On 10/13/2022, I conducted an onsite investigation at the center. I interviewed the Licensee and Program Director, Ms. Elizabeth Ojo. She stated that on 10/07/2022, Child A's Mother brought a letter to her informing her that the Michigan Department of Health and Human Services (MDHHS) payments for child care was closed. She informed Child A's Mother that payments still would be required, and she would have to figure out the payments. On Monday, October 10, and Tuesday October 11, Child A arrived at the facility and stayed all day. Child A's Mother is the type of parent that looks the child over before leaving the facility. She had complained that Child A's pants were dirty from crawling. If anything is out of order, Child A's Mother would complain. On 10/10/2022, around 8:27 p.m. Child A's 2 Mother sent an email, but she did not see it until the morning of 10/11/2022 around 9:36.p.m The email from Child A's Mother had a picture of Child A's face, and she complained about a scratch being on Child A's face. She stated that on the morning of 10/11/2022, when Child A was dropped off at the center, Child A's Mother accused everyone at the center. All the CCSMs informed her they were unaware of the scratch. Child A's Mother still left Child A at the center for the remainder of the day. Ms. Ojo spoke to Child A's Mother later and informed her that no one was aware of the scratch, and that it may have been self-inflicted, done while Child A was playing, as she is very active. I observed the picture of Child A, the scratch was barely visible under the nostril, and appeared to be self-inflicted. The scratch was not noticeable unless it was pointed out, due to the size. While onsite on 10/13/2022, I interviewed the Child Care Staff Members (CCSM) from the infant and young toddler room. CCSM Sojourney Jones, stated this is the first incident that she has ever heard of with Child A. Child A's Mother if very particular and doesn't even want Child A to crawl on the floor or interact much with other children. They attempt to isolate sometimes, but Child A is very active and likes to interact with the other children. She was not aware of the alleged scratch on Child A on 10/10/2022, therefore she could not write up an incident report. CCSM Deann Weaver stated she never saw a scratch on Child A's face. She stated that someone is always near Child A to prevent her from grabbing things and falling, because they know Child A's Mother complains about everything. Child A has been learning to walk, and sometimes she falls down. Child A's Mother complains if her pants get dirty. She recalls asking Child A's Mother to trim Child A's nails and she thinks that may be how Child A got the scratched. No incident report was written because she was also unaware of the scratch and doesn't think it was there when Child A left the center. I called Child A's Mother on 10/12/2022 and left a voice message. I contacted her again on 10/20/2022. She stated that she dropped Child A off at the center and when she picked Child A up, she saw a scratch on her face. On 10/11/2022, she asked the staff members what happened, and no one could tell her what happened. She stated that she is tired of getting the same excuses and thinks that they must not be supervising the kids because they never know what happened to her. I asked Child A's Mother if Child A had long fingernails. She stated yes, and that the CCSM keeping using the nails as an excuse. Child A's Mother did not believe that the scratches on Child A could have been self-inflicted. She stressed that she has never received an incident report when Child A has gotten a scratch on the face. I informed Child A's Mother, that if the staff did not see the scratch, then they would not complete an incident report because they were not aware of it. Child A did not return to the center after 10/11/2022. 3 I called Child B's (2-year-old, male) Mother. She stated that she has never had any concerns or problems at the center. Child

On June 17, 2022, I initiated this investigation with an unannounced on-site inspection. Childcare staff members Toni Anderson and Zena Turner were present. Ms. Turner explained that Elizabeth Ojo, program director was not present, but she called Ms. Ojo to inform her that I was present. Ms. Turner and Ms. Anderson were interviewed separately but remained in the classroom due ratio confinements. Ms. Turner denied that children are treated mean at the childcare facility. She has never observed children being treated bad by any staff member. She also denied that children are sent home unkept. She said that if children get dirty from playing outside or a painting activity, they do their best to clean them up. On June 17,2022 Toni Anderson also denied that allegations. Ms. Anderson reported that none of the children are being mistreated in care. Ms. Anderson said that she has worked at this facility for 5 years and has never seen or heard of any child being treated wrong. She said that children are not intentionally sent home dirty. None of the parents have complained to her about children being dirty at pick up. On June 17, 2022, Child A's (4-year-old, Male) Mother was interviewed on site. She said that Child A has been in attendance since February 2022. She has no concerns and is pleased with the care being provided. She stated that her children "love it here!" She has 2 children enrolled. On July 28, 2022, Child B's (1-year-old, Female) Mother was interviewed via telephone. She stated that she is not aware of children being treated mean or going home dirty. However, she is concerned with the center being short staffed and the turn over rate. Child B's Mother is currently looking for other child care.

