On 12/13/2022, I completed a telephone interview with Child A's Mother. Child A's Mother has two children attending the center. Child A has been attending the center since 8/2022. On Friday 12/09/2022, the center called and informed Child A's Mother that when Child A woke up from her nap her face was swollen. The center informed her that Child A had peanut butter and celery for lunch. On Saturday 12/10/2022, Child A's Mother took Child A to see the doctor. The doctor instructed Child A's Mother to give Child A Zyrtec and Tylenol as it could be seasonal allergies. The next week Child A's face was swollen every day after 2 nap time. The staff did not call her every day however, on Wednesday a staff member told her that she was concerned because Child A's face keeps swelling after nap time. Child A's Mother asked what Child A was eating for lunch, the staff member told her that they had peanut butter sandwiches that day. Currently, Child A does not have any food allergies and Child A's Mother stated that she has given Child A peanut butter several times before and never observed any swelling. Child A's Mother contacted Child A's doctor and spoke with the nurse at the office. The nurse asked if Child A had any allergies and instructed Child A's Mother to use a plain white towel on Child A's cot during rest time. Child A's Mother provided Child A with the towel and told Child A to use it during rest time. When Child A's Mother asked Child A if she used the towel during rest time, Child A informed her that her teacher did not allow her to use it. Child A's Mother is concerned that the cots are not being cleaned correctly or often enough which is causing Child A to have a reaction. On 12/14/2022, I completed an unannounced inspection at Bibleway Center. I interviewed program director Jearline Gailes, CCSM Janay Howell and Dynasti Robertson. Program director Jearline Gailes stated that Child A would come in the center with her eyes running. However, after nap her eyes were worse, they were puffy. Jearline asked Child A's Mother if Child A was allergic to anything. The center does not serve any nut butters such as peanut butter. Instead, the center serves Wow Butter which is a nut butter alternative made of soy. Child A did come to the center with a towel and was not allowed to use it. The center does not allow children to bring in outside bedding. The center provides a fitted cot sheet and blanket to the children to use. Jearline informed the CCSM's that Child A's Mother would need to get something in writing from the doctor stating the need for alternative bedding. The center uses the three-step process for cleaning cots which includes, spraying cots with soapy water, cleaning them with plain water, and sanitizing them with bleach sanitizing solution. The cots are cleaned daily at the end of the day. The three step solution bottles are emptied and refilled daily as well. The fitted cot sheets and blankets are sent out to a laundry service every Friday to be cleaned. The cleaning company uses a fragrance-free sensitive skin detergent. Jearline said CCSMs Janay Howell and Dynasti Robertson work in Child A's room. CCSM Janay Howell, stated that she is not sure when the eye swelling first occurred but knows that it was on the day that the center served the Wow Butter. Every morning for the past few weeks Child A has been coming into the center with runny watery eyes. Child A will sometimes state that her eyes are burning. The day that the Wow Butter was served Child A woke up from rest time with a generally swollen and puffy face. Children rest after lunch. When Child A woke up Child A was being picked up. The next day the same thing occurred, but instead of her entire face swelling only her eyes appeared to be slightly puffy. The following week the same thing occurred, she came in with water eyes, and slight puffiness of the eyes only 3 after rest time. Wow Butter has not been served since. Janay stated that cots are cleaned daily as st
