On 05/06/2022, the department received an incident report from program director, Sherry Steward regarding the above allegations. On 05/11/2022, I called Child A's Mother regarding the allegations. I asked Child A's Mother to tell me what she learned of the incident after being notified. Child A's Mother said on 05/06/2022, she received a message on Bright Wheel about 8:40 a.m. from child care staff member, (CCSM) Makayla McKenzie, stating Child A was injured. A picture of Child A's injuries was sent through Bright Wheel as well. Child 2 A's Mother said she also received a telephone call from program director, Sherry Stewart, stating that Child A was swinging on a piece of furniture that houses the dress-up clothes. The furniture fell on her and hit Child A over her right eye. Child A's Mother said she was informed that CCSM Makayla kept telling Child A to stop swinging on the furniture, but she would not stop. Child A's Mother said she asked CCSM Makayla on the Bright wheel App, why they did not move Child A away from the furniture or move the furniture if Child A kept swinging on it. Child A's Mother said she took Child A to the emergency room after picking her up and she got an x- ray and received stitches. Child A's Mother said Child A has had a headache since the incident. On 05/11/2022, I completed an onsite inspection. I interviewed program director, Sherry Stewart. Ms. Stewart said she was aware of the incident involving Child A being injured by the dress-up structure falling on her eye. Ms. Stewart said she learn of the incident from Ms. Makayla. Ms. Stewart said on 05/06/2022 CCSM Makayla completed the Bright Wheel notification report to notify Child A's parent then both she and CCSM Sydney Stewart completed the incident report and submitted it to the department. Both Sherry Stewart and CCSM Sydney Stewart were not witnesses to the incident. CCSM Makayla was the only staff supervising the preschool classroom at the time of the incident. On 05/11/2022, I interviewed Makayla McKenzie. I asked Makayla to tell me about the incident surrounding Child A getting injured. Makayla said she had five children in the preschool classroom in care on 05/06/2022. Makayla said when they see children playing inside the dress-up structure, they tell them verbally and in sign language, "that's not safe." Makayla said she only told Child A to stop swinging on the structure, one time. I asked Makayla if she told Child A's Mother that she repeatedly told Child A to stop swinging on the structure. Makayla said she told Child A's Mother she told her "Over and over again" on other days but not on 05/06/2022. Makayla recalls only telling Child A to stop once on 05/06/2022. I asked Makayla, if Child A had been told any time prior to 05/06/2022, why she did not redirect her to a different area in the classroom. Makayla said children should not be deprived of playing and exploring with the other children.

On September 1 2020, during an unannounced onsite inspection, ■ and were present. stated that on or about she took approximately 4 child care children to the park and then to her home to pick up vegetables from her freezer. While at her home, she and the children got out of her vehicle but did not enter the home. stated that Yarmon O'Kelley, was standing at the door talking to her when their dhoegr"erm% Shepard) ran out of the house. She said that she instructed the children to be still and not run. However, Child A ran and tripped while running. He reporte hat he took the dog downstairs when he saw and the children outside. Mr. O'Kelley said that he thought that he locked e door, but the dog was able to et out without him knowing. He said that when he opened the door to speak to his , the dog "blew" by him. He stated that told the children to be still an don't run, but Child A ran, and the dog chased and bit . Mr. O'Kelley stated that first aid was administered before return o the day care center. Mr. O'Kelley said he didn't know that and children were coming. He said they were at their home for a total of 40-45 minutes. He reported that their dog has never bitten anyone. On October 6, 2020, Child B's father reported that he to take Child B away from the childcare center. He reported tthraut shets wManoaware of the dog bite incident. Child B's father reported that he did not sign a field trip form on the date in question, but claims called and told him that she was taking Child B away from the child care center. Attempts were made on September 3, 2020 and on September 11, 2020 to interview Child A's mother. At the time of this report, Child A's mother has not returned the call.
