I initiated this investigation on January 28, 2022, via telephone interview with Nicole Gibby, Licensing Consultant. Ms. Gibby reported that an incident report was received regarding this incident. On January 28, 2022, during a telephone interview, Pastor Roy Hill said that the child care staff member that was involved in this incident, Unique Jackson, — On January 25, 2022, Child A urinated on herself getting her blanket wet during nap time. Ms. Jackson asked Child A to give her the blanket, but Child A was playing and pulling the blanket away. Ms. Jackson and Child A was doing a "tug-of-war" with the blanket when Ms. Jackson let go of the blanket. This caused Child A to fall back hitting her head on a table. Pastor Hill added that a call had already been made to Child A's Mother due to her clothes being wet. Child A's Mother's boyfriend, arrived for pick up within a minute of the incident occurring. Child A's head wa"sti getting assessed when he walked into the child care center. Child A's Mother returned to the child care center acting "irate". Pastor Hill had to call the police to get her to leave the premises. Pastor Hill also called EMS to examine Child A. EMS determined that the injury was not "deep" and advised Child A's Mother to apply ice. On February 9, 2022, during an unannounced site visit, I interviewed Tabitha Wilcoxson, assistant administrator. Ms. Wilcoxson explained that the lead teacher, Carneisha Ranson, was not present today. She said that Ms. Ranson was on lunch break when this incident occurred, and Ms. Jackson was relieving her. I observed 3 the classroom, and the tables present in the classroom. The tables were the standard "U" shaped tables commonly used in preschool classrooms. On February 11, 2022, Angela Shy, program director, said she was present in the child care center on date in question, however, she was not in the classroom and did not witness the incident. She was told by Pastor Hill that the child and Ms. Jackson were playing "tug-of-war" and Child A fell and hit her head on a table. She said Child A was no longer enrolled and that Ms. Jackson's On February 11, 2022, Carneisha Ranson was interviewed via telephone. Ms. Ranson stated that she was the lead teacher in Child A's classroom. She said she was on her lunch break when the incident occurred. When she returned from break, she was told that Child A injured her head and went home. Child A did not return to care after this incident. The teacher that was involved, Ms. Jackson, _ Child A's Mother upon her arrival to Kids In Zion Childcare Center. The staff could be heard being "very rude" and would not allow her into the building. On April 22, 2022, Unique Jackson, former childcare staff member was interviewed via telephone. Ms. Jackson denied slamming Child A. She reported that due to the center being short staffed, she was asked to go into Ms. Ranson's class to relieve her for a lunch break. Ms. Ransom had informed her that Child A was not asleep 4 and had just used the restroom. Shortly after Ms. Jackson arrived in the class, Child A asked to go to the bathroom. Because she knew that Child A had just went, she asked Child A to wait because she could not leave the other children in the class alone. Child A then returned stating, "I peed on myself'. Child A urinated on her clothes, the cot and the blanket. While cleaning the area, Ms. Jackson asked Child A to give her the wet blanket. When Ms. Jackson grabbed the blanket, Child A laughed and pulled it back away from Ms. Jackson. Ms. Jackson let go of the blanket and Child A fell backwards. She asked Child A if she was "Okay". Child A responded yes and there was no crying or a scream that indicated Child A had been harmed. Ms. Jackson continued to clean the area and called for help to take Child A to the bathroom to change clothes. Ms. Shy came in, however, there were no clean clothes in Child A's cubby, so a call was made to Child A's Mother to come pick her up. At that time, Ms. Jackson w

On January 28, 2022, Pastor Hill said that Child A's injury happened within minutes of her being picked up. The incident was explained verbally to Child A's Mother's boyfriend. It was explained that a written report was forthcoming. Pastor Hill said that when an accident occurs at the child care center, the staff will access the injury, treat the wound, contact the parent and follow up with a written report. An incident report was completed and forwarded to Ms. Gibby. On February 11, 2022, Angela Shy, program director, said per their policy, parents are notified when an injury occurs. They also complete a written report that the parent signs. An incident report was written regarding Child A's injury and placed in her file, as Child A did not return to care. On February 11, 2022, Carneisha Ranson said since she was not present, she is not certain if an incident report was given to Child A's Mother. However, per the center's policy, when a child is injured, the parents are contacted via telephone and a written incident report is completed.
