On June 08, 2023, I conducted a telephone interview with Child A's Mother. She said the allegations were true and Child A was attending A and W Day Care Center for approximately two years. Child A's Father was supposed to pick her up from school on June 05, 2023 but he failed to do so. When she arrived at the center to 2 pick up Child A CCSM1 was rude to her and cursed at her for being late. Child A's pamper was full of "poop" and she had a scratch on her head. She could tell the "poop" had been sitting for a while because of its texture and it was not moist. She did not see the scratch on Child A's face until she got home. The next day, Child A's fictive grandmother, Witness went to the center to inquire about Child A being scratched. She also went there to obtain an incident report. CCSM1 refused to give her an incident report or talk to Witness. He told her to get out and don't come back. She thought the CCSM's behavior was odd because he always spoke to Witness, and she had never been late before. Child A's Mother said Child A does not attend the facility anymore and she was never given an incident report. Prior to ending the conversation Child A's Mother provided contact information for Witness; however, she did not provide any information for Child A's Father. On August 28, 2023, an unannounced onsite inspection was conducted at A and W Day care Center. CCSM 1 and program director, Ameera Macki were interviewed. Mrs. Macki was interviewed first, she said she was not present during the alleged incident. The only staff left was CCSM1 because it was past business hours and Child A's Mother was late for pick up. The center closes at 5:30pm and it is her understanding that Child A did not get picked up until after 6:00pm. She said Child A has picked up late a few times and they had spoken to Child A's Mother about being on time. Prior to getting off work, Child A did not have a scratch on her face and her diaper was changed. CCSM1 was interviewed and said the allegations were not true. He said no one showed up to get Child A from the center. He tried calling the mother and she did not answer. He called all the telephone numbers on Child A's information card, and no one answered. Child A's Mother arrived at approximately 6:10 pm and he gave the baby to her, because Child A was sleeping in his arms. When Child A's Mother arrived, Child A's Father arrived at the same time, but they were in separate cars. Child A's Father did not exit his vehicle. He said that Child A "did not poop and was not wet" to his knowledge. Child A's diaper was changed prior to her scheduled pick-up time. He is unsure if Child A was wet when she was picked up because he did not check her diaper again prior to her leaving his care. The next day, came to the center and accused him of calling Child A _ baby". She also said there were scratches on Child A's face. He said he did not call Child A "a baby" and there were no scratches on her face when she left his care. He did tell Witness not to come back to the center because Child A's Mother was late for pick up on multiple occasion and often disregarded the center's business hours. Notably, CCSM 1 said Child A's Father's contact information was not on Child A's Child Information Card. On August 21, 2023, I called Child A's Mother on the telephone for some follow-up questions; however, she said she could not talk and that she would call me back. On 06/15/2021 I interviewed the Child A's mother over the phone. She said when she or Child A's father has asked about the caregivers about marks or injuries they have seen on Child A they were told different stories when really it sounded as if the caregivers did not know what happened. When they questioned appropriate care and supervision the center increased their fee in retaliation. On 06/16/2021 I conducted an onsite inspection. I interviewed the owner and program director, Ameera Macki. She stated that the asking the parent for payment 2 had nothing to do with the allegation that Child A had a scratch on him. All the of childcare children except for one family at the center are enrolled through GRSP. The program covers the cost of childcare once a family is approved. The center was providing care for Child A at no cost anticipating GSRP approval. The center had only recently began asking for payments from Child A's family because he was denied through GSRP and would have to pay for childcare. A letter dated 06/09/2021 was given to the parents notifying them of the cost of childcare. A receipt was also observed showing a payment on 06/14/2021 for a week of childcare. While at the center I observed written notes dated 06/04/2021 from caretaker, Saffa Elahmed and Yamama Yousseff. The notes indicated that when Child A's mother was picking him up from the center, she noticed a mark the size of a "lentil seed" on Child A's arm. She was told he may have scratched himself because he did not have on his gloves. Child A's mother contacted Child A's father who then contacted the office. He was very upset and spoke of calling the police. I viewed three pictures of Child A. He had a long linear mark on his neck and a rash on his cheeks from another occasion. Ms. Macki showed me a text message to Child A's mother asking about the marks and Child A's mother responded saying Child A scratched himself. Child A is known to scratch himself due to eczema. Mrs. Macki denied any concerns about the caregiver's ability to provide appropriate care and supervision. Both caregivers in the preschool classroom are fingerprinted and have a signed child abuse and neglect statements on file. While at the center I interviewed lead caregiver, Yamama Yousseff and assistance, Safa Elahmed. Mrs. Yousseff is Child A's primary caregiver. She said Child A had documented eczema and his mother did not regularly cut his fingernails. Child A's mother has started to send him with gloves for his hands. She showed me a written note dated 06/08/2021 about no gloves being sent so she used a pair of socks. The note indicated that this was done so "there is no risks of scratch himself." Mrs. Elahmed denied any knowledge of any marks or injuries caused by any childcare staff member. Neither childcare staff members has concerns with lack of training or inabilities to meet the needs of the childcare children. While at the center, I interviewed Child B's, C's, and D's mother. Child B and C no longer attend the center. She was dropping Child D off for the day. She denied any concerns with supervision or appropriate care. She said the center has always provided quality care of her children. She said all her children learned a lot while attending the childcare center. On 07/07/2021 I interviewed Child E's (7-month-old female) mother, and Child F's (11-month-old male) mother over the phone. Neither parent had any concerns about the services being provided by the center On 07/07/2021 an exit conference was held over the phone with Mrs. Macki. I told her the investigation would be closed as there were no violations found. 3
