    On 07/27/2023, I contacted Child A's Mother. She stated that on 07/05/2023 Child A fell off the playground. She got two stitches on her chin. Child A's Mother stated she has concerns about appropriate care and supervision of the children while they are outside on the playground. She stated that she believes that the staff members were sitting down while their children were playing on the playground rather than interacting and monitoring children. She also mentioned a past incident where her child came home with a bruise on her leg, and no one could tell her anything about it. This incident happened when her child was in a different classroom. On 07/28/2023, I conduced an unannounced inspection at the Childtime Learning Center facility. While onsite I spoke with Program Director Jessica Ellis, CCSM 2 and CCSM 3. I called and spoke to CCSM 1 over the phone while at the center due to her being off that day. Ms. Ellis stated that she was not present at the time of the incident as she left around 4:00 p.m. that day. She was also not made aware of the incident until the following week. She was later informed that CCSM 1 was out on the playground while CCSM 2 brought a child inside to be changed since her parents was there to pick her up. CCSM 1 was standing up against the wall so that she could monitor both sides of the climber. She was told that CCSM 1 saw that Child A was starting to slip but she could not get to her in time. She immediately brought Child A to the door and CCSM 2 grabbed her and took her to CCSM 3. CCSM 3 was in charge while Ms. Ellis was out of the building. As to the past incident that mom is referring to, she has already spoke to staff members about making sure to fill out incident reports. It was shared that Child A has no communication skills at this time and does not speak. She often wonders around the classroom and tries to climb on the shelving. Staff members always intervene when they see her climbing, but they think she could have gotten the bruise from climbing up on a shelf. She was not aware that mom still had concerns about that incident after talking to staff members. While onsite I interviewed CCSM 3. She stated that during the incident she was up in the office. CCSM 2 brought Child A to her and stated that Child A fell. She had lots of blood and so she started to clean the area. Once she got the blood to stop, she saw the gash under her child. She put a cold press and ice on it and applied a bandage. She spoke to CCSM 1 and CCSM 2 to get more details of what happened before she called Child A's Mother. Child A's Mother came to pick up Child A. Child A's Mother asked to see the playground, so she took her out to the playground and provided her a copy of the incident report. At that time Child A's Mother said she was unsure if she would be taking Child A to the doctors. CCSM 3 advised her to take her. She did not find out that Child A's Mother took Child A to the doctors until 7/10. She also stated that she did not know that this incident needed to be reported to 3 licensing. She did not inform Ms. Ellis of the incident either. When Child A's Mother picked her up, she did say she had some concerns about supervision. She also mentioned a previous injury but since there was no other incident reports on file, she was unsure about what happened. I spoke to CCSM 2, who is the lead teacher in the classroom. She stated that Child A was standing on the top of the play structure but was facing inward. She was dangling her feet over the edge and as she walked past CCSM 1 she told her, "Watch Child A, don't let her fall" as she was taking another child inside to go home. As soon as she was walking back outside, CCSM 1 was at the door holding Child A. She opened the door and said Child A fell. She immediately took her to CCSM 3 and then went back to the classroom to clean up all the blood. She also stated that while Child A was waiting to be picked up, she was acting normal and wanted to play. I spoke to CCSM 1

    On 07/27/2023, Child A's Mother expressed concerns about the playground being unsafe for 2 year olds. During the centers renewal inspection on 1/2 07/05/2023, I informed Ms. Ellis that we were missing a copy of the center's playground inspection in our files. She could not find the original inspection onsite and had reached out to the area managers to see if they had a copy. Because no one could find a copy of the original inspection, plans were made to get the playground reinspected. The playground was reinspected on 08/01/2023. The toddler playground was found to be appropriate for all children ages 2 and older. On 07/28/2023, when I spoke to Child C's Mother, Child D's Mother and Child E's Mother, no one mentioned any concerns about the playground being unsafe for their toddler.

