    On 08/22/2025, I reviewed five child care staff member records. Of the five records reviewed, one child care staff member was missing the 2024 health and safety refresher training,

    On 08/22/2025, I reviewed eight children's records. Out of the eight records reviewed, five children information cards were incomplete. All five child information cards were missing employee information and work telephone numbers. One child information card was missing a date and signature by parents. three child information cards had N/A listed in the allergy section. The department does not accept N/A as an acceptable answer.
