On 7/17/2023 and 9/5/2023, I interviewed licensee, Laurie Clark by phone. On 5/24/2023, Ms. Clark stated that Child A’s (1-year-old male) Mother informed the center of scratches on Child A from 5/23/2023. She arrived at the center with Child A’s Grandmother and Great-Grandmother requesting a meeting. No one was aware of Child A being scratched, but an incident report was written 2 The Child Care Organizations Act allows the Child Care Licensing Bureau the ability to utilize available information, including, but not limited to, any of the following: (a) Investigative report, such as a law enforcement report and a children's protective services report. (b) Medical report. (c) Public record. (d) Child care center, group child care home, or family child care home record. (e) Inspection of the child care center, group child care home, or family child care home, as well as criminal history information to determine whether an individual is conducive to the welfare of children. Based on my interview and review of allowable information, the Child Care Licensing Bureau finds that the disposition, temperament, condition, and actions of CCSMs does promote the safety and well-being of children in a licensed child care setting. The Child Care Licensing Bureau finds that CCSMs are conducive to the welfare of children. On 7/25/2023, I interviewed Child A’s Mother. On 5/23/2023, Child A’s Mother picked Child A up around 5:25 p.m. On her way to the center, she called to discuss her co-payment. She arrived and was informed Child A was in the back with the administrator. She stated that she waited in the front for about five minutes and walked to the back. Once she walked in the back, she was informed that Child A was agitated because he was the only child left in his class. They were not aware that Child A was bit and scratched. Child A’s Mother and Child A left the center and were walking to the store. She saw what appeared to be a bite mark and scratches on Child A. Once they were home, she put Child A in the bath and noticed a nickel size mark on his back. Child A wore a sleeveless top and shorts, but no one informed her he was bit. She immediately called the center and told them about the bite marks and scratches. She spoke to three different people, and no one could tell her what happened. No one knew what happened and continued to tell her they would investigate it and find out what happened. Child A’s Mother stated that she spoke with Ms. Clark and she informed her they would attempt to find out what happened. She stated that Ms. Clark never called her back. Child A’s teacher was gone for the day. Child A’s Mother was informed that they would need to find out the next day what happened to Child A. On 5/24/2023, a meeting was held which included Child A’s Mother, Grandmother and Great-grandmother. The meeting was held with Ms. Clark because she never called her back regarding the bite mark and scratches. She was informed that Child A was bitten by another student in class a known biter. She was told that Child A would only be with CCSM 1 and CCSM 2 from this point on. On 7/12/2023, Child A was dropped off at the center. Within an hour of being dropped off, she received a phone call from the center that Child A was bit again. She was informed that Child A was bit on the forehead after entering a tunnel with another child. She stated that the center tried to blame him for the cause of being bit. Child A was playing basketball and walked over to the tunnel. Child A’s teacher was holding him when she arrived. Child A’s Mother waited for CCSM 2 so they both could let her know what happened. Child A’s Grandmother asked about an accident report and they did not have one. She decided that she was not bringing Child A back to the center after this incident. Child A’s Mother stated that 4 she filed a police report after leaving the center. On 8/15/2023, I interviewed Child B’s (2-year-old female) Mother. She has no problems with the center. Child B’

On 7/17/2023, I interviewed Ms. Clark. Child A is fed when all meals are provided. He is not being deprived meals. On 7/17/2023, I interviewed Ms. Hayes, CCSM 1 and CCSM 2. Child A is being fed at mealtimes. He eats when meals are provided to the children. On 7/25/2023, I interviewed Child A’s Mother. She stated that Child A arrives home hungry each day she picks him up as if he has not had anything to eat. On 8/15/2023, I interviewed Child B’s Mother and Child C’s Father. They do not have any concerns with the meals that are being served to their children.

