On 6/21/2022, 1 interviewed the complainant. Complainant witnessed children's diapers being wet and soiled and not changed as often as needed. Complainant observed a child take his Pull up off that was soiled down to the knees. The child took his shorts off and one of the child care staff members eventually changed him. On 6/21/2022, I conducted an onsite inspection. I interviewed licensee, Dr. Constance Price and CCSM, Jamila Peterson. Both stated diapers are checked and changed every two hours. This concern has not been brought to their attention. The infant and toddlers use the same space which consists of three rooms. On 7/27/2022, I interviewed Child A's (1-year-old, female) Mother. There have been instances when Child A was picked up and clothes were changed from diaper soaking through. Child A is changed before she is picked up because she is not being changed as needed throughout the day. On 7/27/2022, I interviewed Child B's (7-month-old, female) Mother. She picked Child B up once and her diaper was extremely wet after she woke up. On 8/4/2022, I conducted a second unannounced inspection to the child care center. I interviewed CCSM, Tonya Graham. She works in the infant and toddler classroom. Diapers are changed quite often. Bowel movements are changed as needed and wet diapers are changed regularly every two hours. Children never leave the center in wet diapers. On 8/4/2022, Ms. Peterson stated that no one brought to her that attention diapers are not being changed. Diapers are changed and checked as needed. On 8/8/2022, I interviewed CCSM, Brianna Mapp. She works in the preschool room during the regular school year. She has an infant in the classroom and has never had concerns about her diaper not being changed. Diapers are changed as needed with bowel movements or when they are wet. She worked in the infant and toddler room for a few weeks. On 8/8/2022, I interviewed witness. Diapers are not changed on a schedule just 3 whenever the staff feels like changing them. Witness observed diapers wet heavily and children not being changed as needed. On 6/21/2022 and 8/4/2022, I observed two diaper changes. I did not observe diapers heavily wet or soiled diapers while I was present. There was no diaper changing log to document the diapers after they were changed. On 8/23/2022, I interviewed CCSM, Jennifer Peterson. She works as a substitute in the infant and toddler room. Her schedule is sporadic, and she works only when the classroom is short-staffed.

On 6/21/2022, I interviewed complainant. Children are left in highchairs for long periods of time. CCSM's do not let them down to play or tummy time. On 6/21/2022 and 8/10/2022, I interviewed Dr. Price. She has not had any complaints from parents regarding their child being restrained in highchair. She has observed children in highchairs for long periods that extend past the allowed time. Staff were spoken to about it and told no longer than five minutes to place a child in a highchair. Dr. Price stated there is no mice problem, but Orkin does treat the facility regularly. The children are not in highchairs for long periods because of mice, but because staff choose to keep them there. On 7/27/2022, I interviewed Child A's Mother and Child B's Mother. They have not observed children in highchairs for long periods of time. On 8/4/2022, I interviewed Ms. Graham. Children are not placed in highchairs for long periods of time. They are put there for feeding and back on the floor to play. On 8/8/2022, I interviewed Ms. Mapp. The children are only in the highchair when they are being fed. They do not sit longer than that. 4 On 8/8/2022, I interviewed the witness. Children are not allowed on the floor and stay in highchairs and cribs. Witness stated there are mice in the center and the children are placed in the highchair for long periods probably because of this. On 8/24/2022, I interviewed CCSM, Jennifer Peterson. She stated Dr. Price has a zero tolerance policy when it involves highchair usage. Dr. Price questions any staff member when there is a child in the highchair. She asks how long the child has been sitting when she observes a child sitting in a highchair. Ms. Peterson has never observed mice but has seen workers from Orkin in the center. She has heard other staff members comment about seeing a mouse.

On 6/21/2022, I interviewed the complainant. Complainant stated that caregiver to child ratio was out of control. There was not enough CCSM's to accommodate the children in the infant and toddler room. This has happened more than once. On 6/21/2022, I interviewed Dr. Price and Ms. Peterson. The classroom is fully staffed and there are no issues with ratio. Superior staffing assists with staffing needs when there are call offs. Staff are monitored accordingly based on the number of children present each day. Four new staff were recently hired. Several of the staff in the classroom have their own children in the classroom. No one has brought to their attention regarding the classroom not maintaining ratio. On 7/27/2022, I interviewed Child A's Mother and Child B's Mother. They have not had any concerns with the caregiver to child ratio. On 8/4/2022, I interviewed Ms. Graham. She said the caregiver to child ratio is maintained in the infant room. On 05/12/2022, I conducted an unannounced inspection at the childcare center. While there I interviewed licensee, Dr. Constance Price, assistant, Director, Jennifer Peterson, and CCSMs, Alazia Wright, Gabrielle Coady-Bey, Dashia Young, Alicia Turner, Jameela Lattin. Dr. Price said all childcare staff members have been fingerprinted. Those in supervised-only status must be supervised by other eligible CCSMs. The center is actively interviewing to fill positions. Dr. Price said she believes the source of the complaint is a disgruntled employee that was recently fired. Jennifer Peterson participated in the interview with Dr. Price but had nothing additional or contradictory to add. I requested that Dr. Price forward to me an updated staffing plan so that a comparison can be done with the fingerprinting 2 system. I reminded her to keep the system updated by way of disconnecting employees that no longer work for her. While at the center I interviewed CCSMs, Alazia Wright, Gabrielle Coady-Bey, Dashia Young, Alicia Turner, Jameela Lattin. All reported being fingerprinted and did not know of any other CCSMs that had not yet been fingerprinted. Ms. Wright said she never thought to ask who was printed and who was not. She believed they all had done as she did. On 06/17/2022 I reviewed the child care background check (CCBC) systems. It shows eight people in pending status and 18 connected and eligible CCSMs. There are no ineligible child care workers that are connected. I also reviewed the center's staffing plan and compared it to the CCSMs in CCBC.

