I conducted an unannounced investigation at the center on 11/09/2022, with the again on On informed the licensee "esigaMnee, Ms. Sharday Quinn of all the allegations. She stated that Child A came to the center with a smashed finger after not being present for a few days. Child A's Mother told her that it occurred while Child A was with her aunt. She said Child A's Mother also stated that she was upset with the aunt because she could not tell her how it happened. Ms. Quinn stated that she was unaware of Child A falling out of a highchair at any time as staff had not reported any incidents to her. During my onsite visit at the center on 12/13/2022, I observed that Child A was still attending the center. I interviewed the program director, Ms. Hattie Murphy. She said Child A did not get her finger smashed while at the center, but she was aware of Child A falling out of the highchair on two occasions. She stated that the Child Care Staff Members (CCSM) were not paying attention to Child A and she wiggled and fell out of the highchair, but she was not injured. Child A's Mother was made aware of the incident. I interviewed Child A's teacher, . She said she has a close relationship with Child A's Mother. Child A's Mother came to her and informed her that Child A smashed her finger while with a relative over the weekend. She said the smashed finger incident did not happen at the center. was unaware of Child A falling out of a highchair as, she did not while Re has been in her care. I interviewed CCSM . She said she was aware of Child A's being smashed but it did not occur while she was at the center. She stated that she did see Child A fall out of the highchair. She was preparing food for the children and the other CCSM, , was talking to a parent. When she was walking back into the room, she saw Child A in the highchair attempting to get out, she could not get to Child A fast enough to catch her. Child A fell out of the highchair but did not have any marks or bruises. I interviewed CCSry She said she was also aware of Child A's finger getting smashed while she was not at the center, but with a family member. She has had concerns about the healing of Child A's finger and does not think it is being properly addressed by Child A's Mother. She also said she heard about Child A falling out of the highchair, but she was not present at the center at the time it occurred. 3 Calls were made to the complainant and to Child A's Mother on several occasions, and voicemail messages were left. As of today, the complainant nor Child A's Mother has returned my calls to discuss the allegations. Contact was made with , there were no findings from her investigation.

She denied allegations that she has pinched any of the children. On 12/15/2022 I called Child B's (5-year-old, female) Mother. She said she has never had any concerns about the care of her two children that attends the center. She said she has never heard of any of the CCSMs pinching, yelling, or cursing at the children. Her two children have attended the center for several years. On 12/15/2022 I called Child C's (3-year-old, female) Mother. She reported no concerns about the care that Child A receives. She has not heard any allegations of children being yelled at and her child has never reported that she has been pinched at the center.

It was alleged that there was no hot water working at the center. At the time of the unannounced visit at 1:00 pm 11/09/2022, Ms. Quinn was working with a company ettin the Hot Water Tank replaced. I spoke to " as he was working to replace the Hot ater tank. He informed me that he would have the new tank in within the next two hours. Ms. Quinn said she informed the owner of the building that there was an issue with the water and that sometimes the water would get hot and sometimes it would not. She reported that the issue had only been going on for a few days. Ms. Murphy said there had been an issue with the water not getting hot, but Ms. Quinn was working on getting it fixed. stated that she has had to boil water to use due to the hot water tank not wwoorrk iinngg,, and she said it had not been working for a few weeks.

It was reported that one side of the building did not have heat and that the children were having to wear their coats and hats while at the center. During my onsite inspection, the entire building was completely warm and there was no concerns with heat. Ms. Quinn said she is surprised that the allegation was made because the weather for September and November has not really been cold and the children have not started wearing coats and hats. She has not had any heating problems. 7 All the CCSMs members interviewed, reported they have not had any problems with the heating in the building. Child B's Mother said Child B has only worn a sweater a few times. She said the building was too warm and so, Child B has had to take his sweater off while at the center due to getting too hot. Child C's Mother reported no concerns with the temperature in the building.

On 08/31/2021, I interviewed Child A and B's Mother. She stated both children are doing well, but they may not return to the facility. Child A and B's Mother has not had any problems or concerns with the facility in the past, but allegations of inappropriate discipline is concerning. She did not witness the incident but was informed when the children arrived home from school. They reportws to have been flicked with a pen or had an ear pulled, for no reason, by their teachers. The children report no witnesses were present, but the center has cameras. They are often told to go into the rear of the building and it's there some form of physical discipline it takes place in the area since there are no cameras in the rear. Child A 2 and B's Mother stated she would call me back and allow me to speak with Child A, but the call has not come as of this entry. According to Child A and B's Mother, Child B really isn't saying anything about the incident. The information is coming from Child A. On 09/7/2021, I attempted an onsite investigation, but the facility remained closed for the holiday observance. I called licensee, Ms. Sharday Quinn about the complaint. She is adamant nothing happened. She was present and did not see or hear anything inappropriate. Further, Child A admitted in front of her mother, nothing happened. On 08/30/2021, Child A and B's Mother and grandmother came up to the facility and reported the incident. Ms. Quinn had heard nothing about it and tried to calm both women down as there was yelling and threats of physical violence. Child A was with them and she asked her to explain what happened. Child A could not remember much of the story so her mother helped her fill in the blanks. Child A ultimately said nothing happened, but another child told her about an ear pulling incident. Ms. Quinn thought the incident was over until she heard from me. Child A and B's Mother was present when Child A admitted nothing happened, so she didn't understand why the complaint was made. Ms. Quinn has cameras throughout the facility. She went to check the footage after being informed of the incident and found the cameras weren't working. If she had proof of inappropriate discipline the staff would be let go. She does not allow any form of corporal punishment. She admits an ear pulling incident occurred in the past and the staff person was terminated. On 09/20/2021, I interviewed CCSM's Lisa Bonner and Connie Sykes. They submitted written statements and indicated nothing happened. Both CCSM's denied the allegations and stated they have never touched any child at the center inappropriately. It's believed Child A made up the story. She likes "hanging out" with the boys and does not like to be redirected. Ms. Bonner states she requested Child A and B sit with other girls at the facility. They were playing with a group of boys who have "sometimes caused problems." Child A immediately yelled, "I'm telling my mother" and started crying. She was never "flicked with a pen and her sister's ear was never pulled." Child A made everything up as she wanted to "hangout" with the boys and became angry when redirected. Ms. Sykes was in the kitchen when she heard Child A crying and say, "I'm telling my mother." She peeked out to see what was going on. She asked Child A to come in the back and tell her what's wrong. When asked what happened and what she needed to tell her mother she stated, "Nothing." Child A went back into the main room and apologized to Ms. Bonner. 3
