On 02/23/2022, I interviewed Child A's Mother. She has never had any problems or concerns with the facility prior to the current allegation. Child A is doin well and continues to attend the facility. On was asked 02/17/2022,U about day at school. " response was, "Ms. Moore kicked me in my back for not listenin ." was also told, "shut your mouth and go to sleep." Child A's Mother checked back and did not find any marks, bruises and there was no complaining of any pain in the area. The incident took place at naptime and Child A reports there were other teachers in the room. On 02/24/2022, I contacted to discuss the allegation, documents received from the agency and scheduled a joint onsite investigation. We agreed on 03/02/2022. On 03/02/2022, an onsite investigation was conducted. The following individuals were interviewed: licensee designee Rhonda Mallory-Burns, Willette Kell Education Specialist, Ramona Brown Site Leader, Child A and Child B year-old, and CCSM Sandra Brown. Mrs. Mallory-Burns and Mrs. Kelly have no firsthand information regarding the incident. Neither of them were in the room when the incident occurred. They have no reason to believe the allegation are true based on the information they have received from staff and child interviews. An immediate attempt was made to review video recordings of the classroom. However, there was a glitch in the system and the 3 footage was not available. On 02/22/2022, Child A was interviewed by licensee designee Debra Kade. Child A's Father was present during the interview. Child A's R Father said, he picked ChildN A up the day of the incident and asked about day and if had taken a nap. reported to have not napped and when as ed why stated, s. Moore kicked me in my back for not listening." Child A w0as "sad" about the incident and told Ms. Moore "that's not nice." Once home, re orted the incident to mother but changed lying down to "was sitting down.' was asked to demonstrate what occurre and two kicks were made. One was ow and light, but the second kick was high and harder. Both were allegedly made to the back area. I received a copy of the interview conducted by Ms. Kade. According to the submitted interview report, Child A "was all over the place" about other events and was reluctant to talk or demonstrate anything about the incident. When initially asked if there had been any accusations of inappropriate discipline in the past, Mrs. Kelly "vaguely" remembered an incident last year where Ms. Moore was accused of hitting a child. Mrs. Mallory-Burns states she remembered reading a message from a parent on the Bright wheel app, but could not recall anything about hitting. She met with the parent and recalls it being about "methods of redirection." On 02/18/2022, Mrs. Ramona Brown was notified of the incident via the Bright wheel App. She informed Child A's Mother a thorough investigation would take place. She reported the incident to Mrs. Mallory-Burns who conducted the interviews. CCSM Sandra Brown is the lead teacher in Child A's classroom and was present the day of the alleged incident. She reports the regular routine was followed. After lunch the children were preparing for naptime. At approximately 12:30pm, Ms. Moore left for lunch and eight of the nine children present were sleeping. Child B was awake and playing quietly on her cot which was in close proximity of Child A. There was no negative interaction between Ms. Moore and Child A prior to her leaving for lunch. She has never witnessed or suspected her of being inappropriate with any child. Ms. Moore returned from lunch at approximately 1:30pm and Ms. Brown left. Child A was sleeping when she left the room and had left for the day by the time, she returned from lunch. On 03/08/2022, I interviewed Ms. Endia Moore. She denied the allegation and states she would never hit, kick or do anything to hurt a child. She does not know why Child A made the allegation. was sleeping when she left for lunch at 12:30pm and was still s
