On 4/20/2023 I called and interviewed child care staff member (CCSM)1. CCSM1 said that she was not outside when Child A's incident occurred. She said she was inside the building preparing the children's lunch. CCSM1 stated that she was informed that Child A had fallen and hurt her private by CCSM2. CCSM2 then told CCSM3, who told her to write up the incident report. She expressed that she attempted to contact Child A's Mother but was told by Ms. Monique Snyder, Licensee Designee, that she could not use her phone to contact a parent. She said she then asked Child A if she was ok, and Child A told her "Yes". The children had lunch then took a nap. When Child A woke up from her nap, Child A informed CCSM3 that "it hurt". CCSM3 informed CCSM2, who took Child A to the bathroom and noticed blood in her panties. CCSM 2 then called Child A's Mother. CCSM1 said that when Child A's Mother arrived at the center around 3:00 p.m. she assaulted her and screamed at her because she did not contacted her when the accident/injury first occurred. CCSM1 is no longer employed at the center. 2 On 4/20/2023, I called and interviewed the Licensee Designee, Ms. Monique Snyder. She said she was not present at the center during the time of the incident. She was contacted by the staff members after Child A's Mother arrived to pick up Child A. She was told that she needed to get to the center immediately because there was a situation. When she arrived, Child A's Mother had physically assaulted CCSM1. She spoke to Child A's Mother, who said she was upset because no one called her when Child A's injury occurred earlier that day. Child A was still bleeding when Ms. Snyder arrived at the center. She was able to calm Child A's Mother down, then she put Child A in her car and transported Child A to the hospital, and Child A's Mother followed behind her. Ms. Snyder denied that CCSM1 tried to use her phone to call Child A's Mother because she was not at the center until around 3:00 p.m. after the altercation happened with Child A's Mother. She said that CCSM1 was suspended for her part in the altercation at the center with Child A's Mother. I called and interviewed Child A's Mother on 4/26/2023. Child A's mother said that she was not informed of the incident when it occurred. She was not notified until 2:25 pm when it was almost time for Child A to leave the center. She received a phone call from CCSM 2, who informed her that Child A had blood in her panties after she woke up from nap time. Child A's Mother was upset that she was not contacted immediately after the injury occurred considering the type of injury that it was. Child A informed her mother that she told CCSM1 after she fell and that CCSM1 told her "you will be ok." CCSM1 did not check Child A, instead she left Child A to continue to play outside. I conducted an onsite investigation at the center on 4/26/2023. I interviewed Ms. Snyder and other staff members. CCSM2 said she was outside with CCSM3 and CCSM4 with the children. She said that CCSM1 is the lead teacher for Child A, but that she was going in and out the building while the children were playing. CCSM2 said that she was told by CCSM3 that Child A hurt herself. CCSM2 said she spoke to CCSM1 and informed her that Child A had an accident outside and asked her to write it up. She does not know if CCSM1 wrote the incident report at the time. She went on to take care of another child and left CCSM3 and CCS4 with the children. CCSM1 was inside making lunch at this time. CCSM2 said that after the children took a nap, she was told that Child A was hurting, she then checked Child A and noticed the blood in her panties, and she called Child A's Mother immediately. CCSM3 confirmed that she was outside with CCSM2 and CCSM4 with several children. She saw Child A crying and asked her what was wrong with her, Child A told her that she hurt herself and that she had already told her teacher, CCSM1. On 06/16/2022, I arrived onsite to complete an onsite inspection. Brainiacs Clubhouse Development Center is an open space divided by petitions that make up classrooms. I interviewed Ms. Monique Snyder, program director regarding the allegations. Ms. Monique said Child A,and his sister, Child B, (7-year-old female) have attended Brainiacs Clubhouse Development Childcare since February 2022 where they learn virtually in a designated private classroom at the center daily. Child A and Child B's Mother was a child care staff member who worked at Brainiacs Clubhouse Development Center and was assigned to the infant classroom. The infant classroom is approximately six feet away from the private classroom, which is directly outside of Ms. Monique's office. Ms. Monique said on 06/14/2022, she observed that Child A had crayons in his area, and some dropped on the floor by his chair. Throughout the day, as Ms. Monique 2 would enter and exit her office, she would remind Child A to clean up the mess of crayons that had fallen on the floor, but he did not. Ms. Monique said she would periodically hear