The licensee did not provide accurate and truthful information to the department when she denied Child A left the basement of the home and went outside unsupervised and when Child A hair I was cut by Child B while in her care.

On 11/8/2021, I interviewed complainant. On 11/2/2021 and 11/3/2021, Child A (1-year-old male) hair was cut. Licensee, Lesa Yenshaw, asked her granddaughter if she cut Child A's hair. Ms. Yenshaw is not watching the children. Age-appropriate scissors could not have been used to cut his hair. Complainant also said two months ago Child A was found outside with the neighbor. He was not wearing socks or shoes. Ms. Yenshaw didn't know he was gone. On 11/08/2021, I conducted a telephone interview with Child A's Mother and again on 11/16/2021. On 9/16/2021, Child A's Mother pulled up to Ms. Yenshaw child care home. Child A was being held by a stranger in her front yard. She asked the stranger who she was and why was she holding her child? The stranger saw Child A standing in the yard with no adult. Child A was not wearing a jacket or shoes; white socks only, that were turned black from walking outside. Child A's Mother knocked on the door and Ms. Yenshaw was surprised to see Child A outside. Ms. Yenshaw asked how he got outside. She apologized and posted a sign on the door for parents to lock the door behind them. Child A's Mother said Child A hair was cut twice while at the child care family home. She noticed it when she took him home and started to take his braids down. His hair unraveled in her hands. She called Ms. Yenshaw to tell her Child A's hair was cut. Licensee, Lesa Yenshaw, asked her granddaughter if she cut Child A's hair. The phone call ended, and communication continued by text. In the text, Ms. Yenshaw stated that Child B finally admitted to cutting Child A's hair. Child B cut his hair while playing in the block area. Child A's Mother provided the text messages to me as proof of the conversation between her and Ms. Yenshaw. Child A's Mother re- braided Child A's hair and he returned on Wednesday. Once Child A was home, Child A's Mother noticed his hair looked cut again. She took his braid down and more hair fell into her hand. She called Ms. Yenshaw a second time informing her Child A's hair was cut again. She began defending Child B, claiming she was not home from school or present at the same time Child A was. Child B could not have cut Child A's hair. Child A's Mother decided this was the last incident and pulled Child A from the child care home. Ms. Yenshaw is not watching the children. On 11/8/2021, I conducted an unannounced inspection at the child care family home. I interviewed licensee, Lesa Yenshaw. Child A's Mother contacted Ms. Yenshaw on 11/2/2021 about Child A's hair being cut. She asked her granddaughter, Child B (five-year-old female). Child B admitted to cutting Child A's hair and then told her mother that she did not cut his hair even though she said she did. Child A's Mother called again on 11/3/2021 and said Child A's hair was cut again. Ms. Yenshaw said that it was not true because Child B did not arrive until after 4:00 p.m. Child A was gone by 3:15 p.m. Child A was not there when Child B 3 arrived from school. Ms. Yenshaw was with the children and never left them alone. The children go upstairs with her when she is preparing lunch. She does not leave the children alone. She asked Child B if she cut Child A's hair because the other children could not reach the scissors on the shelf. Ms. Yenshaw requested a meeting with Child A's Mother and then told her the child care home is not the right fit for her. Ms. Yenshaw denied Child A left the basement of the home unsupervised. Child A was outside with her and the other children. He saw his mother driving down the street and he started running towards her. The neighbor saw him and picked him up before he ran in the street. She later called and asked if I could return to the home where she admitted to not being truthful. Child A's Mother was telling the truth. Child A went outside without her knowing. Ms. Yenshaw admitted Child A was found outside of the child care home by the neighbor and Child A's Mother. This is the first time something like this has
