On 01/04/2022 while conducting the onsite inspection, I asked Ms. Ashley if was an adult household member. Ms. Ashley told me he wasn't. On 01/30/2023, I contacted Ms. Ashley. I asked Ms. Ashley if was an adult household member. Ms. Ashley said, "Yes he was." Ms. Ashley said on 01/04/2023 during my onsite inspection, she was nervous and upset due to the allegations regarding Child A, so she told me did not live there. Ms. Ashley apologized and showed remorse that she untruthful information during the investigation.

On 01/30/2023, Ms. Ashley informed me that resides in her home. She said she was unaware that she needed o notify Per consultant prior to him moving in the home. Ms. Ashley is in the process of preparing all the necessary documents to submit to the department for - to be an adult household member.

Child A ) was observed by licensee with sores/blisters in mouth "anwEas taken to the hospital for medical treatment. It was etermined the blisters were second degree burns from eating something very hot. stating possibly been from the result of food that was too hot. On 01/04/2023, 1 completed an onsite inspection. I interviewed licensee, Ashley Davis. Ms. Ashley said, on 01/02/2023, Child A was dropped off at 7:00am. Ms. Ashley recalled Child A wanting her to pick up and hold . Ms. Ashley said that Child A did not usually want her to pick up upon arrival each morning, as is mostly just handed to her by Child A's o her. Ms. s ley said Child A's other told her that$ had been up since 2:00 a.m. and was "very whiny." Child A's Mother told Ms. Ashley, "if whins again, give a sippy cup that has almond milk and Hershey's Chocola Syrup in it." Ms. s ey said she took Child A upstairs, puts in her bed with her, and they slept till about 12:18 p.m. Ms. Ashley received a personal telephone call and she; her w and Child A were awakened from the ringing of the phone. took l ~ownstairs and made some frozen comdogs. cooked he corndogs in the air fryer for four minutes and gave them to Child A, along with sippy cup. After Child A finished eating, began crying. Ms. Ashley asked to bring back u stairs to w ere she was finishing her telephone call. When she picke up, leaned back and opened■ mouth. Ms. Ashley noticed 3 Child A's tongue had a white coating on it which looked like thrash. Ms. Ashley immediately texted Child A's Mother with pictures accompanying the text and said, "Hey did you see this? been fussy all morning." Child A's Mother responded, "What's wrong with it?"' Ms. Ashley said, "it looks like something is wrong with tongue." Child A's Mother said she would pick up and take to the doe or. Ms. Ashley gave Child A Motrin because she fe temperature getting warm. Child A's Mother picked Child A up at 1:45p.m. and took to M Ms. Ashley said Child A's Mother called from the hospital and asked a eaten something hot. Ms. Ashley told Child A's Mother that only ate comdogs, but she didn't think they were too hot because she `always lets food sit a few seconds before serving" Child A's Mother said she asked because she was informed by the bum unit , that Child A's tongue looked like a thermal burn from eating something hot. Child A's Mother told Ms. Ashley that she was disenrolling Child A immediately as a result of the burn. On conducting an onsite inspection, I interviewed Ashley Davis, said Child A was E ssy the morning of 01/02/2023. brought Child A downstairs when woke up to fe said a put corndogs in the air fryer, turned the tem0p1e/r0a4tu/2re0 2o%35 egrgees, nd set the timer for four minutes. He said when they were done, he put the comdogs on a plate and set them on the table that was in the living room. said Child A was standing, so took the plate of corn dogs off the table !Rn aced them on the couch near the area where Child A was standing. began playing the video game that was on the television screen. He said the televsion was having a problem, so he started working with the television for a few seconds, said Child A did not touch the food right away. He said when he finally started playing the video game, he heard Child A say, "Ahhh." turned around to see why Child A made that sound, he saw ChilWd Ah sepint t"hceom g out and it fell on the floor. said, "Child A picked it right up and ate it again, so I know the corndog couldn't have been hot, or else Child A wouldn't have eaten it again." While on site, I asked to demonstrate how he prepared the corndogs by putting them in the air fryer just as it was done on 01/0212023, when feeding Child A. Ms. Ashley put the comdogs in the air fryer and set the timer for four minutes, I also set the timer on my cell phone for four minutes. After four minutes, Ms. Ashley brought the corndogs into the living room where I was sitting. I broke open one of the corndogs with my hands. The outside of t Ms. Ashley Davis provided untruthful information during the onsite inspection on 01/04/2023. Ms. Ashley did not notify the department that she had an additional adult household member prior to him moving into the home. On 01/02/2023, licensee Ashley Davis took Child A on the second level of the home for over five hours. The second level of the home is not child approved space. On 01/02/2023, licensee Ashley Davis put Child A in unapproved sleeping equipment. Ms. Ashley did not submit the proper documents to determine eligibility before he had unsupervised contact with PIRR 11111111

On 01/02/2023 when Child A arrived, Ms. Ashley took Child A upstairs to return to rest. The rooms that are approved in this child care facility are the living room, the right bedroom, the kitchen and bathroom. The approved rooms are all on the main level of the facility. 1 R 400.1915(2) Indoor space; play equipment and materials. Only space that has received prior approval for child use by the department may be used for child care. m ANALYSIS: Ms. Ashley failed to keep Child A out of unapproved child use space. Ms. Ashley allowed Child A to utilize the second floor of the facility. Child A should only have been on the first floor of the facility because only the first floor is approved for child care, not the second floor. CONCLUSION: I VIOLATION ESTABLISHED

Ms. Ashley Davis failed to ensure appropriate care and when she left in the care of~ iiiiiiiais not a CCS , and Ms. Ashley avis was on the second floor of her child care home, while Child A and was on the main floor. There is insufficient evidence to show that Child A was injured while in the child care home. Doctors initially thought Child A contained a thermal bum, but the hospital reports concluded that this was not a thermal burn. It is unknown what caused the injury to Child A's mouth.

During the onsite inspection on 0110212023, had unsupervised contact with Child A. He made lunch for Child A on the main floor of the home and remained with until finished eating, all while Ms. Ashley was on the second floor of the home, had unsupervised contact with Child A. On 0212712023, a review of the child care background check system reveal that was fingerprinted on 1/05/2023 and was determined to be not eligible to e is connected to a child care facility. On 02/27/2023, I contacted Ms. Ashley regarding eligibility status. Ms. Ashley admitted that he has continued to reside "inte home. Ms. Ashley said she was with the understanding that eligibility was lifted immediately on 03/12/2023 and thought that waste date he needed to be re-fingerprinted. I told Ms. Ashley that because of status, he needed to vacate the premises immediately. Ms. Ashley said she would not continue to allow to reside the home in light of this information. ■ _ will be moving out immediately.