    On 08/24/2022, I spoke to Child A and B's Mother regarding the allegations. Child A and Child B started attending the center around August 8, 2022. Child A was in the preschool room and Child B was in the summer camp program. Child A's Mother noticed the rash on Child A's hand within the first week. The rash began to blister. It is unknown what caused the rash. Child A's Mother stated that she observed the child care staff members (CCSM) in Child A's room dump hot oatmeal down the sink where the children washed their hands. Child A's Mother was unsure if the blister came from the hot oatmeal or from the hot sand box. Child A's Mother did not speak with the program director at the center regarding the injury. However, she called the center's corporate office and discussed her concerns. She was advised that the information provided would be confidential and she was told that the center would not know who called. Child A and B's Mother stated that she was not notified of the injury to Child A's hand and an incident report was not provided. She stated that she did not have a picture of the initial blister that was on Child A's hand. Child A and B's Mother stated that she used some skin scrub, and she was able to burst the blister. On 08/25/2022, Child A's Mother, sent a current picture of Child A's hand. There was a small, healed circle above Child A's knuckle on her right hand. On 08/24/2022, I completed an unannounced inspection at Childtime Learning Center. I spoke to Roslyn Williams, assistant program director, and child care staff member (CCSM) Raven Kyles. The entire center was observed. The center was clean and there were no concerns regarding the safety of the building. In the infant room, there were two infants, ages 6 weeks to 12 months, with one CCSM. In the toddler room, there were four children, ages 12 to 16 months, with one CCSM. In 3 the two's-1 room, there were four children, ages 18 months to 2 years with one CCSM. In the two's- 2 room, there were six children, ages 2 years, with two CCSMs. In the early preschool, there were 12 children, ages 2 to 3 years, with 1/2 two CCSMs. In the preschool room, there were 9 children, ages 3 to 4 years, with 1/2 one CCSM. In the PreK room, there were 14 children, ages 4 to 5 years with two CCSMs. The following information was provided for review: child information cards and menu. There were no incident reports provided. According to Ms. Williams, Child A was enrolled in the preschool class and Child B was enrolled in the summer camp program at the center. Ms. Williams was aware of the allegation. Child A and B's Mother stated that Child A had received a blister from the germs at the child care center. They were contacted by management on 08/18/2022 and they were made aware of the parent complaint by Child A and B's Mother. The complaint was regarding Child A having a rash. They were unsure where the rash was at on Child A, and they told the CCSMs to make sure they clean the toilets after the children used the bathroom. Ms. Williams stated that the center does serve oatmeal for breakfast. During breakfast, a bucket is provided to each room for leftover food. The bucket is placed into the classroom sink. Any food the children did not eat is placed in the bucket. Usually, the classroom CCSMs clean up after breakfast is over or once they get the children settled. The food should not be left in the classroom all day. Ms. Williams stated that she did not believe that oatmeal from breakfast was left at the end of the day. I reviewed the center's menu for the week of 08/08/2022, and 08/15/2022. Oatmeal was served on Friday each week with apples or applesauce and milk. The child care center has a sandbox that is built into the ground in the outdoor play area. The sandbox is covered by a blue sunshade. The children go outside twice a day, weather permitting. At the end of the day, the sandbox is covered with a tarp. The week of August 8 through 13, I checked the weather. The temperature

