R 400.8158 · 1 R 400.8158 Incident, accident, injury, illness, death, fire reporting. (2) A licensee, licensee designee, or program director shall report to the child's parent and the department, directly or via phone, fax, or email, within 24 hours of the occurrence of any of the following: (d) The center is evacuated for any reason.

On 11/18/2022, licensee designee Diani Dagle reported there was a fire at Bethune Elementary on 11/02/2022 but childcare staff members did not report it to her. Mrs. Dagle notified the department about the incident via email and followed it up with a written report. According to Mrs. Dagle, a childcare staff member (CCSM) was discussing another issue and mentioned "There was a fire in the building." The fire was contained in a bathroom stall located on the second floor away from approved childcare space. No children or staff were injured but the building was evacuated for approximately 5-10 minutes. Mrs. Dagle has trained CCSMs to report all incidents and accidents directly to her or someone within the 2 PreK administrative team. The most recent training took place in August of 2022 during a scheduled professional development session. On 11/28/2022, I conducted an unannounced investigation. Childcare staff members (CCSMs) Shelly Lyons, Jameica Aaron, Lakidia Crawley and Ebony Crawford do not remember being instructed to report any incidents not involving childcare directly to Mrs. Dagle. It's assumed the school principal communicates accidents or incidents to management. According to Ms. Lyons, she recalls being asked to document classroom related repairs or custodial needs in a binder located in the school's main office. She was not aware she could report anything directly to the department. Licensee Amanda Matteo stated all staff have been trained to report issues. She and her team will continue to train them on reporting incidents and develop a clear and concise protocol.
