    On 12/08/2021, I spoke to Child A's Mother regarding the allegations. There have been several incidents, where Child A re orted being hit and pinched by other children in ■ class. 5 -year Child A stated that Child F I -old, male) ripped shirt. Child A's Poter spoke to Ms. Davis about Child A s shirt. Ms. Davis state tat Child A had a bad day however, she was unsure as how Child A's shirt was ripped because Ms. Toran left at 2:00 p.m. that day. It took a couple of days to receive a res onse to what occurred. She was told that Ms. Toran stated that Child A ripped shirt is concern that the children are not being supervised. On 12/08/2021, according to Ms. Newton, the children are supervised in the -year I preschool room. There are some children, Child E and Child F -old, male) that hit other children in the class. Ms. Newton was unaware of any incident nvolving Child A. i s. Newton stated that they worked with i and a avior changed but recently Child A started reverting (i.e., outbursts in class, inappropriate language, and exposing in class). Each incident was communicated to Child A's Mother. Regarding, Child A's ripped shirt, Ms. Newton stated that it was sweater that was ripped last week not a shirt. Ms. Davis told Ms. Newton that t ere was a little hole in Child A's sweater and According to Ms. Toran, she always supervises the children in the preschool room. She tries to always stay close to the children or in the middle of them. She has never seen another child hit or pinch Child A. Ms. Toran has never seen any marks or bruises on Child A. Ms. Toran said Child A's Mother reported to the child care center that Child A had a ripped shirt. Child A stated that Child I (unknown, male) ripped ■ shirt. According to Ms. Toran, Child I is not in her class and is unknown. According to Ms. Davis, the children are supervised, and she has never seen another child hitting or pinching Child A. She has never observed any marks on Child A. When she goes into the preschool room most the children are napping and still on their cots. They get up at 3:00 p.m. and they put up the cots and blankets. Snack immediately follows and Child A is often close to her assisting with putting the plates down on the table for snack time. After snack they complete a project or game, bathroom break and get ready for pick up. According to Child A, can talk to Ms. Toran if something happens to. Child A stated that if someone its hits them back, and. tells Ms. Toran. Child A stated that Ms. Toran puts t em in the office.     On 12/08/2021, I spoke to Child A's Mother regarding the allegations. Child A's Mother stated that she is unsure if Child A is getting food or if On 12/08/2021, during the onsite inspection the weekly menu for 12/06/2021 through 12/10/2021 was posted in the child care center lobby. Lunch time was also observed in the center. On 12/08/2021, the children were served pepper steak, rice, mixed vegetables, and milk. The menu listed the following meals for breakfast: on Monday, oatmeal, apples, and milk, and Tuesday- bagel cream cheese, apple sauce and milk was served. For lunch, on Tuesday they were served, baked BBQ chicken, mac and cheese, sweet peas, and milk. On Thursday they were served, spaghetti, broccoli, garlic knots and milk. The following snacks were provided throughout the week of 12/06/2021 through 12/10/2021: goldfish crackers, apple sauce, water, cheese, crackers, and apple slices. On 12/08/2021, Ms. Newton stated that the children are feed well at the child care center. The children are made good meals like pepper steak and rice, chicken 7 According to Ms. Toran, Child A arrives . The child care center offers breakfast, lunch and two snacks. Most days Child A eats all food. Child A has a regular appetite, some days does not eat breakfast. Chi A does not ask for seconds when. eats. Child A's other sometimes send Child A to the child care center with a lunch. Child A's Mother never requested a copy of the food menu from Ms. Toran. According to Ms. Davis she has never heard Child A says is hungry while she was in the preschool room or during picked up. usually eats all ■ food. While she is in the room the children are provided a snac once they wake up from naptime. however, she stated that the weeKly menu is posted every Monday. Consultation was provided to Ms. Davis regarding notifying the parents that the weekly menu is posted in the lobby. 0 According to Child A,. stated that the food is good at the school. described ate for lunch on 12/08/2021 as rice and yellow things. They stated that they have no concern regarding their children being fed at the child care center. Child B, C and D's Mother stated that her husband has mention that Child D acts like she is starving when she gets home but she is very greedy.     On 10/05/2021, I interviewed Complainant. On several occasions, she saw mice, gnats, flies and mold in the facility. Complainant observed the mold in the infant room and it had been present since the facility opened. It was further alleged, the facility does not have water. Complainant has no additional information, but is concerned for the safety of the children. Complainant did not share her concerns with the center management. On 10/06/2021, I conducted an unannounced investigation at the facility. The licensee designee, Carmen Newton was present and was interviewed. She denied 2 the allegation and stated, "This place is always clean. We have never had anything like that here and you can look around." She also added, there had never been mold in the building and encouraged me to discuss the issue with child care staff member (CCSM) Paisley Ousley who works in the infant room. Ms. Newton admits the hot water heater had gone out the previous Friday (October 1, 2021). She immediately sent the children home and called for service. She called someone out to repair it and learned it was just the pilot lite. During my inspection, I walked throughout the facility and found it to be clean and well organized. There were no gnats, flies or mold observed anywhere in the center. I checked corners for rodent droppings and found none. The water faucets and water temperature in the bathrooms and the kitchen area was also checked. All were working properly with hot/cold water, except for the unit in the bathroom adjacent to Ms. Newton's office. There was no water coming from the spout when I turned the handles. According to Ms. Newton, the unit had not worked in sometime but children, or staff, do not use it. Prior to my leaving the facility, she called a plumber to have it repaired. The faucet is now working properly. On 10/20/2021, I interviewed child care staff members Paige Ousley and Shawntoria Hubbard. They have worked at the facility for a while and have never seen mice, gnats or flies. They report it is always clean. According to Ms. Ousley, Ms. Newton is really into things being clean and would not allow anything less. Ms. Hubbard works in the infant room and states it's always clean and there has never been any mold in the area. ..... ............................... ..........................................
