I conducted an onsite visit to the center on 04/05/2022. At the time of my arrival, I interviewed the Program Director, Ms. Yolanda Parson, the Licensee Designee, Ms. Angela McDaniel, and two Child Care Staff Members, Daniell Kendrick and Shandra Snow. Ms. Parson stated that she was unaware of any incidents occurring at the center involving Child A on 03/24/2022. Child A's Father and Mother called the center on 03/28/2022 stating that Child A received a black eye while at the center on 03/24/2022. Child A was not at the center on 03/25/2022, nor did they receive a phone call inquiring about an incident/accident. Ms. Parson stated that Child A enrolled at the center on 02/28/2022. At the time of his enrollment, it was noted and entered in the Child Plus System that Child A had discoloration of his left eye. When Child A's Mother called and questioned them about his eye injury on 03/28/2022, she thought Child A's Mother was referring to the same discoloration that was noted when he initially enrolled. It was stated that Child A's Father also called and threatened them to allege that someone at the center had caused the injury to Child A. Child A's Father also took Child A took the hospital for medical attention on 03/28/2022, and Child A's Mother filed a police report on 03/31/2022. Ms. Parson and Ms. McDaniel also stated that a Detroit Police Detective also came to the center on 04/05/2022 and was downloading all the camera footage for the day in question. The Detective showed them a picture that he was given of Child A's face. Ms. Parson stated that when Child A left the center on 03/24/2022, Child A did not have the bruise on his face that was in the picture. An incident was not reported at the time because there was no incident to report. They completed the incident report, after they were accused by the parents that Child A was injured at the center. I interviewed Child Care Staff Member, Daniell Kendrick and Shandra Snow. They are the assigned CCSM for Child A. They both stated that nothing happened to Child A while in care at the center on 3/24/2022, nor did he have a black eye when he left the center. Ms. Snow stated that nothing appeared abnormal about his eyes, and that his eyes always look sleepy and dark underneath. Ms. Kendrick stated his eyes were the same as every day when he comes and leaves, looking as if he is tired. When I asked both CCSMs about the allegations that someone informed Child A's Mother that Child A had fallen on a toy and injured his eye, they both stated that the information was not correct. No one made the statement to the mother, and they had not spoken to Child A's Mother. On 04/05/2022, I called Child A's Mother. She stated that Child A attended the day care center on 03/24/2022. She arrived at 3:15 p.m. to pick him up. She noticed something on his eye and asked Ms. Ineshia Bell, she referred to her as the advocate, what was wrong with Child A's eye. Ms. Bell responded and said, 3 "Remember his eye has always been like that." She said when she got home, she called the center to ask again about his eye. She stated she took a picture immediately of his eye. I asked if she took Child A to the doctor, and she stated that she took Child A to doctor on Saturday 03/26/2022. On, Monday, 03/28/2022 she stated she called the center and spoke to Ms. Parson and told her that Child A had a black eye at pick on Thursday, 03/24/2022. She was informed that an internal investigation would be conducted to see what could have happened. Child A's Mother stated that she notified the Detroit Police Department and filed a complaint. While on the phone with Child A's Mother, I asked her to send me the pictures that she took of Child A's face on 03/24/2022 after she arrived at home. She forwarded me the pictures. The pictures were dated stamped for Friday, 03/25/2022 at 7:15 a.m. There was a deep purple bruise to Child A's left eye.
