Child A (3-year-old, male) attends Harper Gratiot. The center receives their food from Harvest Food Trucks. On 12/13/2022, Child A vomited due to eating outdated barbequed chicken for lunch. When Child A leaves the center, he is hungry, his shoes are not tied, and he has a runny nose. The school does not allow parents or outside individuals to enter the school. There are no windows in the school or air conditioners to keep the school cool. The inside of the school is abnormally hot inside. 5

On 12/14/2022, I spoke to Child A's Mother regarding the allegations. Child A started attending Harper Gratiot in September 2022. Child A is 2 in classroom 2. He has been sent home three times for (i.e., cough, COVID and he threw up in sleep). Child A's Mother took Child A to urgent care, and he was fine. They stated that the food may not have agreed with him. Child A's Mother kept him home one day to make sure he was fine. The center has precautious in place due to COVID-19. Child A's Mother stated that she has no concern with the center food or the care that is provided to Child A. The center provides the children with breakfast, lunch and snack each day. Child A has a running nose, but Child A's Mother stated it could be because they take the children outside twice a day while at the center. Child A's Mother stated that she understands that children get sick. Child A will continue to attend the center, he would be returning on 12/15/2022. On 12/15/2022, I conducted a telephone interview with the complainant. The complainant stated that he is a concerned relative of Child A. He picks up Child A from the center and he watches him when he is sent home. The complainant did not understand why Child A has been sick three times since September 2022. He stated that Child A had green snot coming from his nose, someone in the classroom was positive for COVID and then he threw up in his sleep. He stated that Child A's Mother was not concerned and planned on returning Child A to the center, but he wanted someone to look into this. On 12/15/2022, I completed an unannounced inspection at Harper Gratiot. I spoke to Roxanne Campbell, licensee designee, Madeira Clark, family service worker, Beatrice Rudd, Diane Green, child care staff members (CCSMs), and Gloria Soloman, food service worker. The entire center was observed. In classroom 1, there were 13 children, ages 2 to 5 years, with two CCSMs. In classroom 2, there 1/2 were 14 children, ages 2 to 5 years, with two CCSMs. In classroom 3, there were '/2 10 children, ages 2 to 5 years, with two CCSMs. In classroom 4, there were 13 '/2 children, ages 2 to 5 years, with two CCSMs. In classroom 5, there were seven 1/2 children, ages 2 to 5 years, with two CCSMs. The kitchen was observed at the 1/2 center. There was a thermometer on the refrigerator that read 38 degrees. The thermometer on the freezer, read -3 degrees. There was no spoiled food or outdated food observed at the center. The following information was provided for review: child information cards, menu, classroom 2 temperature log, Child A's medical release, and the center health guidelines. According to Ms. Campbell, she was aware of the complaint. The children are not served outdated or spoiled food at the center. The food that is served to the children is purchased from Atlas Wholesale Food Company and Sysco Detroit food supplier. The food is delivered weekly to the center. The food is prepared on site by the food service worker, Ms. Soloman. According to Ms. Soloman, she has been working at the center since September 2022. However, she has been a food service worker for the past 30 years. On 12/13/2022, the children were served barbeque chicken, mixed vegetables, dinner rolls, and applesauce. Ms. Soloman reported that the children loved the meal. The menus are planned, and the center accommodates children with allergies. The 3 chicken was ordered from one of the food vendors, Atlas or Sysco. The chicken came into the center on 12/07/2022. All the food is delivered to the center from the food vendor company. Refrigerated items go into the refrigerator and items that were frozen are put into the freezer upon arrival. I asked Ms. Solomon if the children are served outdated food from Forgotten Harvest. She stated, "No, the children are only served food that is purchased from the food vendors. The center receives food from Gleaners, which is offered to parents that need the food. On 12/14/2022, I spoke to Child A's Mother regarding the allegations. She stated that she has no concerns regarding the center. She was informed at enrollment that the parents were not allowed in the center. On 12/15/2022, I conducted a telephone interview with the complainant. The complainant provided the same information stated in the allegations. On 12/15/2022, I completed an unannounced inspection at Harper Gratiot. The center has six classrooms but there were five operating. Each classroom was observed. I observed windows near the top of the ceiling in each room. The windows did not open. In each of the classrooms, they recorded the room's temperature each day. The temperature on the thermostat was set at 65 degrees but the classroom temperature was different. The temperature in classrooms 1 and 2 was 78 degrees. The temperature in classrooms 3 and 5 was 72 degrees. The temperature in classroom 4 was 68 degrees. There was a portable air conditioning unit in each classroom. According to Ms. Campbell, parents are not allowed into the school since COVID-19. Parents are explained the no-entry policy at enrollment. The children are received from the car and taken to their classroom. The center building has high ceilings and there are windows in each room that allow light into the rooms. The windows do not open but they have been inspected and approved. Ms. Campbell stated that the center was hot in October 2022 due to the heat being turned on at the building. They called the landlord about the building being overly hot. The issue was taken care of and addressed. There are thermostats in each classroom to ensure the temperature is safe and comfortable. According to Ms. Rudd and Ms. Green, classrooms 1 and 2 are the warmer classrooms in the building. The classrooms are connected and share a thermostat. They monitor classroom 2 temperature and document it on the log daily. They are aware of the classroom temperature exceeds 82 degrees, measures shall be taken to cool the children. They have a portable air conditioning unit in their classroom. On 12/15/2022, I reviewed the center's initial environmental health inspection report completed on 07/08/2005 by Paul Max. The report states there are no exterior windows, but artificial lighting meets the requirements for illumination. There are no operable windows in classrooms 1, 2, 3, 4, 5, and 6, but the rooms are ventilated mechanically. A letter from a licensed mechanical contractor certifies that 4800 cfm of outside air is supplied to the proposed childcare areas through a central air conditioning unit. The fans on the central air conditioning unit must be in constant operation while the children are in the building. The environmental health inspection from 07/28/2022 by Paul Barry, states the ventilation at the center complies with licensing rules. On 01/25/2023, I spoke to Child B and C's Mother and Child D's Mother regarding the allegations. They stated the children have no concerns regarding the A temperature of the classroom. Their children have not mentioned that their classrooms are too hot. Technical assistance was provided to the licensee designee, to ensure that a plan is developed to allow parents to enter the building to observe their children if requested.

On 01/31/2022, 1 conducted an unannounced onsite investigation and interviewed the program director Betty Reed, licensee designee Roxanne Campbell and Lance Reed, property operations manager. They all admit there was no heat in the building when they arrived at approximately 07:30a.m. that morning. The building was "freezing." Families were immediately called and informed the building would be closing for the day. Some families could not be reached by telephone and may have been enroute. Staff were made to stay in the building to ensure all families were notified of the situation, especially those who could not be reached by phone. According to Mrs. Reed, staff were not in the building for no more than an hour or two. Mr. Reed begin working on getting the heat restored and learned the boiler had been turned off the night before. It's believed the grounds maintenance staff were working in the boiler room and someone accidently hit the switch. Due to the size of the building it took some time to reheat it once the unit was turned on. 2 Mr. Reed has put measures in place to prevent anyone from accessing the room without his knowledge or permission. The heat was restored later that day. During the onsite investigation, I inspected all licensed classrooms and found them to be warm. The thermostats were above 65 degrees and the children and child care staff members present did not have any complaints or concerns when I casually asked them if they were ok.