On 05/12/2022, Dr. Price denied that ratio is an issue. She stated for the most part there is little to no groups of mixed-age children. On occasion, a toddler may go with their preschool sibling toward the end of the day for the ease of picking up. The center continues to hire more child care staff members as they have lost many during the pandemic. The center currently has eight GSRP CCSM and four infant/toddler CCSM totaling 12 child care staff members. Most of the children leave by 4 PM so there's no need to mix the children up. I interview childcare staff members, Alazia Wright and Gabrielle Coady-Bey. Both are child care staff members that work in the infant-toddler classrooms. Both denied that ratio is an issue. They denied that other ages of children are mixed with infants and toddlers. They both are aware that the ratio is based on the age of the youngest child in the mixed group. 3 Before leaving the center, I obtained contact information for parents for future interviews. There were no mixed groups of children observed. The center maintained an appropriate ratio for all ages. On 06/12/2022 I interviewed Child A's Mother and Child B's Mother over the phone. Both parents denied any concerns about the services being provided. They also denied any ratio concerns. Child C's Mother said they really don't come into the school building but believe that ratio is being met.

On 03/04/2022, I interviewed Child A's Mother by phone. She said she was very concerned when her daughter said her teacher. Ms. Hartman, choked her. Child A's Mother said she was getting her daughter ready for school, and she stated that her teacher Ms. Hartman choked her until she could not breathe. Child A did not say when this happened. Child A's Mother confronted Ms. Hartman who told her it did not happen. Child A's Mother stated, " I know my child lies but she keeps putting her hand around her neck." On 3/08/2022 1 made an unannounced inspection at the center We interviewed Constance Price who is the owner and program director of the center. She said on 03/02/2022 Child A's Mother came to the center with the complaint information and called the police. She told the police that Child A said Ms. Hartman choked her when they were alone in the classroom. Child A placed both her hands around her neck showing what had occurred. ~ Ms. Hartman does not have any disciplinary actions on file and Dr. Price reported Ms. Hartman's training including all policies and procedures of the school. She has a signed child abuse and neglect statement on file. During the visit, we interviewed Tawanna Hartman. She denied the allegations stating she would never do that to any child. She said she has a bachelor's degree and a 20-year history in childcare without incident. When disciplining children, she uses redirection. Ms. Hartman said on 03/02/2022. Child A was dropped off and came into the class like ordinary. When Child A was picked up, she did not say anything. The next day Child A's Mother asked her to come to the door in a text. Ms. Hartman said Child A's Mother walked through the door and informed her Child A said she (Ms. Hartman) had choked her. Ms. Hartman said she noticed Child A's Mother outside the classroom pacing as if she was upset and ready to start a physical altercation. Ms. Hartman said she called the center directors, Ms. Peterson and Dr. Price, and the police were called. Ms. Hartman tried to discuss the matter with Child A's Mother, but she was to upset. The police arrived at the school and took Child A outside and talked with her. The police came back into the building and brought Child A in front of her, and Child A hugged her and said she was not afraid of Ms. Hartman. Everyone was standing there, and Ms. Hartman stated she left the area and sat with Ms. Peterson. She denied the allegations stating she would never do that to any child. She has never had any issues with Child A's Mother or Child A. She has never observed any injuries or bruising on Child A. A Safety Plan was discussed with Dr. Price. Twanna Hartman will be supervised by a coworker that is not involved with the investigation while working with children in the child care center. On 03122/2022 On 05/12/2022, an unannounced, in-person inspection was conducted at the center. Dr. Price informed me that Child A continues to attend the center without incident. She also stated Ms. Hartman is no longer working at the center. I told her I was recommending no change in the status of the license. 4

On 10/07/2020 I interviewed the program director and licensee, Constance Price over the phone. She verified Child Care Staff Member 1 was employed at the center as a lead caregiver. I shared with her that further investigation of Ms. Davis would need to be conducted. The center had another ongoing investigation that would first have to be closed. On 12/01/2020 Licensing consultant, Lawanda Washington, completed and closed her special investigation. On 01/08/2021 I attempted an on-site interview with Ms. Davis. I called while outside the center to make sure they were open and that Ms. Davis was on-site. Dr. Prince 2 said Child Care Staff Member 1's last day of work was 10/22/2020. She left due to "personal issues". She is not longer employed at the center.