    On 08/24/2022, I spoke to Child A and B's Mother regarding the allegations. She stated on 08/23/2022, Child A and B's Mother picked up Child A, around 4:00 pm. Child A and another child was sitting outside with CCSM, whom she did not know. Child A and B's Mother approached the CCSM, and she immediately began to apologize for sand being Child A's hair. The CCSM reported that she had taken Child A's pony tail out and tried to get as much sand as she could out of Child A's hair, wiping her head with a baby wipe. The CCSM also stated that the program director, Queayna Battle continues to open the sand pit even though the children do not do well with it. Child A and B's Mother observed a bald spot in Child A's head which she believed it was from the CCSM attempting to get the sand out of Child A's hair. The CCSM put Child A's hair back into a ponytail, but her edges were messed up and her hair bows were missing. Child A's face, body, and clothes were dirty. Child A's Mother saw the other children in the classroom being cleaned up with baby wipes. On 08/24/2022, Child A and B's Mother provided a video showing sand in the bottom of the bathtub. She stated that she gave Child A a bath and the sand that was left in the tub came from her daughter's body and hair after being at the child care center. In the video, the bath water was drained and there was a small amount of sand observed in the bottom of the tub. The recording is on file with the department. Child A and B's Mother went and spoke to Ms. Battle about her concerns, and Ms. Battle acted as if she did not care. She stated that the children get to pick what they want to do, and that she will continue to open the sand pit. The conversation between Child A and B's Mother and Ms. Battle escalated and became aggressive. Child A and B's Mother recorded the incident on her phone. Ms. Battle told Child A and B's Mother that she knew that she reported the concerns about the condition of the facility, and Child A rash even though the information was supposed to be confidential. Ms. Battle came from behind her desk and knocked her phone out of her hand. Her phone flew into the wall and the screen was shattered. According to Child A and B's Mother, Ms. Battle lunged toward Child A and B's Mother as if she was going to hit her in front of her children. Child A and B's Mother had to step in front of Child A and B to prevent them from being hit. Ms. Battle called the police, and a report was filed regarding the assault. On 08/24/2022, Child A and B's Mother provided a video of the incident that took place in Ms. Battle office at the child care center. I reviewed the video footage; it is difficult to see the footage because the camera is unsteady. The video shows Ms. Battle getting up from her desk, while on her desk phone. She has her hand up and she walks around the desk toward Child A and B's Mother. Ms. Battle is trying to block the parent from recording her. Child A and B's Mother yells, "You touch me! She touch me". Ms. Battle asked Child A and B's Mother to leave the office several times and Child A and B's Mother refused. Child A and B's Mother's phone never drops to the floor. You can hear her in the background yelling at Ms. Battle. Child A and Child B can be seen standing next to their mother. Ms. Battle is on the phone with the police reporting the incident. 7 On 08/24/2022, I spoke to Ms. Williams regarding the allegations. She stated that there is a sandbox in the playground area. Child A and B's Mother did not want Child A to play in the sandbox. She stated that Child A got a ringworm from her previous school from playing in the sand. Child A and B's Mother asked for the sand box to be covered. Ms. Battle told Child A's Mother they could not cover the sandbox. They would redirect and exhibit correct behavior for the children to use while playing in the sand box. Ms. Battle spoke to the CCSMs and asked them to watch the children to make sure they were not throwing sand and redire

    On 08/24/2022, I spoke to Child A and B's Mother regarding the allegations. She stated that Child B began attending the summer program for older children at the beginning of August 2022. During enrollment, Child B's ADHD and socialization issues were discussed. She provided the center with Child B's health appraisal, and it listed his medication. Child A's and B's Mother was told that after the summer program ended, Child B would transition to after school program at the center. On 08/22/2022, Child B and Child C got into an argument while at the child care center. According to Child A and B's Mother, the argument started because Child C was yelling at Child B. Ms. Battle approached the children to talk to them, Child B would not talk because he was upset and has ADHD. Ms. Battle spoke to Child A and B's Mother and stated that Child B could not return to the summer program due to the altercation and Child B was not willing to respond when she spoke to him. She also stated that the summer program was ending. Ms. Battle was asked about Child B attending the afterschool program at the child care, and Ms. Battle stated that they do not allow older children to attend the center. On 08/23/2022, Child A's Mother saw older children at the child care center