On 11/16/2022, Child A (4-year-old, female) ran out the center and was found approximately two blocks away in a police car. On 11/22/2022, the department received an email notification of Child A having ran from the classroom and not being located for approximately 10-15 minutes.

On 11/30/2022, 1 interviewed Child A's Mother. She has never had any problems with the facility but is concerned about Child A having ran out the 2 school. She admits Child A has run out of the home before, and it was not reported to the facility. She wants to know what the school will do to prevent any future occurrence and is waiting to meet with school officials. Child A's Mother was immediately called and notified about the incident and again when she was found. She does not recall who contacted her or the time but remembers Child A was in police custody and had to be picked up by a family member. Child A has returned to the facility and a safety plan is in place. On 12/01/2022, I conducted an unannounced onsite inspection. I inspected the classroom and adjacent refrigerator space. I was able to briefly interview childcare staff members (CCSMs) and drove the route Child A allegedly ran before she encountered the Detroit Police at Rex and Manning. She did not cross any major intersections as the school sits in the middle of a quiet residential area. The posted speed limit is approximately 25 miles per hour. According to reviewed attendance records, there were eight children and one childcare staff member present at the time of the incident. A second CCSM had left for a morning break. I also requested and received written statements from involved CCSMs. Childcare staff member (CCSM) Malita Clark states, Child A was having some problems adjusting to school and often misses her brother who's enrolled in a kindergarten class at the school. She repeatedly asked to go visit with him and begin crying when told "no" and wanted to go home. Child A continued to cry and Ms. Clark rubbed her back while agreeing to take her to visit her brother's classroom later. She stopped crying and was encouraged to play with other children. Ms. Clark turned her back and begin putting items in the adjacent refrigerator closet when she heard the door slam. She turned around, looked in the room and saw Child A had left. She yelled out Child A's name and went in the hallway looking for her. Ms. Clark did not see her and begin calling out to other CCSMs for assistance. She also notified the assistant principal's office via her cellular phone. Child A was called over the school's intercom and building staff were told to lookout for her. The school's security cameras were also viewed, and Child A was observed leaving out of door #8, which is close to her assigned classroom. The video is not available for review. Therefore, the time she left the building is not known but Ms. Clark believes it was approximately 9:15 am and she was found shortly before 09:30 am. CCSM Ms. Reanetta Harris-Scott was working in the classroom the day of the incident but had left for a morning break. She was gone for "maybe" five minutes when Ms. Clark called at approximately 09:25 a.m. to tell her Child A ran out the building. Mrs. Scott returned and began looking for Child A. She does not remember the exact time but within a few minutes of her returning, she was told Child A had been located and was safe. According to Mrs. Harris-Scott, Child A was having a "pretty good day" before she left for break. There was no indication of her having any problems that would lead to running out the building. Ms. Stathopoulos is the pre-k teacher in the classroom next door. She left her classroom after learning Child A ran out the building and drove around looking for 3 her. Approximately two blocks from the school, she observed a parked police car and the officers were leaning into the backseat appearing to talk to someone. She asked if they had found a child and was told, "Yes." Ms. Stathopoulos informed them the child attended Fischer Elementary and had run out the building. Child A was 9th ultimately transported to the Precinct and released to her mother. Ms. Stathopoulos stayed with Child A until she arrived. 9th On 12/01/2022, I went to the Detroit Police Department Precinct. I requeste

On 11/22/2022, licensee designee, Amanda Mattea notified the department about the incident that occurred on 11/16/2022, immediately after staff reported it. It was told to her; staff was concerned about reporting the incident and concerned about losing the childcare license. CCSMs, Ms. Clark and Mrs. Harris- Scott informed me they do not recall being told serious incidents or missing children had to be reported to the department. They were "focused and worried" about Child A and it never crossed their mind to report the issue to the department or the licensee designee. They now understand and will report issues. 4
