On 05/10/2023, I received an incident report notifying me of the accident. On 05/11/2023, I contacted Child A's Mother. He is doing well and continues to attend the facility. He received eight sutures to a cut on his forehead that has since healed. The day of the incident, the school contacted her and explained the fall and injury. She is uncertain who called but was told he jumped off a chair and fell, hitting his head. He was immediately taken to the school nurse for examination and care. Child A's Mother has not had any problems or concerns involving care and supervision but does not understand how Child A received a cut from the incident since the chair is only a few feet off the floor. 2 On 05/15/2023, I conducted an unannounced investigation at the facility. I inspected the classroom and discussed the incident with CCSM 1 and CCSM 2. I did not observe any blind spots in the room. I observed the chair Child A attempted to jump over; it is a small, blue PreK size chair. Based on the layout and staff recollection of events, they were in close proximity to Child A. The day of the incident, CCSM 1 was sitting at a table engaging other children in a board game, there were 11 children present at the time of the accident. It was "Choice Time" and Child A was playing with another child in the Movement and Dance area before transitioning to the reading section. CCSM 1 positioned herself at the table where she could monitor the entire classroom. While talking to a child at the table she could see Child A, with her peripheral vision, falling forward, after attempting to jump over a chiar. She got up and walked toward him. He was crying, got up and walking toward her holding his forehead. She removed his hand and saw a wound. She applied pressure to prevent heavy bleeding. CCSM 2 came over to help and CCSM 1 asked her to take him to the school nurse. CCSM 1 called Child A's Mother and informed her of the incident and that he may need stitches. CCSM 1 does not recall the exact day but following the accident, she showed Child A's Mother where and how the accident happened. CCSM 2 was in the classroom but did not witness the incident. She was sitting at table engaging other children with a writing activity when she heard crying. She looked up and saw Child A walk to CCSM 2 while holding his forehead. CCSM 1 checked his forehead and showed it to her. CCSM 2 saw a small amount of blood and applied a paper towel to the area. She then carried him to the school's nursing office where he was examined, and a band-aid was applied. Child A's Mother was called and notified of the incident. Child A reported to have tried to jump over the child size chair in the reading area.
