Based on our findings, investigation, and review - which includes considering the investigation and findings of MDHHS -CCLB determines CCSM 1 is still conducive to the welfare of children and is still eligible to be associated with a child care organization.

I conducted an unannounced investigation at the child care center and interviewed Ms. Angela Todd, licensee. She stated that the program director resigned the day before due to some issues that were not work related. She was informed of the allegations, and she provided me with the names of the current staff members and their files. The allegations identified CCSM 1 as a staff member 2 that should not be allowed to work at the center and alleged that the staff member did not have background check completed. While on site on 9/06/2023, I reviewed the staff members files. All the staff members had the required comprehensive background checks completed and they were connected in the Child Care Background Check (CCBC) portal. CCSM1 did have the required comprehensive background checks and was found eligible 03/07/2023. I asked her if she had ever had a volunteer work at the center and she stated yes and identified two people. I asked for the files of the volunteers, and she did not have a file for them. She admitted that one of the volunteers did not have the required background check completed and the person was at some point alone with the children she was assisting. One of the volunteers completed a background check and was deemed eligible as of 02/03/2023, per the eligibility letter that I reviewed. Ms. Todd stated that when the program director suddenly left on 09/05/2023, she did have the person she identified as a volunteer to come to the center to help maintain compliance. The person was at the center most of the day. The same person had also assisted in July 2023 and assisted with setting up the classrooms.

Ms. Angela Todd admitted to not having a file for the person she stated had volunteered at the center. The identified person did not have any of the required clearances completed prior to providing care and assisting at the center.

Ms. Angela Todd admitted to allowing two different volunteers at the center. Neither of the volunteers identified had completed any paperwork before being allowed to be in the center. She is aware of the required documents for a volunteer and stated that it was an emergency at the time, and she needed the assistance due to staff walking out at the last minute.

While at the center I conducted a walkthrough of the facility. At the time of the onsite inspection, there was three classrooms operating. One classroom had two CCSMs present with 11 children, 3 years of age, and they were merging with another classroom upon my arrival. I interviewed the CCSMs. CCSM1 stated that there were only 11 children enrolled in the classroom at the current time. They were combining the classroom with the 4-year-old students during my visit for an activity. CCSM1 stated that there had been one time that since his employment that he was out of ratio, and Ms. Todd came to assist to ensure compliance. CCSM 2 was present. She said she works with CCSM1 and that she has not been out of ratio while working at the center and was unaware of any other classrooms that had been out of ratio. She stated that the center has 11 children enrolled and, on some days, they have only had eights children due attendance issues. CCSM3 and CCM4 were present with the 4-year-old children. They had 16 children present, and they were combining with the 3-year-olds at the time of my inspection. CCSM 3 and CCSM 4 said that they are always incompliance because they do not have the max number of children enrolled in the classroom. They have 18 children enrolled in the 4-year-old classroom. The other classroom was the infant room. There were two child care staff member present with seven children. I interviewed CCSM5 and CCSM6. They both stated that their classroom is never out of ratio and that there are only seven infants enrolled in the program at the time. I reviewed the sign in attendance log for the four previous days. The center was within ratio compliance on the days reviewed. I called Child A’s Mother on 10/5/2023. She stated that she did not have any concerns with the center other than no one being present when she arrives early mornings. She has had to wait in the car for staff to arrive to unluck the door to drop off Child A (4-year-old, female). 5 I called Child B’s Mother on 10/5/2023. The number was no longer in service. I called Child C’s Mother on 10/5/2023. She stated that Child C (4-year-old, male) no longer attends the center as of September due to relocating. She had no concerns while Child C attended the center and wished that he could return because he learned so much while attending the center.
