On 10106/2022, I spoke to Child A's Mother regarding the allegations. She stated that Child A has been attending Children's First Learning Center for a few months, every Thursday. Child A's Mother stated that she was told about the child care center by child care staff member (CCSM), a family friend, was previously watching Child A at her home. She stated that she "i s unsure but believed is renting a room at the child care center. Child A's Mother stated that she did not complete any enrollment paperwork. She was told by no paperwork was needed as Child A was under her supervision. s Mother drop off Child A at the center at various times between 10 and 11 a.m. Child A's Mother would walk in and met at the front door with most times. When the wea her began to change would tell ild A's Mother to meet them in the back at the playground if theywere already outside. Child A's Mother was never asked to sign Child A in would handle that. Child A's Mother paid $30 a day for the day attended. Child A's Mother stated on the video footage, Child A was in the room with an unknown CCSM. Child A was not running, and it "W"WiMi Child A, causing ■ A was taken to urgent care ty 1ather. Child A has not returned to the child care center since the incident occurred. On 10/06/2022, Child A's Mother provided the text messages and the incident reports for review. On 09/29/2022, Child A's Mother texted at 6:11 p.m. asking her where she was going to be and that she was on hergw"ay pick up Child A. Child A's Mother received a text message back from at 6:21 p.m. The text stated, "Hey o . So sorry, wanted to wait but I couldn't wouldn't sit own and was running at the child care and ran into the chairs. o sorry you will see it when you come." Child A's Mother stated that she was provided two incident reports for the incident that occurred to Child A on 0912912022. The first incident report stated that the incident occurred at 5:59 p.m. on 0912912022. According to the written report, during dismissal Child A trippedoverR pants which were too big, fell on This observation was made reviewing e cameras. Thereported stated thatChild A's Mother was notified by text message as soon as it happened. On the second incident report, additional information was written at the bottom of the report and when the parent was notified was changed. The additional information written at the bottom of the incident report stated that, "Child A is no longer with the child care. gets dropped off to ■ at her job to go home with her so she can watt at home un I mom get off. Child A only come once a week on Thursdays. hild A's Mother reviewed the camera at the child care center to see what happened to Child A on 0913012022? It also stated that charged her phone up then she !Rify immediately sent Child A's Mother a text to her of the incident. On 10/07/2022, and I completed an unannounced inspection at Children's First Learning Center. We spoke to licensee, Sherika Cheney, CCSMs, ! . On 6/17/2022, I interviewed Child A, B, C's Mother by telephone. She stated Child B told her that he heard CCSM Ms. Woodard tell Child A to "pee on herself." He was sitting in a bean bag chair near Child A's classroom. Child B is a 2 school ager. He was not in school that day due to disciplinary reasons. Child A attends GSRP at another site. Child A and Child B are drop-ins. Child A, B and C's Mother told the program director Ms. Cheney about the allegations. Ms. Cheney told her she would watch the video and get back to her. Ms. Cheney told her there was no sound to the video so she could not say if Child A was told to pee on herself or not. Child A did not have an accident at naptime. Ms. Cheney told her she would use this opportunity to provide training to all her staff about appropriate language when talking to children. Child A, B and C's Mother went on to state Ms. Woodard tried to bribe Child B by buying him McDonald's for lunch so she would not tell on her. Child A, B and C are still attending the center. Their mother stated she does not have anything against the center and Ms. Cheney. Her problem is with Ms. Woodard, and she wanted her fired. On 6/21/2022 I conducted an unannounced inspection at the center. I interviewed the licensee/program director Sherika Cheney. Ms. Cheney stated Child A, B and C's Mother came to her about Child A being told to pee on herself. Ms. Cheney asked her to put it into writing. She would review the video, but she did let Child A's Mother know the video did not have sound. Ms. Cheney asked other CCSM's, and no one heard Ms. Woodard tell Child A to pee on herself. Ms. Cheney stated Ms. Woodard has been with her for a while. She would be surprised if Ms. Woodard would say something like that. It is not in her character. Child A and Child B are drop-in for the school year and attend full-time during the summer. They have attended the center for a few years. Child C attends full-time and is present every day. I interviewed Ms. Loretta Woodard. She denied telling Child A to pee on herself. Ms. Woodard stated all the children were taken to the bathroom right before naptime. Child A had used the bathroom then. Child A took a nap and did not have an accident. Child B was nowhere near Child A's classroom. Ms. Woodard admitted she brought Child B back McDonalds, but it was not to bribe him. She was doing something nice since he was the only school ager in the center. I interviewed Ms. Dajonna Kimble. She works in the same room as Ms. Woodard. She did not hear Ms. Woodard tell Child A to pee on herself nor does she believe Ms. Woodard would say anything like that. On 7/8/2022, I interviewed Child D's Mother by telephone. She stated she had no concerns regarding the care at the center. Child D is her second child that attended the center. She had no problem with any of the CCSM. They have always been appropriate in their communication with her and her children. On 7/8/2022, I interviewed Child E's Mother by telephone. She did not have any concerns regarding the center. Her interaction with the CCSMs has always been appropriate. 3 Note: Child A, B, and C's Mother also stated the center does not keep up with Child C's mask. This is not a licensing rule violation.
