On 07/05/2022, I conducted an unannounced visit to the daycare center located at 19300 W 7 Mile Rd in Detroit. I conducted the visit at this location due to the location at 17610 W McNichols Detroit, MI being closed The staff from Christios Childcare & Academy 3 was at this location I interviewed Ms. Janay Schaeffer, the new Program Director, and Licensee Designee. She stated that on 06/28/2022 she was contacted and informed that Child A was left outside on the playground unsupervised at the W McNichols location for an unknown amount of time. She went to the location to see what had transpired. 2 She obtained all the necessary information from Ms. Dionne Hill, the Program Director at the time. On 07/06/2022, I called Ms. Dionne Hill, who was the Program Director and Licensee Designee at the time of the incident. She stated that on 06/28/2022 a lady from a few doors down barged into the facility stating that Child A was left outside alone on the playground. She went to the toddler classroom located by the exit door to the playground, when she opened the door, Child A was sitting outside crying. She spoke to the Child Care Staff Member, Patricia Portice and she admitted to her that she did not complete a head count when bringing the children back into the building and that she was unaware she had left Child A outside. I obtained the contact information for Child Care Staff Member, Patrice Portice and contacted her via telephone call on 08/02/2022. She stated that on 06/28/2022 she took eight children outside to play at 11:30 a.m. She returned to the classroom at 11:40 a.m. and did not do a head count when returning to the classroom. She admitted that she did not notice that Child A was missing until she was notified by Ms. Hill. She provided a written statement that stated Child A was outside alone on the playground for approximately 15 minutes. Both Ms. Hill and Ms. Portice were terminated from their employment at the center. I interviewed Ms. Debra Lomax on 08/08/2022 at her daycare center located on W McNichols. She stated that on 06/28/2022 when she returned to her facility, she noticed a child outside on the playground at the daycare center a few doors down. She kept looking and did not notice an adult outside. When she finished unpacking her groceries, she noticed the child was still outside alone. She immediately went to the daycare center and informed them that they had left a child outside alone, and at that time they still were unaware they were missing a child. I observed the playground at the daycare facility. It is fenced-in, and the gate had chains with locks on it. There was no way Child A could have left the playground. When Child A was found outside, he was by the door crying and did not sustain any injuries. Ms. Schaeffer stated that all the staff that was present on the day of the incident was either terminated or voluntarily quit their jobs. The center was closed for several weeks due to not having staff to accommodate the children. All the children were taken to location on 7 Mile Rd in Detroit. As of 08/08/2022, the West McNichols location is open for business and Ms. Schaeffer as acting as the current Program Director at the location. During the investigation, it was noted that Child A's Mother was not notified about the incident until the following day. Ms. Schaeffer notified Child A's Mother at drop off on 06/29/2022 about the incident. A written statement was also provided to Child A's Mother, and she signed an acknowledgment on 07/01/2022. 3 I contacted Child A's Mother on 08/10/2022. She stated that Ms. Schaeffer informed her on 06/29/2022 that Ms. Portice left Child A outside on the playground unsupervised. She was somewhat upset that no one notified her on the day of the incident, as she recalled speaking to Ms. Hill when she arrived. She noticed things did seem somewhat strange, but she did not know why. I contacted Ms. Patrina Dobbins, the Licensee, on 08/18/2022. She was made aware of the allegat

(7) If there are children of mixed ages in the same room or well- defined space, then the ratio and group size is determined by the age of the youngest child, unless each group of children is clearly separated and the appropriate child care staff member- to-child ratios and group sizes, if applicable, for each age group are maintained.
On 12/2/2021, I completed an unannounced inspection onsite. I interviewed program director, Dionne Hill. Ms. Hill is also the preschool teacher. She is currently in the process of hiring additional staff. She denied the center is out 2 of ratio. She said on 12/1/2021, she was present at the center and the child care staff member (CCSM) to child ratio was 3:11 by 7:53 am. There were 18 children enrolled at the center. I interviewed Ms. Hill a second time on 2/9/2022. She said the children eat breakfast together in the morning and then are separated and taken into their individual classrooms. Each staff member is present with their children. Once they are done eating breakfast, they are taken to their classrooms. On 12/2/2021, during my inspection, the CCSM-to-child ratio was 3:10. The ages range from 2 to 3 years of age. On 1/12/2022, I interviewed Child A's Mother. She has noticed quite a few children through the window when dropping off. She is not aware of how many child care staff members were present because parents are not allowed to enter the building. On 1/12/2022, I interviewed Child B's Mother. She is not aware of the amount of children present when dropping Child B off. She has observed at least three child care staff members when she passes the window that shows inside the center. On 1/19/2022, I interviewed CCSM, Scheree Towns. She said there has not been any issues with ratio. There is enough staff to accommodate the amount of children scheduled. On 1/19/2022, I attempted to interview CCSM, Brianna Robinson. To this date, she has not returned my phone call. She no longer works for the company. I reviewed sign in sheets for 12/1/2021. The CCSM-to-child ratio was 3:11 by 7:45 a.m. The CCSM to child ratio was 5:18 by 8:45 a.m. The children ages range from 1 to 4 years of age. On 2/9/2022, I interviewed CCSM, Trenesia Watson. The CCSM to child ratio was always maintained by the time she arrived to work. There have not been any issues or concerns with ratio and staff present.

On April 28, 2021, I initiated this investigation with Essence Hickman, CDC Consultant. Ms. Hickman stated that there was not an incident report regarding these allegations. She also was not aware of any alleged abuse at this facility. On April 29, 2021, I interviewed Child A's mother via telephone. Child A's mother reported that Child A will this child care facility. She said that Child 2 M. A was enrolled for a Child A's mother stated that `Ms. Kenna" hit Child A and Child B. On May 7, 2021, during an on-site inspection, child care staff members, Kenna Coleman, Janay Schaffer and Trenesia Watson were interviewed. Child B was also Kenna Coleman re orted that she has been employed with this child care facility since . She denied hitting Child A and Child B. She denied hitting any of theRWren in care. Ms. Coleman states that she did not touch the children in any way that would give them the feeling of being physically disciplined. Ms. Coleman disclosed that Child A and Child B are "good kids" and do not have to be redirected often. She said that she is aware of the center's discipline policy and received a copy at the time of hire. Janay Shaffer identified herself as the center's manager. She reported that she is aware of the concerns. She said that she was not present when the incident allegedly took place. Ms. Shaffer denied knowledge that Ms. Coleman has hit Child A and Child B. Trenesia Watson stated that she has been employed at this child care center since as well. She reported that she has never witnessed Ms. Coleman or any child care staff member hitting the children in care. Ms. Watson said that none of the children has ever reported to her that Ms. Coleman hit them. Ms_ Watson is aware of the center's discipline policy. At the time of this report. Child B's mother has not returned my telephone call.
