    On 7/30/2025, during an unannounced investigation at the facility, staff interviews confirmed that a basketball hoop was unweighted and lying on the ground when children went outside on 7/24/2025. CCSM 1 attempted to remove the basketball hoop during outside play time and lost balance resulting in Child A’s injury. The outdoor equipment was not maintained in a safe condition and was not inspected before children went outdoors.
