  On 03/05/2021, 1 spoke to Child A's Father regarding the allegations. He stated that he was unsure what classroom Child A is in because Child A is dropped off at the door of the center due to COVID-19. However, he believed Child A's caregiver is Alexis. On 03/01/2021, Child A was taken to Childtime Learning Center and did not have any bruises on body. That 2 evening, Child A was picked up around 5.00 p.m. by Child A's Mother and, was taken home. Child As was changed at 6:00 p.m. and the bruise was observed on Child A's inner right thigh. Child A's Mother and Father took pictures of the bruise. Child A was not taken to the regarding the injury on 03/01/2021. The following day Child A's Father took to the child care center. He spoke to the person at the door, and he told her that Child A had a bruise on thigh. She told him the information would be related to the supervisor. Child A's tither received a call at 10:00 a.m.. from the child care center and they stated that Child A had fallen and hit head on 03/01/2021, On 03/03/2021, Child A's Mother was given an inciden report, but she refused to sign and take the incident report because she did not believe Child A failing could cause a bruise on inner thigh. Child A's Mother and Father asked to view the camera of the incidei and they were told that there is only live footage. Child A's Father stated that they believe that the child care center is covering up what occurred to Child A. Child. On 0310512021, five pictures were provided for reviewed of the bruise on Child A's inner right thigh. There was one picture dated for 03/01/2021 and four pictures dated 03/04/2021.. The picture dated 03/01/2021, there was a reddish bruise, the size of a nickel, inside of Child A's right thigh. The pictures dated 03/04x12021, the bruise was about an inch to inch and half in length and the color was yellow/ green. On 0310512021, 1 completed an unannounced on-site inspection at Childtime Learning Center. I spoke to Janet l ize-Brown, program director, Chris Seraphino, school education manager, and child care staff members, Loria Onakola, Bridget Riggs, and Toya Young. I observed the child care center. In the Infant 1 room, there were six infants with two child care staff members. In the Infant 2 room, there were four infants with one child care staff member. In the Toddler room, there were two children with one child care staff member. In the Twos room, there were six children with two child care staff members. In the Early Preschool room, there were eight children with one child care staff member. In the Pre-If room, there were six children with one child care staff member. A copy of the attendance record from the room and the incident report, for 0310112021 was reviewed. According to to sign in sheet, there were four children present on 0310112021 with two caregivers. The incident report stated on 03/01/2021 at 9:55 a.m. Child A was behind the rocking chair, fell and hit the right j side of forehead. There was a knot sorehead and ice was put on it. The onT incident report was completed by child care staff member, Alexis Grigsby. There is a written note at the bottom of the incident report indicating that on 03/02/2021 at 11:05 a.m. Ms. Brown spoke to Child A's Father about bruise on Child A's inner left thigh. According to Ms Miza- Brown Child A is enrolled in the oom. The children are ages in the room.fe"cicircare staff members are Ms. rigsby and Ms. Young in the= room. On 03/01/2021, at 10;00 a.m. Ms. Grigsby came to Ms. Miza- Brown and reported that an incident occurred in room. An incident report was written by staff and it ■ was supposed to be provided to Child A's Parent at pick up.
