On 11/09/2021 I conducted an unannounced onsite inspection. I interviewed site manager, Kimberly Alston. Mrs. Alston said she was contacted by Child A's Mother with the complaint information. Brenda Stinson is the lead in the Head Start classroom #2 with Oienriette Plair, as her assistant. She has worked with the company for six and half years without incident. The center does have a camera system but there was no date offered. She has fingerprints on file, a signed child 2 abuse and neglect, and is fully trained and oriented. Despite the allegations Child A is still attending the center and was present. While at the center I interviewed Ms. Stinson outside on the playground. She denied at any time kicking Child A anywhere on her body. She said she would never do something like that and doesn't know why Child A would say something like that. She said she is aware of the center's policies and procedures including discipline, that she is a mandated reporter, and that child abuse/neglect is against the law. She described Child A as a typical, talkative 3-year-old female that is usually honest. She said she really uses a hands-off approach with the children to encourage independence and self-regulation. While outside I also interviewed Ms. Plair. She described Mrs. Alston as "really good with the kids'. She denied ever seeing the alleged incident happen and added that Mrs. Alston wouldn't do anything like that. She too is aware of the center's policies and procedures including discipline, that she is a mandated reporter, and that child abuse/neglect is against the law. I observed Child A playing on the playground. Ms. Stinson and I sat down on a bench, and I asked her to call Child A to her. Child A came without hesitation and talked with her while standing between her legs. I asked Child A her name, age, and complimented her hair before Ms. Stinson sent her back to play. I noted Child A had no reservations coming and talking with Ms. Stinson. On 11109112021, I interviewed Child A's Mother over the phone. She said Child A told her the allegations and she reported it to the center. Child A does not have any injuries and continues to attend the center. She had no prior concerns with the child care staff members prior to this matter. She doesn't know Child A to be untruthful but was not sure if she knows the difference between truth and lie. She said she did not have any additional details or contextual information i.e., when the incident happened, witness information, or where they were when it supposedly happened. On 111109/2021, 1 spoke with I let her know I had already been out to the center. i On 11123112021, I spoke with who scheduled a pre-disposition hearing to discuss the allegations for on 12110212021. On 12110212021 a predisposition hearing was held over the phone. I shared that the center has sever, been no findings. I told investigation. iC] On 01/11/2022 I interviewed Child B's (3-year-old male) Mother and Child C's (3- year-old female) Mother over the phone. Both parents denied having any concerns about the care their children are receiving from Mrs. Alston and the center.

On 10/21/2021, I conducted a telephone interview with program director, Jennifer Dukes. Ms. Dukes said Child A's Mother came into the center and informed her Child A said CCSM, Lisa Taylor spanked him. Ms. Dukes reported the incident to the Child Care Licensing on 10/20/2021 and Child A was moved into another class, per Child A's Mother request. The incident was reported to the Matrix Human Services Human Resources (HR) Department. The HR department will complete an internal investigation and decide if any disciplinary action is needed. 2 On 10/21/2021, I conducted a telephone interview with Child A's Mother. She said on 10/19/2021, Child A was picked up from school by her best friend. She arrived home at approximately 7:00 p.m. While putting Child A to bed, he told her Ms. Taylor whooped him. Child A's Mother asked why this happened, and Child A said, "because I wouldn't sit down, and I was crying." She asked Child A to show her how Ms. Taylor spanked him, and he tap her on her butt. Child A said CCSM, Jameila Ford was sitting at the table when the incident occurred. Child A's Mother explained that the next morning, while she was getting him dressed, Child A said he did not want to go to school because Ms. Taylor was going to whoop him. When Child A's Mother arrived at the center, she spoke with Ms. Dukes about the allegations. Child A was moved into another classroom. Child A's Mother said he is happy in the new room and thinks it is a new school. Child A has been in the program for over a year and has never said anything like this about a CCSM. Child A's Mother said she is not sure if the incident happened, but she felt that she should report it. Child A has never been whooped before, therefore she was worried if he was spanked at the center. She does not have any other concerns. On 10/26/2021, I conducted an unannounced inspection at the facility. I interviewed CCSMs, Lisa Taylor and Jameila Ford, custodian, Teddie Williams and program director, Jennifer Dukes. Ms. Taylor and Ms. Ford are the CCSMs in Child A's room. Ms. Taylor denied whooping or spanking Child A. She said on 10/19/2021, Child A was crying, she put her arm around him, then rubbed and tapped his thigh to calm him. This was the only eventful things that happened on this day, related to Child A. Ms. Taylor has not had any problems with Child A, he has been in her class since the school year began and she worked with him via Zoom, during the spring. On 10/20/2021, Child A spoke with Ms. Ford, and she asked Child A if Ms. Taylor hit him. Child A said she did not. However, when Ms. Ford rephrased the question and asked if he was whooped, he said yes. Ms. Taylor feels that maybe there is some confusion as to how Child A defines what a whooping is. Ms. Taylor said Child A remained in her classroom for the first part of the morning and out of nowhere, Child A walked up to her and said, "You didn't hit me". The custodian, Teddie Williams, was present and heard Child A say this. Ms. Taylor said she would never hit a child and believes that maybe Child A is confused. Ms. Ford said on 10/21/2021, Child A's Mother asked to speak with her. Child A's Mother informed her that Child A said Ms. Taylor whooped him. Ms. Ford informed Child A's Mother she did not hear or see Ms. Taylor whoop Child A. Ms. Ford and Child A's Mother informed Ms. Dukes. Ms. Ford asked Child A about the incident and Child A said Ms. Taylor whooped him. Child A also said Ms. Ford was sitting at the table when the incident occurred. Ms. Ford asked Child A to demonstrated how Ms. Taylor whooped him and he tapped himself on his side. Ms. Ford said the only thing she could think of is that Ms. Taylor was patting Child A on his side, because he was crying. Ms. Taylor was instructing Child A to use his words, instead of crying to let her know what was wrong with him. Ms. Ford believes Child A may have 3 misinterpreted what happened because she did not see anything else happen on this day. Ms. Ford said she
