On 8/9/2022, I conducted an onsite investigation at the child care center. I interviewed program director, Samah Abdulaziz. She said Child A (2-year- old male) climbed a dollhouse in the young preschool room onto the windowsill. She was not in the room when it happened. She was called into the room by the Child Care Staff Member (CCSM) who was working with Child A. The CCSM who was working in the classroom at that time is no longer employed by the center. She asked CCSMs in the adjoining room to have Ms. Abdulaziz come to the room. Child A was taken down as soon as he climbed by his CCSM, and the dollhouse was removed from the classroom immediately by Ms. Abdulaziz. On 8/9/2022, I interviewed child care staff member CCSM, Madelyn Cayton. She works in the toddler room and said no children have climbed the windowsill in her classroom. They cannot reach it. On 8/9/2022, I interviewed CCSM, Jade Rose-Griffin. She works in the toddler room. There is no dollhouse in the classroom. She is not aware of a child climbing onto the windowsill. On 9/29/2022, I interviewed CCSM, Mona Chami. She said no one can reach the windowsills in her classroom. They only have the small dollhouses. The complainant did not leave a phone number to contact. I emailed the complainant requesting an interview, but to date I have not heard from the complainant.

On 8/9/2022, Ms. Abdulaziz stated that children's diapers are changed immediately when soiled. Diaper changes are documented in the Procare app. If there is a "blowout", which is where the bowel movement exits the diaper and messes the clothes, they are changed immediately. On 8/9/2022, Ms. Cayton stated that children are changed every two hours or when they smell a dirty diaper. 3 On 8/9/2022, Ms. Rose-Griffin stated children are changed every two hours unless they are wet within those two hours. Diapers are not kept on children especially if it is a "blowout". They are changed and parents are notified on the Procare app about the accident. On 8/9/2022, I interviewed Child B's (1-year-old, female) Mother. This has never happened to Child B. If it had she would have complained herself if Child B was not changed after having a "blowout". On 8/9/2022, I interviewed Child C's (1-year-old, male) Mother. She has no concerns regarding diaper changes. On 9/29/2022, Ms. Chami stated the children are changed every two hours or soon as they smell the diaper. The diaper changes are documented on Procare. Ms. Abdulaziz showed me the Procare app and how diapers are documented and shared with the parents.

On 8/9/2022, Ms. Abdulaziz stated that child care staff member (CCSM), Mona Chami, did not speak to the children this way. Previous employee Samara Boyles made this up after she was fired for using inappropriate words in the classroom and leaving her classroom. Ms. Chami last day was 6/20/2022, and she worked as the lead from May to June. On 8/9/2022, Ms. Cayton has never heard Ms. Chami use those words or speak that way toward children. She has never heard her call children ugly or yell at them. On 8/9/2022, Ms. Rose-Griffin has never heard Ms. Chami use offensive language. She has never heard her yell or tell them they are ugly. On 8/9/2022, Child B's Mother has never heard this language being used at drop-off 4 or during pick-up. On 8/9/2022, Child C's Mother has never heard anyone say anything like this in the center. On 9/29/2022, Ms. Chami stated that she has never spoken this way to the children. She has never heard Ms. Boyles call the children ugly.

On 8/9/2022, Ms. Abdulaziz stated that an accident report is written out on paper if there is a physical mark left on a child. The Procare app is used to notify parents of injuries that details the accident. The report also goes to the parent email on file if they do not check the Procare app. On 8/9/2022, Ms. Cayton stated parents are notified through the Procare app if a child is injured in addition to a written accident report if a child is hurt. On 8/9/2022, Ms. Rose-Griffin stated that parents are notified each time they are injured through the Procare app and incident report form. On 9/29/2022, Ms. Chami stated that the program director is notified first when a child is injured, and the parent is notified on the Procare app. I was able to view a copy of the plan in writing which is part of the plan and procedures provided to the parents and also part of the Procare app.

On 8/9/2022, Ms. Abdulaziz stated that the children's blankets were in their cubbies, but not in bags. During my inspection, I observed children's bedding to be stacked in bags and others not to be in bags. The bedding that was not in bags were touching each other inside plastic storage containers.

On 02/09/2022, I com leted a face-to-face interview with Child A", anMi A's Mother. Child s Mother stated that while jumping in the bounce house they have in their home, Child A requested that she throw in. Child A said Suekani Alzarquany does it while in school and pulls ■ ants own. She asked Child A how, then Child A told her Ms. Alzarquan inches Child A's Mother asked Child A to show her, and Child A placed hands on . Child A gets ■ body parts mixed up but was able to point to the area. Child A's Mother then texts the director to inform her of the allegations. Child A's Mother said she has requested that Child A sometimes skip nap due to 11 waking up late. She also has a newborn and doesn't want Child A too well rested. hild A has been at the center for about a year and Child A's Mother does not have any other concerns about the care or safety of her child. She said she thinks it is a great center but, she felt that she shouldn't discredit what her child told her. She will continue to send Child A to the facility. On 02/09/2022, I completed an unannounced onsite inspection at Smart Start Learning Center. We interviewed licensee designee, Ms. Abdulaziz and child care staff members, Ms. Albadri, Ms. Alhaidar, and Ms. Bazzi. Ms. Abdulaziz stated that Child A's Mother sent her a long text message on 2/2/2022 stating what was provided in her interview. She said that this occurred during nap time when taken to the gym by Ms. Alzarquany. After this was reported to her, she ffiirree ootthh CCSMs in Child A's classroom, Ms. Alzarquany and Liqa Alhathrawi. any further information. Ms. Abdulaziz feels that once an allegation like that had been made it was grounds for immediate termination. She has never had an allegation like this made against any of her staff members. Ms. Alhathrawi is Child A's teacher. Her daughter is Ms. Alzarquany. Ms. Alzarquany works as a floater in the center and sometimes helps in Ms. Alhathrawi's classroom. Ms. Alzarquany 3 typically works with the "difficult kids" and helps to soothe or settle them when they were having a rough time. Ms. Abdulaziz said staff are sometimes alone with one or two children in the gym area during nap time. She has cameras throughout the center, but not in the gym. Ms. Abdulaziz said she is going to add cameras to the gym and other areas of the building that staff use for individual time with the children. Ms. Albadri is a child care staff member for the 3-year-old classroom. She stated that the center's nap routine consisted of kids laying on their mats and resting in the classroom. She denied that she has ever sent children to the gym who are not resting. Ms. Alzarquany was a floater who has helped in her room. Ms. Albadri has never had any concerns about her interactions with children. She has never had Ms. Alzarquany take any of her kids to the gym. The gym is only used during approved times by each classroom, the classroom goes down all together. Ms. Alhaidar is a child care staff member for the 3 to 4-year-old classroom. She described a similar nap time routine as Ms. Albadri. She said some kids do require that someone sits next to them and rub their backs to get them to sleep or they may read a book instead of napping. She said some staff members have the floaters take children to the gym to calm them or let them get their energy out, but she never sends kids from her room out. Ms. Alzarquany has floated and assisted in her classroom, but she has never had any concerns about her with children. Ms. Bazzi is a floater who works in classrooms as needed. She described the same nap time routine as the others. She said the older kids (3 and 4-year-old) classrooms tend to nap easily, and their teachers allow them to either color or read a book when they don't nap. The younger kids, however, tend to have a harder time resting and when they do, floaters take them to the gym to play. She has floated in Ms. Alhathrawi classroom but never during rest time.
