On April 11, 2023, I conducted an on-site inspection. I interviewed Program Director and School Principal, Leticia Smith. She said on 04/03/2023 at approximately 2:30 PM lead teacher, CCSM1 was caring for 14 GSRP students ages 4 to 5 years old along with his assistant, CCSM3. He took 14 children outside to the playground alone. He returned to the classroom with all but one student. With further investigation it was found that from approximately 2:48 PM until 2:54 PM, six minutes total, Child A was outside alone. CCSM2 went to a door to start, her vehicle in preparation for leaving for the day when she found Child A outside alone. The center is currently on a provisional license due to improper supervision. The same child care staff member was found to have improperly cared 2 for another student in his classroom a few months ago. Mrs. Smith said that CCSM1 has not been receptive to corrective action plans and training regarding supervision. It is the center expectation that there are always two caregivers with the children keeping them in ratio. Prior practices include lining the children up and counting them before and after leaving the building. CCSM1 has been employed at the school for 30+ years and continues to "not take ownership nor recognize the severity of his actions". Students and caregivers were not able to be interviewed at that time as the school was on a field trip to Greenfield Village. To ensure proper supervision several parents and support staff joined CCSM 1 on that days' field trip. While at the center I interviewed CCSM2. She stated that every day she starts her car a few minutes before she ends her workday. On that day when she opened the door to hit her automatic start on her vehicle, she saw Child A standing at the door. Child A said, "he left me". She walked Child A to her classroom and CCSM2 asked, "Where were you?". When CCSM2 told CCSM3 that Child A was outside alone CCSM3 said, "That's on him [CCSM1]. CCSM2 reported the incident to the principal before leaving for the day. CCSM2 walked me from the classroom to the door #7 and then out to the playground. The outdoor play area was observed to be fenced stand and approximately 103 feet away from the door used to exit the building to the playground. The classroom is approximately 268 ft into the building. Child A essentially was unsupervised for six minutes and 371 ft away from her classroom. I observed the door has a pull handle and requires a key to enter or a key fob. The door has a to be metal with a 6"x 8"glass window with a metal grate, approximately 5 feet from the ground. CCSM2 and I agreed and noted that a child's head could not even be seen through the window. Child A banged in the door and probably could not have been heard with the occupied gymnasium right at the entrance of the doorway on the other side of the hall. On 04/16/2023 I interviewed CCSM1 over the phone. He admitted he took the children outside alone to the playground. He said Child A must have walked away from the group and was regrettably left outside. He did not know for sure how many children were in attendance on that day but is aware of the ratio of one child care staff member to 12 preschoolers. He could not recall if he counted the children before leaving the classroom, the school building or upon his return to the classroom. On 04/16/2023 I interviewed the Child A's Mother over the phone. She was very concerned about supervision. She said her child was involved in another incident a few months ago. On 05/11/2023 I interviewed Child B's Mother and Child C's Mother. When asked if they had any concerns regard supervision or the services, they are receiving they both said they had no concerns. 3 On 07/06/2023 an exit conference was held with Licensee Designee, Amanda Mattea, over the phone. On 04/03/2023, CCSM1 took 14 preschooler outside alone. The ratio for preschoolers, ages for 4 years of age until school-age is one CCSM to 12 children. The center was out of ratio as they would have needed at least one additional CCSM.

On 02/14/2023, licensee designee Amanda Mattea notified me of an incident involving a child having her hair cut. She did not have additional information but requested involved staff complete an incident report. On 02/15/2023, I called Child A's Mother and Child B's Mother to discuss the incident. Child A's Mother recalls receiving a call from CCSM, Mr. Rick Rodriguez informing her Child A's hair had been cut. According to Child A's Mother, "He was laughing and makes jokes all the time, so I did not know what to think." He "never apologized" for the incident and did not appear to take it seriously. She was "angry and hurt" when she saw Child A's hair and was concerned things could have been much worse. When Child A left for school, her hair was mid-back in length and up in a ponytail. Upon returning home, it was jagged, uneven, and short like a bob. She had worn a gray t-shirt to school which had also been cut several times on the left arm closest to the wrist. Child A's Mother feels she could have been seriously hurt and does not understand how the incident happened without anyone seeing anything or intervening. She did not have any problems with the facility before the incident but is now concerned about the lack of supervision. Child A is doing "ok" but does not like her hair. She continues to attend the facility. I requested and received before and after photos of Child A's hair. The photos showed a picture of Child A's hair that was mid-back length and another picture where her hair had been unevenly cut as described by the parent. Child B's Mother received a call from Mr. Rodriguez and was told Child B (4-year- old, female) had cut "someone's hair." He wanted her to be picked up early and stated they could talk when she arrived. During the call he was joking around so she had no idea how serious the incident was until she arrived at the school. She was talking to Child A's Grandmother, who works at the facility, about another issue and she mentioned the incident. Child B's Mother immediately went to the classroom and met with Mr. Rodriquez. He did not see what happened but knows Child B was redirected during rest time for being off her cot. When the hair was found she initially denied cutting it but later admitted she did. Child B's Mother was "hurt" when she learned it was Child A's hair that was cut and the "whole ponytail." She was not aware the shirt was cut until I mentioned it. She has had concerns relating to supervision based on past information from Child B. When questioned, Mr. Rodriquez has assured her they supervise children and suggested Child B had an active imagination. Child B's Mother believes the incident would have taken some time and does not believe staff were supervising the children. On 02/16/2023, I conducted an unannounced investigation at the facility and inspected the classroom, discussed the incident with school principal Mrs. Latisha Smith and interviewed CCSMs, Rick Rodriquez and Dora Rodriquez. Mrs. Smith said she learned of the incident from Child A's Grandmother, who works at the school. She immediately discussed the incident with Child A's Mother and Mr. Rodriguez. He could not offer an explanation as to how Child B obtained the scissors or had time to 3 cut hair or clothing without staff being aware. Mrs. Smith expects all children to be properly supervised and is following district protocols in handling the matter. During the investigation, I discussed the importance of direct supervision, and the potential of serious harm had the scissors punctured Child A. I also requested CCSMs show me where Child A and Child B were resting the day of the incident. Their cots were on opposite sides of a 12- cubby storage unit. Staff refer to it as a shelf. I observed each cubby had a bin with manipulatives except for one on the bottom. It had a single pair of child size scissors. I asked if there was a bin for the scissors and where was it. Mr. Rodriquez showed me the bin was sitting on a counter out
