On 01/19/2022 I initiated the investigation by calling Child A's Mother to conduct an interview. Child A's Mother also has a set of twins who are 5 years old that are enrolled at the center. Child A's Mother said on 01/18/2022, she received a telephone call from the center asking her to pick up her twins because they were throwing chairs and acting out of control. Child A's Mother told the center that she would come to the center as soon as possible. When Child A's Mother picked up her three children, she put their coats on, signed them out and left the center. When she left, she noticed a scratch on Child A's face. Child A's Mother said her children are nonverbal so they could not explain what happened to Child A's face. Child A's Mother said she called the center and spoke with program director, Ms. Rose Gulley. She asked Ms. Rose what happened to Child A's face. Ms. Rose 2 told Child A's Mother that she would ask the child care staff members that were in the classroom and call her back. Child A's Mother said Ms. Rose called back 15 minutes later and reported that Child A fell. Child A's Mother said she asked Ms. Rose, "why didn't anyone tell me Child A fell when they called me about the twins?" Child A's Mother said Ms. Rose responded, "the child care staff members said they didn't have time to write a report." Child A's Mother said she became very upset and decided to disenroll her children. On 01/21/2022 I completed an onsite inspection. I interviewed child care staff member, Yolanda Smith. Ms. Yolanda was in the classroom on 01/18/2022 when Child A was injured. Ms. Yolanda said on 01/18/2022 the children were playing while dancing to music, and Child A fell. She said she applied an ice pack and put first aid A & D Ointment on the scratch and placed a band-aid on it. I asked Ms. Yolanda if an incident report was written or if the parent was notified. Ms. Yolanda gave me a "Boo Boo Report." The Boo Boo Report stated that at 11:30am during play time, the children were playing with toy cars and listening to music. It further read that a certain song came on and the children started running. Child A slipped on a car that was on the floor and scratched her face. I asked Ms. Yolanda if the parent received the Boo Boo Report. Ms. Yolanda said no. Ms. Yolanda said Child A's Mother came in quickly and left without receiving the report. Ms. Yolanda was not sure if Child A's Mother was called. I asked for the sign in/out sheet for Child A's Mother that was dated for 01/18/2022 as well as the Parent Handbook indicating the center incident and injury policy. I received the documents and left the center. The center's policy for when a child incurs a minor injury, staff will take the following steps: Immediate care is provided to the child. A phone call, written report or both will be provided to the parent at or before dismissal on the day of the minor injury. On 01/27/2022, I completed a telephone interview with Ms. Rose Gulley, program director. I talked to Ms. Rose about her conversation I had with Child A's Mother. Ms. Rose said she never told Child A's Mother that the child care staff members didn't have time to write the incident report. Ms. Rose said the report was written on the day of the incident and agreed that the parent should have received the report on 01/18/2022. I asked for the contact information for child care staff member, Jada Smith who was the other child care staff member in the classroom on 01/18/2022. On 01/27/2022, I attempted to contact Ms. Jada Smith to conduct a telephone interview. I did not reach Ms. Jada Smith. On 02/01/2022, I completed an onsite inspection at the facility to interview child care staff member, Jada Smith. I asked Ms. Jada to tell me about the incident on 01/18/2022 to explain what happened to Child A. Ms. Jada said the children were playing and listening to music. Ms. Jada said a certain song came on and all the children jumped up and started running. Ms. Jada said a lot of the children