On 7/28/2022, I interviewed Child A's Mother. On 7/25/2022, she arrived around 2:30 p.m., rang the doorbell and announced she was picking up Child A. Child care staff member (CCSM), Shantel Booker pushed Child A out the door and tried to hurry and close the door. Child A's Mother pulled the door and asked 2 about her day and why Child A was looking the way she looked. Ms. Booker stated she just woke up. CCSM, Stephen Petties was walking by and stopped and noticed Ms. Booker looking through the door and he said he would talk to Child A's Mother later. He was looking as if he wanted to tell her something but changed his mind after seeing Ms. Booker standing in the door. Once in the car she showed Child A the dresses she purchased for her. She noticed that her bottom lip was swollen and busted after she lifted her head up to look at the dresses. She asked what happened to her lip and Child A said she was locked in the bathroom. Child A then stated she hit it on the door. Child A's Mother asked her again, if someone hit her and she told her Ms. Booker hit her for being bad. care knowledge regarding Child A's daily routine. I interviewed program director, Cassandra Hayes. She said no incidents were reported to her regarding Child A. Child A enters the center with a mask on and removes it to eat. Someone would have noticed a busted lip on her at some point during the day. Child A arrives around 8:301 a.m. and her teacher signs her in and out at the end of the day. It is never the same staff person to greet the child at the door and they are taken to their classroom. The children are observed for obvious scrapes and bruises before the parent leaves. The child's teacher examines the child again, for bruises once in the classroom and makes note of any observations. They do not check under their clothes. She is not aware of abrasion under Child A's shirt. Child A mask was removed for lunch, and she would have said something while eating. The juice from the fruit she ate would have irritated her cut lip. Someone would have noticed her lip busted during mealtime. Ms. Hayes said the injury would have been addressed, Child A's Mother would have been contacted and an incident report would have been provided. The children do not go to the restroom alone. They can open the door from the outside if a child is locked inside the stall. The staff does not go in the restroom with the children but stands outside the door. The children wake up around 2:30 p.m. from nap and the entire class is taken to the restroom. The children are taken back to their classroom, given snack and then outside. The parents arrive and pick up from the play area or parking lot. Parents are not allowed in the building. Ms. Hayes saw Child A when she left and did not observe any noticeable bruises on her. A photo of Child A's lip was shown to Ms. Hayes, and she denied seeing the bruise on Child A while she was at the center. The photo was provided by Child A's Mother. I reviewed video footage of Child A during lunch time, gross motor, naptime and her exiting the building. She was skipping around the room and received a toy from Ms. Booker and joined other children at the table. Child A was doing pushups with the other children on the carpet. At 11:09 a.m., the children were sitting at the table eating lunch. Ms. Booker passed out lunch and a student helper sat near Child A. Child A's mask was pulled under her chin while she ate. At 11:26 a.m., the children were on their cots. At 2:23 p.m., Child A was taken to the door near the parking lot and left the parking lot at 2:26 p.m. She was holding a white bag in her hand swinging it until her mother took it away from her. On 7/28/2022, I interviewed (CCSM), Michelle Thomas. She observed Child A in the parking lot. Child A's Mother was showing her clothes that were in a bag. They were outside for about five or ten minutes in the parking lot. Child A was excited. She did not observe any injuries on Child A while she was at school.

On 7/28/2022, I interviewed Child A's Mother. She said Child A is treated differently than other children. On 7/22/2022, the children went on a field trip, and she returned with a "crusted" top lip. No one told her what happened. Child A told her that Child B hit her. Child A's Mother asked if she told her teacher, and she said her teacher told her to hit the girl back. Child A said they continued to hit each other but no one ever stepped in to stop them. Child A initially told her mother that she hurt herself on a blue chair while on the field trip. On 7/28/2022, Ms. Hayes stated no one reported any incidents to her during or after the field trip. Ms. Hayes attended the field trip in place of Ms. Booker to Belle Isle. She also worked in Ms. Booker's classroom and Child A did not have any injuries. She initially stated that Child A did not go on the field trip. When asked a second time, she stated Child A went to Belle Isle, but no injuries occurred while they were there. She has not heard any child care staff members tell a child to hit a child back after being hit. Child B did not attend the field trip and has not been in attendance in over a month. 5 On 7/28/2022, Ms. Thomas stated Child A did attend the field trip to Belle Isle. Ms. Booker did not work that Friday and Ms. Hayes filled in. There were no incidents that occurred during the field trip. On 7/28/2022, Ms. Booker did not work on the day of the field trip. She stated Child B has not been to the center in over a month.