On 8/27/2020 1 interviewed complainant. On 8/17/2020 complainant stated that Child A -year-old } had a red mark on chin at pick up. On 9/1/2020, 1 interviewed licensee, Elizabeth Ojo. Ms. Ojo stated that the children were dancing to an activity on Youtube and Child A was accidentally kicked. Ms. Robinson informed Ms. Ojo that Child A was given paper towel wrapped in ice to hold on face. Ms. Robinson did not observe a bruise on Child A's face. Ms. Ojo stated t a Ms. Robinson did not complete an accident report form because she stated there was no bruise. Ms. Ojo stated that she was not aware of the small amount of blood in the corner of Child A's mouth. She stated that she would have asked Ms. Robinson to complete an accident report form for Child A's Mother. Child A's Mother was informed of the incident in person. Ms. Ojo stated according to the centers policy, the parent is notified when a child is injured. She stated that she informed Ms. Robinson in the future when a child is injured, she must complete an accident report form even if there is no bruise. Ms. Ojo stated that Child A's Mother contacted her about the incident later that evening. She brought Child A back the following the day. There were no other concerns regarding the incident. On 9/1/2020, 1 interviewed Ms. Robinson. She stated it was time for pickup and she walked a child to the door. She walked back to the children and Child A was holding face. She asked ■ what was wrong, and Child A continued to hold ■ face. s. Robinson stated that she observed a small amount of blood in the comer of Child A's mouth. She wrapped ice in a paper towel for Child A to hold on face. Child A put the ice down a couple minutes later and went back to play. C t A's Mother arrived shortly after and asked about a red mark on~ face. Ms. Robinson stated she informed her the mark was not on ■ ace when she checked. 3 Ms. Robinson stated that Child A's Mother was verbally informed that she assisted Child A after she saw■ holdings face even though she did not observe what happened. Ms. Robinson informed Ms. Ojo of the incident as well.

Complainant stated that Ms. Ojo informed her that she picked Child B up and took in her office because was out of control. MS. Ojo informed her that she was trying to calm down because was having a tantrum. She stated that she did not whoa with a stick_ Comp ainant stated that she asked Child B what happened, and ~m armed her that was kicking■ feet and kicked someone. On 9/1/2020, I interviewed Ms. Ojo. Ms. Ojo stated that devotion time is in the morning with the children singing, dancing and prayer. She stated that the children are not yelled or screamed at. Ms. Ojo stated that Child B is She stated that on the day in question, Child B was hyperventilating, uncomfortable and did not want to lie down on cot. She stated that she picked Child B up and took in the office to calm down. She asked if■ had a and c ecked 4 jL'M She walked back and proceeded to give a MerChild B's ,. fell off to sleep. Ms. Ojo s a ed that d B was not taken in the office or kickina another child. She stated that she has never r hands on Child B or any child. On 91112020, 1 interviewed Ms. Armstead. She stated that she has never put her hands on any children. She stated that she has never screamed or yelled at Child B for being hyper or yelling out. She stated that she has never slammed Child B to the floor. Ms. Armstead stated that when she has to discipline a child, she has the child sit next to her. Ms. Armstead stated that devotion time is between 9:00 a.m. and 10:00 a.m. There is movement and prayer during this time. M On 9/4/2020, 1 interviewed Child D's -year-old -) Mother. Child D's Mother stated that she was pleased with the center. Child D has been enrolled at the center for over a -. She stated that Child D loved ■ teachers and going to the center every day. Child D stated that she is glad she found the center. -year 0 On 914/2020; 1 interviewed Child E's -old Mother. Child E's Mother -) stated that the child care staff members work with Child E and take their time when assisting . She stated that Child E listens to them. Child E's Mother stated that Child E has been enrolled at the center for years and she loves everyone there.