who attended the summer program with Child B, despite Child A and B's Mother being told the summer program was over. According to Ms. Williams, Child B had been attending the child care center for the past two weeks. He was enrolled into the summer camp program during the last two or three weeks. This information was provided to Child A and B's Mother at enrollment. Raven Kyles and Kirsten Marshall were the CCSM that was working with summer camp children. On Child B's first day, he threw a toy at another child's face and the child's nose started to bleed. There was an incident report written regarding the incident. Child A had some behavioral issues and at times he would tease and call the other children names. Ms. Williams stated that she was not present on 08/22/2022 when Child B and Child C got into the altercation. Ms. Williams was told that Child B and Child C were arguing back and forth. Child B was refusing redirection and was calling Child C out of his name. Child B was unable to calm down and was taken to talk to Ms. Battle. Child A and B's Mother was called at 10 4:00 p.m., 08/22/2022 was the last day of the summer program. Ms. Williams stated that she informed Child A and B's Mother at enrollment that there was not a school age program for Child B to attend. According to Ms. Kyles, she is a floater and has been working with the summer camp children for the past two weeks. Ms. Kyles noticed that Child B did does not like adult authority and has no boundaries. Child B always deny responsibility for his actions and the other children always told on him, saying, "Yes, he did". Ms. Kyles tried to talk to Child B, but he would not listen. Ms. Kyles stated that there were some issues with Child B. There was an incident where he caused another child's nose to bleed. Child A and B's Mother was informed of the incident, and she talked to Child B in the classroom. Ms. Kyles was not there when the incident occurred. On 08/22/2022, Ms. Kyle stated that Child B was aggressive with the other children and being a bully. He took the ball from the girls, playing keep away and he would not give them back the ball. Then he started name calling (i.e., cry baby) with the other children. Child C (9-year-old, male) has autism and at times he talks to himself. Child B began to pick on Child C. He called Child C a big fat cry baby. The two boys were going back and forth. Ms. Kyles decided to separate the boys. Child B continue to yell and argue with Child C. Child B was taken to the office, and he called Child C a "fag" after he sat down in the office. Ms. Battle tried to talk to Child B, but he would not talk to her.     On 06/1712022, the investigation was initiated by telephone. Child care licensing consultant Selika Johnson contacted Child A's Mother. He is doing well but will not return to the center. He has not attended the facility since early April of this year and had been in care for approximately one month. According to Child As Mother, he was having difficulty sleep for several weeks prior to leaving the facility. On 06/14/2022, he informed her of a dream involving child care staff member (CCSM) Demetra Hawkins taking a toy from him and hitting him with an open hand on the arm which made him cry. There were never any bruises or signs of injury when he returned home from the facility. He is now having repeated nightmares about the incident. Child A's Mother did not have any additional problems or concerns regarding the center. On 06/20/2022, l called Child A's Mother, but I have not received a return call as of this entry. 06/21/2022, l called and discussed the allegation with On 06/21/2022, l conducted an onsite investigation and discussed the complaint with program director Queayna Battle and CCSM Demetra Hawkins. Ms. Battle and Ms. Hawkins denied the allegation. Ms. BattleFes a es Child A's Mother never mentioned any hitting incident to her and she was `'surprised" to hear it. Physical discipline is not permitted at the center and all staff are aware of the redirect policy. Ms. Hawkins states she has no idea why the complaint was made. She has never hit Child A or any child at the facility. Child B only attended the facility briefly and was absent quite often. Staff are encouraged to redirect children Child A did not exhibit any behavioral issues and she does not recall having to use the redirection method with him. On 07/07/2022, CCSM Christina Terrazas was interviewed. Ms. Terrazas is the assistant teacher in the room with Ms. Hawkins. She denied having seen any hitting take place and states Child A was rarely in class for the short time he was enrolled.

