child B came to care with a bruise. Child A and Child B's aunt reported that Child B bangs her head on the wall at home. Ms. Williams said that Child B has never banged her head while in care. Both children are still enrolled in care. Each child was observed in their classrooms. On 01/12/2023, childcare staff membei reported that she is Child A's CCSM. She denied inappropriate supervision or any form of physical discipline. She stated that Child A would fall forward, like he was losing his balance. On 12/13/2022, while playing, he stopped walking, and fell forward. On 12/14/2022, he was sitting down, once he stood up, he immediately fell forward and hit his head on 3 the table, said that since his return after the holiday break, Child A has fallen buts e now keeps him close to her because she never knows when he will fall. has never seen Child B bang her head on a wall in care. Child A and Child s aunt has reported to them that Child B bangs her head on the wall at home. On 12/27/2022. during the rxedisroosition conference a preponaerance of evidence to support the allegations. On 0111212023, childcare staff member is the lead caregiver in Child B's classroom. She denied that Child has banged her head at childcare. She asked Child B about the bruise when she arrived to school with it. Child B told her that her brother did it. Child B did not say which brother or how. said that she shared this information with Ms. Williams. denied inappropriate supervision or using any form of prohibited discipline. On 0111212023, childcare staff member reported that she is a CCSM in Child B's class. She has never observed Child bang her head in the childcare center. She has never witnessed Child B fall causing the bruise to her head. Child A and Child B's aunt reported that Child B bangs her head at home. On 02103/2023, Child C's Mother stated that she does not have any concerns regarding care and supervision at this facility. Child C has been enrolled since September 2022. On 02/03/2023, Child D's Mother stated that she does not have any concerns regarding care and supervision at this facility. Child D's Mother reported that Child D is the is 1 of 6 children that has attended this program. She described the care that her children receive as "top notch". On 02/1612023, Child A and Child B's aunt reported that she was notified on 12/13/2022 and 12/14/2022 when Child A was injured. She took child A to the hospital on 12/14/2022 where he received stitches for