Child A's Mother talk over the petition to Child B and ask her to pick up the crayons that were dropped by Child A. Ms. Monique said she told Child A and B's Mother to continue with the duties in the infant classroom and not allow Child B to clean up Child A's mess. Ms. Monique said eventually, Child A began picking up the crayons he dropped. Later in the afternoon around 3:30p.m., Ms. Monique did rounds around the center as she does daily. She said she noticed blue crayon marks on the floor by Child A's chair. Ms. Monique said she immediately went and got a mop and a bucket and told Child A he needed to clean up the blue crayon marks off the floor. Ms. Monique said she got some Murphy's Oils soap and put it on the floor on top of the marks. Ms. Monique said she put her hands on top of Child A's hands as he held the mop and motioned the mop back and forth. Ms. Monique said, "Child A, this is how you mop the floor." During the time Child A was mopping, Child A and B's Mother was coming in from outdoor play with a class. Ms. Monique said as soon as Child A and B's Mother saw Child A with a mop in his hands and Ms. Monique's hands on top of his hands, she became extremely upset. She did not want Child A to use the mop. Ms. Monique said she informed Child A and B's Mother that there were no chemicals in the Murphy's Oil Soap and that Child A would not touch the wet portion of the mop. Child A and B's Mother wanted to wipe up the marks with a paper towel. Ms. Monique did not want Child A and B's Mother to clean the area, she wanted Child A to do it. Ms. Monique said although Child A and B's Mother was not happy about Child A using the mop, she said, "bye" when it was time to leave at 4:00p.m. The next day, Child A and B's Mother did not come to the center, but she contacted Ms. Monique and requested to take a few days off. After being denied because of the late notice, Child A and B's Mother decided not to return to Brainiac's Clubhouse Development as a child care staff member or for child care. I interviewed Ms. Tiffany Beasley. Ms. Tiffany is a child care staff member in the preschool classroom. Ms. Tiffany said on 06/14/2022 she was in Ms. Monique's office working on some paperwork and recalls Ms. Monique asking Child A more than once to clean up and saw Ms. Monique get the mop. Ms. Tiffany said she did hear Ms. Monique showing Child A how to mop the floor and she heard Child A and B's Mother sounding upset. Ms. Tiffany said she heard Child A's Mother ask Child A, "What are you doing?" Ms. Tiffany said she heard Child A's Mother say, "I will do it!" Ms. Tiffany said Ms. Monique told Child A's Mother, "No, he can do it." Ms. Tiffany said Child A's Mother walked away, trying to get paper towel to assist Child A in cleaning up. I completed an unannounced inspection on 2/4/2021. I spoke with the assistant director, LaToya Finley. She was aware that Child A did fall and hit her head on 1/27/2021. She provided me with an incident report that Child A's mother did sign. Child A fell at 3:40 p.m. that day and according to the attendance records, Child A's mother arrived at the center at 3:42 p.m. Ms. Finley stated that the center did not have enough time to call Child A's mother before she arrived within that two-minute window. I reviewed the center's policy about notifying parents of an accident or injury. The center did follow their own policy of attending to the child first and then notifying the parent.

On 2/4/2021, I spoke with assistant director, LaToya Finley. She stated that there was miscommunication with Child A's mother as to the contents of the bottles she was providing for Child A. Ms. Finley stated that the center provides formula for the infants in the program and those bottles are made on site. She believed that Child A's mother was providing bottles from home that contained breast milk, although it was actually formula. Ms. Finley stated she believed that the breast milk could be left out for two hours. She stated that if Child A did not drink the full 8 oz bottle provided by Child A's mother, they would allow it to sit for up to two hours and try to feed it to her again. 3 I spoke with lead infant caregiver, Sandra Plumber, who reiterated that after bottles were warmed up, they are allowed to leave out for two hours. They are not allowed to reheat the bottles though. She recalls feeding Child A some of her bottle in the morning and that she did not drink too much, so she fed the rest to her in the afternoon. I observed the daily record for Child A for 1/27/2021 with Ms. Finley and Ms. Plumber. It showed that Child A was fed 3 oz of formula at 11:36 a.m. and was fed 4 oz of formula at 1:28 p.m. Both Ms. Finley and Ms. Plumber agreed it was the same bottle that was fed to Child A twice. I provided consultation and explained the one-hour rule. On 2/4/2021, I spoke with licensee and program director, Monique Snyder. She too admitted that she thought bottles with breast milk could sit out for two hours. She agreed to change her policy immediately.