    On 06/09/2022, I received a complaint alleging the facility allows infants clothing to be wet with slob from teething. Bibs are not allowed to be worn. No additional information was provided to help with the investigation. On 06/13/2022 and 06/21/2022, I conducted unannounced inspections at the facility. I inspected the infant room and discussed the allegation with child care staff members (CCSMs) 1, 2 and the program director Queyna Battle. On both days, the room was within ratio with 5-6 children and 2 staff members. I did observe two children to be wearing bibs and they were not eating. CCSMs stated the children were teething and that bibs are changed throughout the day. No parents had complained to them about the issue. According to Ms. Battle, staff work really hard in the infant room, and they would never deny the use of a bib to any child. She was not aware a parent was having an issue. CCSM 1 and CCSM 2 stated the allegations are false. They work in the infant room and has never allowed any child to be soaked with slob or anything else. All teething children wear bibs throughout the day and are changed when needed. On 07/06/2022 I interviewed CCSM 3. She denied the allegation and states she always provide bibs for children and especially those that are teething. She is frustrated she and other CCSMs are working hard, and people keep making up stories or calling in petty complaints. On 06/13/2022, I discussed the complaint with Child Care Licensing Consultant Erica Hogan. She had recently been at the facility working on a complaint regarding allegations in the infant room. She did not witness any issues with care and supervision. On 05/05/2022 and 06/09/2022, Mrs. Hogan contacted several parents of children currently enrolled in the infant classroom. See Special Investigation Report (SIR) 2022D0937012 dated 06/30/2022. Because infants cannot report mistreatment, Mrs. Hogan asked each parent how pleased they were with the infant staff and if they had any complaints or concerns. Each parent spoke highly of the CCSMs and the facility. They had no complaints about anything. The complainant was not credible. The name and contact information provided was not accurate. 3

    On Monday, 05/02/2022 I arrived onsite and interviewed program director, Queayna Battle regarding the allegations. Ms. Battle said she was aware that a child's shoes were on the wrong feet and one sock was on halfway on 04/27/2022, however, she did not know who the child was or had knowledge of any specific details. On 05/02/2022, I interviewed the two child care staff members who were present in the preschool classroom on 04/27/2022. Child Care Staff member 1, (CCSM1) recalled Child A having on shoes that were on the wrong feet and having on one sock halfway on 4/27/2022. CCSM 1 said Child A's shoes came off and could have accidentally been put on incorrectly. CCSM 1 said Child A took off his shoes and had on one sock moments before his mother arrived. CCSM 1 said that Child A was not being ignored. I interviewed Child Care Staff Member 2, (CCSM2) who was in the preschool classroom on 04/27/2022. I asked CCSM 2 to recall the day Child A was present with his shoes on the wrong feet and one sock halfway on. CCSM 2 does not recall any details surrounding the incident of Child A being present with his shoes on the wrong feet. CCSM 2 said she possibly could have been attending to another child. After interviewing CCSM 1 and CCSM 2, I went in Ms. Battle's office to complete my interviewing. While in Ms. Battle's office, I asked for the child information records of random parents. Ms. Battle gave me the records and I retrieved contact information of parents who had children in the center. On 05/02/2022, I interviewed Child A's (2 year-old male) Mother. Child A's 1/2 - Mother said on 04/27/2022 when she arrived to pick up her son, she observed Child A with his shoes on the wrong feet and one sock halfway on. Child A's Mother said she stood looking in the window of the preschool classroom and saw CCSM 1 standing with her back to her son. Child A's Mother said she stood at the window for about 25 seconds before she was noticed and then went inside. On 06/07/2022, I contacted three random parents to gather information regarding concerns of care and supervision. Each parent indicated that they did not have any concerns of care and supervision with the staff at Childtime Childcare Center. Child B's (2-year-old male) Mother said she has no concerns of care and supervision with the staff. Child C's (2 year-old female) Mother said her child is happy when she leaves her 1/2 - each morning, so she has no reason to have any concerns. Child D's (2 year-old male) Mother said she does not have any concerns of care 1/2 - and supervision with her son's classroom teachers. 4 .. .. ..........

    On 05/02/2022, I arrived onsite and interviewed the program director, Queayna Battle regarding the allegations of the early preschool classroom being out of ratio. I told Ms. Battle that according to the allegations, Child A's Mother observed nine preschoolers with only one childcare staff members on 04/27/2022. I asked Ms. Battle if the center could have been out of ratio. Ms. Battle said, "No. We try not to ever be out of ratio." I went to each classroom and counted the number of children present in care. Below is a chart that shows the number of children to child care staff members who were present in each class while I was onsite on 05/02/2022. I interviewed CCSM 1 regarding the center being out of ratio on 04/27/2022. CCSM 1 said the center was out of ratio on 04/27/2022. CCSM 1 said there were nine preschoolers (30 months to 3 years of age) and two child care staff members present. CCSM 1 said sometimes the center is out of ratio, however, the center tries to fix it. I interviewed CCSM 2 regarding the center being out of ratio on 04/27/2022. CCSM 2 said she did recall the preschool classroom being out of ratio on 04/27/2022. CCSM 2 said the center if often out of ratio, but the center tries to fix it. I interviewed child care staff member 3 (CCSM 3) regarding the center being out of ratio. I interviewed child care staff member 6 (CCSM 6) regarding the center being out of ratio. CCSM 6 said her class is never out of ratio. I interviewed child care staff member 7, (CCSM 7) regarding the center being out of ratio. CCSM 7 said her class is never out of ratio. I interviewed child care staff member 8, (CCSM 8) regarding the center being out of ratio. CCSM 8 said her class is never out of ratio but could not speak for the other classrooms in the center. I went to Ms. Battle's office to discuss the issue of ratio. I informed Ms. Battle that her infant classroom was out of ratio during my observation of the classrooms, having nine infants and two CCSMS. Ms. Battle was not aware the infant classroom was out of ratio. Ms. Battle said she was trying to hire staff so the center would always be in ratio. I requested that Ms. Battle send the attendance records for 04/27/2022. Ms. Battle informed me that I would have the attendance records by the close of business. On 05/02/2022, I interviewed Child A's Mother. Child A's Mother said on 04/27/2022, she looked through the window of the early preschool classroom and counted the children. She saw that they were out of ratio. She counted nine children and two child care staff members. Child A's Mother said being out of ratio has been an ongoing problem since January 2022. A The attendance records I received from Ms. Battle for 04/27/2022 indicated that Child A was picked up at 3:56 PM. According to the attendance records received from Ms. Battle, there were 11 children who were still present after 3:56 pm with two child care staff members. On 05/03/2022, I completed an onsite inspection. The infant classroom was out of ratio. The infant classroom had two child care staff members with nine infants. CCSM 1 was the designated child care staff member in charge. I told her that I counted the infants, and they were out of ratio. Two new hired staff was present for their first day of work.     On 05/02/2022 I interviewed Child A's Father. Child A's Father said he had observed children who were asleep, still strapped in their car seats in the mornings. On 05/03/2022, I conducted an onsite inspection. I arrived at 8:25 a.m. I wanted to speak with Ms. Battle regarding the new allegations of infants remaining in their car seats. While waiting to speak with Ms. Battle, I went to the infant classroom. There were no infants who remained in their car seats. I interviewed CCSM 1 regarding the allegations. CCSM1 said the infants come in, their jackets are removed by either the parents or the child care staff members, and they proceed with the day. I interviewed CCSM 4 regarding the allegations. CCSM 4 said the infants do not remain in their car seats, but they are picked up and the child care staff members hold them or they engage in tummy time. From my arrival until my departure at 9:28am, I did not observe any infants who were left sleeping in their car seats. On 05/05/2022, I contacted three random parents of children who attend Childtime Learning Center to ask if they have observed infants left in their car seats in the mornings. Child E's (1 year old female) Mother said she has not seen any infants left in their 1/2 car seats in the mornings. I contacted Child F's (2-year-old male) Mother. Child F's Mother said she has not observed any infants in their car seats when she drops off her son. She said they are all in their classroom. I contacted Child H's (1-year-old-male) Mother regarding the allegations. Child H's Mother said when she has dropped off her son, she takes off his jacket and gives him to the child care staff member. She said she has never seen any infants left in their seats.

    On 06/08/2022, I received a complaint regarding the allegations. When I called the complainant, he denied calling in the allegations and was unsure why his name and number were provided. On 06/09/2022, I arrived on site to interview the infant teachers. I met with the Assistant Program Director, Rosalyn Williams. I shared with Ms. Williams the allegations regarding the child care staff members pinching the infants. I interviewed CCSM 1. CCSM 1 said the allegations are false. She is in the infant classroom from 7:30-9:00am depending on the day and she has never seen or heard of a child care staff member pinching a baby. I interviewed CCSM 4 regarding the allegations. CCSM 4 said she would never pinch an infant, nor would she allow anyone else to pinch an infant. CCSM 4 said if the infants cry, they cry on their own because they want to eat. CCSM 4 said she treats the infants as if they are her own. I interviewed CCSM 9. She said 06/08/2022 was her first day in the infant classroom. She said she did not think the child care staff members who were in the class would do that. She did not see anything like that while she was in the classroom. I interviewed CCSM 10 who is not a child care staff member in the infant classroom. I asked her if she had ever heard of a child care staff member pinching an infant in the infant classroom. She said she had been there for four months and has never heard of an infant child care staff member pinching or hurting an infant. On 06/09/2022 I received infant child information records from Ms. Battle via email. On 06/09/2022, I contacted three parents who has children in the infant classroom. Because infants cannot report mistreatment, I asked each parent how pleased they were with the infant staff and if they had any concerns of inappropriate care. Each parent spoke highly of the child care staff members. 10

    On 02/17/2022, I interviewed Child A's Mother. She believes the center "let my baby go hungry due to a mistake." Child A (3-month-old, male) was dropped off at approximately 06:45 a.m., with bottles and the formula to prepare them. The receiving staff never said it was a problem. Child A's Mother received a call "almost an hour or so later" stating she needs to come and mix the bottles. Child A's Mother heard him crying and requested they feed him. The caller insisted she had to make them, so she returned to the center. She was not aware they did not mix formula and was surprised to get a call and admits to getting upset. Once she returned she and the staff were calmly talking about the issue and Ms. Bond arrived "with an attitude." She believes someone called her since she arrived and was seemingly angry. They entered her office to talk, and she tried to share her side of what happened but Ms. Bond consistently cut her off and was repeating "Good Morning." Child A's Mother alleges Ms. Bond was behaving so unprofessional that she became upset and uncomfortable in the room alone with her. Once they left the office she repeatedly asked staff to get all of Child A's belongings since they would be leaving and not returning. While she waited, Ms. Bond was being inappropriate and they begin to argue. Ms. Bond then "charged" at her, trying to push her out the building making her drop the formula. On 02/22/2022, I conducted an unannounced investigation at the center. Ms. Bond denied the allegation. She has never charged or attempted to hit a parent at any time and was only trying to protect the children and staff by repeatedly asking Child A's Mother to leave. The day of the incident, she arrived at the facility and was informed of Child A's Mother being angry about the center not preparing a bottle for her baby. Ms. Bond informed her of the bottle policy during enrollment and tried to talk with her about it again. She attempted to deescalate the situation and requested Child A's Mother come into her office and talk. Once in the office, she refused to sit and continued to yell about the staff not preparing a bottle. Ms. Bond ultimately left the office and Child A's Mother followed her yelling and cussing. She was asked to leave due to the inappropriate behavior, but she refused. She continued to yell, use profanity, and walk toward her threatening to hit her. There were three child care staff members (CCSM) present who witnessed the incident, intervened, and called the police. There was no arrest and Child A's Mother left when the police arrived. Child A no longer attends the facility. I requested to review any video from the incident and was referred to the administrative office. On 02/22/2022, I spoke to licensee designee, April Cole. the facility had video recording of the situation but no audio. At the time of my request, the recording was no longer available for review. According to Ms. Cole, she reviewed the footage prior to it being recorded over and it did not show Ms. Bond charging at anyone. She observed Child A's Mother consistently moving toward Ms. Bond who was backing up from her. 3 On 03/11/2022, the staff present during the incident shared the following information: CCSM, Asia Andrews was present and heard yelling in the hallway. She stepped out of the room and observed Child A's Mother yelling, threatening, and getting in Ms. Bond's face. Ms. Bond repeatedly asked her to leave but she refused until the police was called. She then left without further incident. CCSM, Tawanna Lyons states at approximately 8:45 a.m., Child A's Mother walked into the facility and demanded they give her Child A's bottle and formula. Ms. Lyons walked away and called Ms. Bond to tell her what was happening. Ms. Lyons walked back to the area and found Child A's Mother was still yelling and told her to get the rest of Child A's bottles. She left to get the bottles and heard Child A's Mother and Ms. Bond yelling at each other. She got between b

    On 10/06/2021, I completed an unannounced onsite inspection at Childtime Learning Center. I spoke to Georgetta Bond, program director, regarding the allegations. The Toddler 1, Toddler 2 and 2-year-old classrooms were closed. A copy of the health and temperature, attendance sheet and the center notice were received. The arrival process was observed on 10/06/2021. At the entrance, there was hand sanitizer, a thermometer and the health and temperature 2 form. I observed several parents arrived with their children at the center entrance. Masks were worn by the parents and the children. The parents checked the children temperature with the thermometer and then they recorded the child's time of arrival and temperature on the health and temperature form. The parents walked the children to their classroom. According to Ms. Bond, on 09/29/2021, a child care staff member in the Toddler 2 classroom was not feeling well at the end of the day. The child care staff member was tested after work for COVID-19 and she tested positive. Ms. Bond received a text message from the child care staff member at 8:59 p.m. on 09/29/2021 regarding the positive test result. Ms. Bond contact her district manager, April Cole but she was not available. She then contacted Kathleen Conrow, another district manager, and reported that the center had a child care staff member that tested positive for COVID-19. On 09/30/2021, Ms. Conrow contacted Ms. Bond to get more information. It was explained that the child care staff member worked in the Toddler 2 classroom but on 09/29/2021 the Toddler 1, Toddler 2 and the 2-year-old classrooms combined at the end of the day with the child care staff member. Ms. Conrow then contacted the health department to report the positive COVID test and to get guidance on how to proceed. Children were arriving at the child care center at 7:00 a.m. from the Toddler 2 classroom. The health department called Ms. Conrow around 9:00 a.m. that morning and provided the recommendation to close the Toddler 1, Toddler 2 and 2-year-old classroom at the child care center. The health department recommended that the classrooms needed to quarantine for 10 days. A message was sent immediately to all the parents of the children in the Toddler 1, Toddler 2 and 2-year-old classroom through the Bright Wheel application and follow up telephone calls were made. Information was posted on each classroom door regarding COVID-19, providing the parents with name, symptoms and prevention measures. Most of the children were picked up by their parents by 11:00 a.m. According to Ms. Bond, none of the children's parents were upset about having to pick up their child on 09/30/2021. Ms. Bond followed up with some parents after the classrooms were closed to see if any of the children displayed any symptoms. To date, none of the parents have contacted the child care center stating their child have been sick or contracted COVID due to the closure on 09/30/2021. On 10/15/2021, the parental notice regarding COVID- 19 was reviewed. According to the parental notice that was sent through the Bright Wheels application to parents on 09/30/2021: "Alert- Dear Families, We've been made aware that a teacher has been diagnosed with COVID-19. This teacher was last at our school on Wednesday, September 29, 2021. This teacher is a member of our Toddler 1, Toddler 2, and 2-year-old classrooms. While working the Wayne County Health Department, a decision has been made to close these classrooms for the remainder of this week and next week, reopening on October 12, 2021. This required time will allow the school opportunity for a deep clean. We are asking parents to pick up their children as soon as possible. In working with the 3 Health Department and Learning Care Group precautions, we will ensure a professional cleaning of the entire building to be completed before reopening on October 12th, 2021.
